While setting a roof bolt in the roof, the employee was using a drill steel wrench to finish pushing it to the roof. With his hand on the wrench, he attempted to set the bolt. The wrench slipped off the bolt and kicked over smashing his hand between the wrench and the drill steel clamps, resulting in a Laceration which required 5 stitches.

Employee was attempting to set a simplex jack, he was holding the jack with his left hand & had slide the top section of the jack up to the roof. As he was jacking the jack upwards, the top section of the jack released & slide down inside the bottom section of the jack & smashed his left index finger between the top and bottom sections of the jack. Laceration required 9 sutures.

After drilling a hole, the employee placed a roofbolt on the drill head and as he attempted to push the bolt into the hole, the bolt missed the hole & hit the mine roof causing the roofbolt to bow out and smash his left index finger between the bolt and the drill steel clamps resuting in a laceration which required 7 sutures.

Employee was building a metal cross over on the 2nd West Mains belt conveyor at cross cut #8. As he would tack weld the pieces together, he neglected to lower his welding hood and when he started to weld is when he would lower his welding hook. Employee sustained flash burns to both eyes.

Employee was operating a Fletcher double boom roof bolter in the face of the 21st West development section. As he went to drop his canopy, his right hand was placed on the material tray and as the collar on the canopy boom went past the cable tray it smashed his little finger on his right hand thus resulting in the last 1/4 inch of his little finger being cut off.

Employee was operating the taigate drum of the shear in the 17th West Longwall at shield #141 when a bounce occurred. He was struck either by the rubber bounce guard or by a piece of coal in the right shoulder and right side of the airstreams helmet. Employee claims to have been unconscious for a few seconds. Employee finished working his shift.

Employee sustained chemical burns to both eyes, while operating the roof bolter in the #1 Entry face. While drilling holes to install roof bolts, excessive water was gushing out of each of the holes. Employee would try to shove the tubes of resin and roof bolts into the hole, the gushing water would splash up onto his face and into his eyes.

Employee was operating miner in the mains. Water was dripping from the roof and running into his eyes. Later in the shift they became irritated and sore.

Employee had finished drill a hole and had inserted a tube of resin and a roof bolt into the hole. Employee pushed the bolt up into the hole with the drill head and then lowered the drill head and inserted the extension wrench. As he pushed the bolt in, the bolt bent and the wrench pinched his left hand between the clamps. 3 stitches were required

EE was using his right hand to guide a 2 piece sectional drill steel into a drill hole with the aid of the drill head when the steel got stuck in the hole had bowed the steel resulting in his hand being smashed between the steel and the clamps. Resulting in 3 stitches to close the wound.

As the shear was heading towards the tailgate, the headgate drum was grading bottom. EE had placed his left hand on top of the panline to maintain his balance, when a piece of rock measuring 12" by 6" was flipped up from the cutter drum and struck him on top of his left hand thus resulting in a fracured bone on the back of his hand and 2 lacerations which required stitches.

As EE was lowering the tip of shield #7, a small piece of rock rolled off of the shield and struck him on the left hand resulting in the middle finger and the index finger being cut. EE was positioned on shield #6.

There was a bounce on the logwall face at the tailgate at shield 123. EE was operating the tailgate cutter drum and was pinned between the bounce guard and the sheild resulting in ee being bru ise and put on light duty for 3 weeks

EE had drilled a hole with a starter steel. Inserted top piece of his section steel in the hole and was pushing the bottom section up with the drill head. When the two sections met the bottom section bowed out smashing his hand against the drill head.

The ee was transfering rock dust from a mobile tank to a pressurized holding tank. The compressor for the duster was not de-energized and the pressure not bled off, when the cap was unlatched it blew off and struck the ee in the face.

THE EE WAS OPERATING THE CANSETTER WHEN HE WENT TO PARK IT HE HAD HIS HAND ON THE ROOF AND THE CAB HIT A ROOF BOLT. HIS HAND WAS PINCHED BETWEEN THE CAB AND THE ROOF BOLT.

THE ROOF BOLTER WAS BACKING OUT OF THE CUT TO ALLOW THE CONT MINER BACK INTO THE CUT. THE CONT MINER OPERATOR BENT OVER TO KEEP THE BOLTER FROM RUNING OVER ITS CABLE. WHEN HE BENT TO PICK UP THE CABLE HIS FACE STRUCK MATS THAT WERE LOADED ON THE MACHINE.

EMPLOYEE WAS DRILLING A TEN FOOT HOLE IN THE ROOF TO INSTALL CABLE BOLTS. HE WAS USING A SECTIONAL STEEL. WHEN HE COMPLETED DRILLING THE HOLE HE LOWERED THE STEEL OUT OF THE HOLE, THE TOP SECT ION REMAINED IN THE HOLE SO HE REACHED WITH HIS RIGHT HAND TO RETRIEVE IT. THE STEEL RELEASED FROM THE ROOT SUDDENLY AND PINCHED HIS RIGHT INDEX FINGER BETWEEN THE TWO SECTIONS OF THE STEEL.

INDIVIDUAL HAD COMPLETED MINING CROSSCUT AND HAD MADE AN INITIAL CLEAN UP PASS ON THE OPERATOR SIDE OF THE CROSSCUT. INDIVIDUAL PIVOTED MACHINE TO CLEAN OPPOSITE SIDE OF CROSSCUT AND THE RIB B SLOUGHED AND PINNED HIM AGAINST THE RIGHT REAR CORNER OF THE MACHINE.
