Operator & Oiler were in process of moving power cable to maneuver Machine. Oiler hooked up 2" sling around power cable & using 1" sling hooking to fan tail hook. Before Oiler was ready Machine started to swing while Oiler's hand & fingers were still under sling on power cable side. Resulted in four fingers getting injured from the pressure of the sling when Machine swung.

Showing all 22EE was assisting in installing bucket cylinder to linkage. A crane and sling were being used to pull cylinder forward to align. EE noticed a piece of plastic covering bore of cylinder and tried to remove it. This caused the sling to slide up, swinging the cylinder and EE's hand was caught in pinch point.
During re-socketing of 8750-1 dragline hoist rope, Employee was positioning the wedge into socket when the hoist rope loop being pulled by a dozer snapped and hit his right thumb. Employee reported at end of shift. He visited Mine clinic and had minimal bruising/swelling. The next day, a hospital doctor diagnosed a bruised thumb ligaments and tissues. He was excused for 3 days.
Maintenace was being conducted on a dragline when incident occurred. Hoist rope was being changed out. Winch line was attached to hoist rope to remove it. Winch line got hung up on lug, released and struck worker above left ankle causing a fracture.
The dozer left the work area and traveled along the edge of the highwall and came to a stop. When the dozer went into reverse, the ground underneath the left track gave way causing the dozer to fall off the highwall. The dozer landed in upright position at the base of the highwall.
The rubber tire dozer operator claims low back pain after his shift and informed he has a scheduled chiropractor appointment the following day.
The welder was cutting a dragline dump rope. When it was cut loose, the released energy in the twisted rope whipped out hitting the left knee.
A pair of scrapers were excavating top soil using (push-pull) method. The rear scraper rode up onto the front scraper's stringer area, when the front scraper hit a soft a soft pocket of material; jarring the rear scraper operator. He was jarred again as he was trying to remove the bale from the front scraper's hook. He felt pain to his back.
Crew was positioning plate...EE was welding with back to plate...plate slipped & fell puncturing EE behind lt ankle.
EE was using pipe beveling machine wearing work gloves...thumb of right glove got caught in machine pulling EE's thumb into machine...removing nail & left portion of thumb to bone.
EE was preparing to enter base/tub...got his clear safety glasses out & sat them next to manhole...he was wearing tinted glasses...exhaust sucker fan sucked his glasses into the fan...he turned off the fan & reached in to retrieve the glasses...evidently the fan had not stopped. broken fingers & multiple lacerations.
THE INJURED WAS WALKING/OBSERVING ON THE NORTH SIDE OF A CRUSHER WHILE IT WAS BEING CLEANED WITH A HIGH PRESSURE CLEANER WHEN A PROJECTED PIECE OF ICE FLEW OFF FROM THE PRESSURIZED SPRAY HITTING HIS LEFT EYE. INJURED WAS WEARING EYE PROTECTION AT THE TIME INCIDENT MINIMIZING THE SEVERITY OF THE EYE INJURY.
THE WELDER WAS STANDING TO THE SIDE APPROXIMATELY 20' AWAY FROM WHERE A HYDRAULIC HAMMER WAS HITTING ON A ROPE SOCKET WEDGE TO FREE IT, WHEN A PIECE OF METAL SLIVER FLEW OUT AND HIT HIS RIGHT CHEEK.
EE WAS HELPING REMOVE CENTER PIN BOLTS WITH A PNEUMATIC IMPACT WRENCH AND A SWIVEL EXTENSION WHEN THE SWIVEL & BALL SOCKET CAUGHT HIS RING FINGER AS HE ACTIVATED THE TRIGGER OF THE IMPACT WRENCH WITH THE OTHER HAND. THE ROTATING HAZARD OF THE SWIVEL EXTENSION AND BALL SOCKET INJURED HIS FINGER.
THE INJURED PLACED 2 NUTS ON 660 LBS D11N BELLY & HE DECIDED TO BACK OFF 1 NUT, THEN ONLY 1 NUT REMAINED SECURING THE BELLY PAN. HE YELLED TO HIS HELPER WHO WAS INSIDE THE CAB TO LOOSEN THE HO IST LINE & REMOVE THE WEB SLING. HE INTENDED TO BACK OFF THE 2ND NUT WITH IMPACT WRENCH BUT IT CAME COMPLETELY OFF. THE BELLY PAN HAD 2 HINGES. IT SWUNG DOWN & LANCED BY HIS LEFT THIGH.
THE INJURED & A HELPER WERE REMOVING BOLTS AND NUTS FROM GEAR CASE. THE INJURED WAS HOLDING THE BOLT FROM LOWER FRAME & THE HELPER WAS LOSING THE NUT WITH IMPACT WRENCH. THE VIBRATION CREATING BY IMPACT WRENCH CAUS ONE BOLT TO COME LOOS & DROPPED. THE BOLT RICOCHETED OFF XBEAM & HIT HIS LEFT EYE BETWEEN THE EYEBROW AND EYE LID.
THE INJURED AND TWO OTHER HELPERS WERE USING HOT HOLE AUGER TO UNLOAD THE OVERLOADED DRILL HOLE. WHILE IT WAS GETTING THE POWDER OUT OF THE DRILL HOLE, THE AUGER HIT A ROCK AND THE WHOLE UNTI WENT INTO A JERK MOTION. THE INJURED WAS HOLDING THE HANDLE AND IT TWISTED HIS RIGHT ARM.
THE WELDER WAS CUTTING ON THE DRAG CABLE WHEN IT CAME LOOSE FROM THE SOCKET STRIKING HIM IN THE LEFT LEG.
THE INJURED PERSON WAS WORKING IN THE LOWER FRAME OF THE 8750. THEY WERE PREPARING TO LIFT ONE RETAINER PLATE FROM #2 SWING CONPARTMENT. AS THEY WERE REMOVING THE RETAINER PLATE WITH AN OVERHE AD CRANE AND CABLE SLING, ONE OF THE CABLE HOOKSCAUGHT ON A PIECE OF METAL AND WHEN IT RELEASED IT STRUCK EE IN THE FACE.
INJURED PUT A CONTROL BOX FOR FILTER FAN #1 BACK TOGETHER AT SHOP. HE THEN TOOK IT OUT TO THE 8750 DRAGLINE. WHILE IN THE PROCESS OF INSTALLING IT INTO POSITION ON MCC PANEL, IT FLASHED.
MECHANIC WAS ROTATING A CARRIER ROLLER ON A DOZER WITH THE TRACK SUSPENDED WITH HOIST. THE CHAINSLIPPEDON TRACK CATCHING HIS RIGHT HAND BETWEEN ROLLER & TRACK
INJURED WAS INSTALLING "O" RING IN A CYLINDER HOSE WHEN THE TORQUE FROM THE HOSE THREW HIS RIGHT HAND INTO THE FAN CAUSING LACERATION.
