Employee was installing a roof bolt and strap to support a "V" beam that had been set. As he was holding the strap in place and started drilling the hole, the steel kicked back pinching his middle finger against the "V" beam. This caused a cut to the middle finger on his right hand that required 2 stitches.

Showing all 32EE was roof bolting in #2 entry. The EE was putting up a metal strap. As the EE started spinning the bolt the metal strap spun around hitting the EE on the right arm. This resulted in stitches being used to close the cut.
EE was trying to push up a bent roof bolt. The wrench he was holding slipped, causing drill to mash his right middle finger.
Employee was roofbolting in the #3 RT crosscut. Employee was pushing up a roof bolt when the bolt wrench broke. The bolt wrench hit the employee on the left side of his face cutting a place about 1" inch long in his upper lip. (required 4 stitches).
EE went to foreman and told him that his right shoulder was hurting. EE is a roof bolter operator. EE stated that his shoulder started hurting while putting up a roofbolt. The foreman asked the EE if there was an accident, the EE responded no. EE stated again that his shoulder started hurting.
EE was welding bolt heads on planetary hub on miner, when EE got flash and burnt his eye's. Doctor at Whitesburg ARH removed a piece of metal from the EE's eye.
EE was raising drill head, when fast feed lever stuck causing drill steel to bend, striking his left hand.
Employee was roof bolting on 002 section. While drilling the hole in the roof, the drill steel bowed out, twisted, kicking around and hitting employee on the right hand.
Employee was roof bolting on 003 section. A piece of drill steel was hung together with the drill steel inside the drill hole. While the employee was trying to get the steel apart, the two pieces that were hung together came loose or apart and hit the employee on his right side.
While putting up roof bolts, employee's hand slipped off causing the bolt to strike his right hand.
EE was bringing drill steel out of the hole on last bolt in place when coupling broke. The bottom piece of steel spun hitting employee in the face on the left checkbone and under the eye and mouth.
Employee was tramming miner to the face. The miner pan got caught in some broken sandstone in the floor causing the miner to shift quickly to the right catching him between the miner tail and rib.
EE was welding, arc of welder flashed his eyes, causing burn to both eyes.
Employee was installing a roof bolt thru a roof mat. While pushing on the roof bolt, he slipped and hit the roof mat with his mouth causing a cut to his lip and loosening 2 teeth. The cut required 3 stitches.
While loading shuttle car, a rock slid off shuttle car and landed on employee's left foot.
EE got 3 of his teeth knocked out while roof bolting. He had a long piece of drill steel laying in tray and was swinging drill head to inside. The drill steel caught on roof bolter and flew out, striking EE in the mouth.
While roof bolting, EE finished drilling a hole and began to drop the head. As he dropped the hesd, he grabbed the drill steel and the head shifted and caught the tip of his finger of his left hand little finger between the head and drill steel resulting in amputation.
While roof bolting, employee was removing the drill steel from the bore hole when the drill steel struck him below the right eye (cheekbone area) causing a laceration.
INJURED WAS HELPING MOVE THE CONTINUOUS MINING MACHINE WHEN A COAL RIB ROLLED OFF THE CORNER OF THE BLOCK STRIKING HIM ON HIS RIGHT LEG AND ANKLE. THIS RESULTED IN ABRASIONS AND BRUISES TO HIS RIGHT LEG AND ANKLE
WHILE PUTTING UP ROOF BOLTS, EMPLOYEE GOT HIS LITTLE FINGER CAUGHT IN CLAMP THAT HOLDS ROOF BOLTS IN PLACE.
EE HAD FINISHED ROOF BOLTING A PLACE AND STEPPED UP ON THE PINNER AND REACHED BACK TO SWING HIS CANOPY IN AND PINNED HIMSELF BETWEEN THE CANOPY AND THE ATRS.
Draw rock fell from roof striking employee on his right foot.
Ee was holding the miner cable away from the miner with his right foot when the miner cable got tight and jerked his leg into the rub rail on the miner.
EE WAS PUTTING UP A CABLE BOLT WHEN THE CABLE BOLT BENT CAUSING THE WRENCH TO LEAN OVER AND BREAK. WHEN THIS HAPPENED THE EE'S ARM WAS BENT BACK OVER HIS SHOULDER AND THEN OVER THE CANOPY CAUS ING A STRAIN TO HIS RIGHT SHOULDER.
EE WAS ROOF BOLTING WHEN THE RIB ROLLED OUT AND STRUCK HIM IN THE HIP.
EMPLOYEE WAS DRILLING A HOLE WHEN HIS DRILL STEEL BEND AND SLAPPED HIM ON TOP OF HIS RIGHT HAND FRACTURING HIS RIGHT INDEX FINGER.
EE WAS BOLTING TOP AND STATED THE STAB JACK LEAKED OFF CAUSING THE CANOPY TO STRIKE HIM IN THE HEAD.
EMPLOYEE WAS INSTALLING ROOF BOLT WHEN HE LOOKED AWAY FOR A SECOND, ROCK FELL STRIKING HIS RIGHT FOREARM.
EE WAS OPERATING A JOY 14CM15 MINER WHEN A LARGE ROCK )APPROX. SIZE 4 1/2' X 3' X 2-4" THICK) FELL OUT FROM BETWEEN THE BOLTS NAD STRUCK THE EE IN THE HEAD. THE ROCK THEN LANDED ON BOTH WRISTS AND PINNED THE EE TO THE GROUND, CAUSING HIM TO BRUISE HIS BACK ON THE ROCK THAT HE LANDED ON.
WHILE BOLTING TOP A PIECE OF DRAW ROCK FELL FROM THE TOP STRIKING THE EE ON THE LEFT ELBOW. THE EE WENT TO THE DR. AND RECEIVED FOUR STITCHES.
EE WAS OPERATING A FLETCHER ROOF BOLTER WHEN A PIECE OF DRAW ROCK FELL OUT AND LANDED ON IS LEFT FOOT. EE'S LEFT FOOT NEEDED 4 STITCHES, APPROX. SIZE OF ROCK WAS 12" IN DIAMETER, 3-4" THICK, 4 -5 POUNDS.
