EE was helping the continuous miner operator move the miner from the #5 entry to the #6 entry the miner operator stopped the miner to hook up a slack rope the miner operator hooked the rope to the boom of the miner went to turn around and stumbled and somehow hit the boom swing lever and somehow EE's hand was where the boom swung into the rib smashing his left hand.

Employee had changed a pressure gauge on the continuous mining machine, employee was positioned at the rear bumper of the machine, he asked the machine operator to start the machine, the machine operator bowed the head of the machine causing the machine to go to the mine floor mashing the employee's foot.

Spot bolting went to move the lifeline out of the way and cut finger with roof bolt plate against the top.

EE was cleaning up cut, when a piece of rock came off of conveyor chain striking the EE on the knee.

employee was spot bolting the roof, when his glove got caught on a burr on the drill steel being used, twisting his right hand and wrist around the drill steel causing strain type injuries to the wrist and a slight fracture to the right thumb.

Employee was installing roof bolts accidently hit rotation lever causing roof bolt to spin stricking employee on left hand.

Employee was freeing the V belt on a rockduster and got four fingers on his right hand caught in the pulley.

The miner operator bent over to move the cable, when his chest activated the boom swing on the remote box and pushed his left leg into the rib.

Employee was postioning the roofbolter into place, when a piece of roof fell on the ATRS, causing the rear of the machine to raise upward. The employee struck the mine roof, injuring his neck.

Injured employee was installing a flyboard with the roofbolting machine. When he raised the drill head to install the board, his right index finger was caught between the board and the roof, causing a laceration that required 14 stitches.

Injured employee was trying to remove drill steel from the mine roof when it came loose, fell and hit his right eye resulting in a burn injury to the eye.

Injured employee was swinging roof bolter boom to install outside roof bolt and caught his legs causing bruising to both legs.

Miner was hung and he raised the stabjack and put a crib under it; when he let it down it busted and hit him.

Trying to get steel out of drill hole and caught his finger between drill steel and pot.

Operator was installing bolt into hole while attempting to straighten bolt, it spun around twisting his upper back.

Spinning drill steel & pulling down on it to remove stuck steel out of top when glove got caught.

Miner's hand was on drill pot when right middle finger was caught between uneven top and drill pot which cut end of finger off between finger nail and first joint closest to fingernail.

Trying to get drill steel out of top, the drill steel fell out striking him on the right hand.

Employee was putting pusher & finisher together caught left index finger between steel, pinching end of finer off.

Person was putting roof bolts up and got glue in right eye at #4 heading.

Employee was installing roof bolt & caught his left hand on boom resulting in injury. (Ring finger.)

While drilling anchor hole, the fitting connected to the hose busted, splashing hyd. oil in eyes and mouth.

EE was bolting top in #6 heading on #2 section, drill steel fell out of hole and landed on his left middle finger and ring finger.

Removing drill steel from hole, got right forearm between boom and roof.

Straightening bolt and slipped forward pulling lower back.

Loading pin plates, pin plates fell on right ring finger, broke finger cut it open.

INJURED WAS BOLTING SHEILDS ON A MACHINE WHEN SHIELD FELL ON HIS FOREARM CAUSING A LACERATION

BENDING OUT ROOF BOLT FOR LAST PUSH UP TO INSTALL IN TOP. BOLT TURNED & SPRANG BACK, STRIKING EE ON THE TOP OF HIS NOSE.

Putting up last row outside bolt swung pot into right knee and leg.

WHILE BOLTING CAUGHT HEAD BETWEEN POT AND TOP SEVERLY CUTTING RIGHT EAR.

A PIECE OF STEEL CAME OUT OF HOLE CATCHING FINGER AGAINST DRILLPOT ON PINNER

WAITING FOR PINNER TO MOVE UP TO NEXT ROW, HE HAD LEFT FOOT LAID OUT BEHIND HIM WHEN PINNER PULLED UP AND RAN OVER FOOT.

WHILE INSTALLING ROOF BOLT, BOLT KICKED BACK AND HIT EE IN THE SHOULDER. PRESSURE FROM DRILL POT.

Drilling holes-getting ready to leave-twisted and pulled back old injury.

RATCHING JACK UP TO ATRS ON BOLTER, HOOK CAME LOOSE AND ALLOWED JACK TO FALL ON THUMB. EMPLOYEE HAD THUMB RESTING ON CRIB BLOCK.

ROCK FELL BETWEEN RIB AND BOLTER AND PINNED LEG TO GROUND.

Ee was operating #2 bridge. Miner backed up, bumped #1 bridge and shoved #2 into ee.

EE WAS WELDING ON MINER REPORTED HE WENT HOME AND SHOWERED ADN HIS EYES BEGAN TO BURN.

LOADING COAL WITH MINER PIECE OF DRAW ROCK FELL FROM BETWEEN PINS HITTING HIM IN HEAD & RIGHT SHOULDER.

A PIECE OF ROCK BROKE OFF FROM THE RIB TO THE FIRST BOLT. EE RECEIVED A STIFF NECK.

WHILE PINNING A BOLT, THE PIN PLATE CAUGHT GLOVE AND PULLED IT OFF, BRUISING HIS THUMB.

PUTTING PUSHER & FINISHER TOGETHER WHEN LEVER MALFUNCTIONED CAUSING POT TO KEEP GOING UP & PINCHING HAND. EE REQUIRED STITCHES.

EE WAS BOLTING THE ROOF TOP-PUT DRILL STEEL AGAINST TOP-A LOOSE PIECE OF DRAW ROCK CAME LOOSE HITTING EE, CAUSING A SMALL CUT ABOVE WRIST AND SOME SWELLING.

SMALL PIECE OF ROCK FELL WHEN STARTING STEEL IN TOP & BROKE RT THUMB.

A PIECE OF ROCK FELL FROM BETWEEN THE RIBS AND THE BOLTS, BRUISED BACK AND KNOT ON SHOULDER.

CUT FINGER WHILE BOLTING TOP. CAUGHT FINGER BETWEEN STEEL AND PT #4 ENTRY.
