The miner operator was trying to get up out of the bottom rock. The miner was grading the bottom as it backed up the shuttle car "377" got hung on the back of the miner machine "224". This picked the SC up off the ground approx. 16". Miner helper tried to flag miner operator to stop, but operator was already moving forward. This caused the SC to drop off of the miner approx. 16".

The 413 RBM was tramming to #6 face. As it was being trammed, employee was hanging cable on the rib. As the bolt machine made a turn into #6 entry the cable tightened and was pulled towards the outby rib. The cable pulled the employee's feet out from under him causing him to fall to the bottom. As he fell he extended his arm to catch himself which dislocated his left elbow.

EE was pulling up to 223 miner when miner started backing up. His shuttle car got stuck on the bumper of the miner. He started flagging miner operator when miner started to pull forward dislodging the bumpers, letting the s/c drop to the ground throwing EE up into the canopy knocking his mine hat off. His head then jerked backwards into a bolt/nut lacerating the back of his head.

Upon an investigation, the EE alleges the RBM cable struck him across the head, knocking him down to the ground. The EE's complaints were the lower back and knee. EE was transported by ambulance to the ER where he was treated and released. He is on transitional duty with restrictions for a back and neck strain noted as subjective without spasm.

Investigation the ee was completing the rib bolt installment process along the l. hand rib #4 entry. After spinning resin he was appling hold pressure when 12" wrench broke allowing upper portion of wrench striking him in the l. side of head & face. He was trans. to ER ambulance and transferred to where he was admitted for contructive sugery to his cheek bone and nose.

EE stated he was struck on the head by the EE's canopy when peparing to swing the boom over toward the inside bolt. He attempted to drop the canopy hy level control from the mine roof and it would not release. he then attempted slide the boom sideways thinking the canopy was hung on the bolt plate. As he slid the boom the canopy fell from it position and struck the ee on the head.

EE WAS PUTTING FIRST BOLT(OUTSIDE BOLT) UP IN FACE OF #3 ENTRY.THE RIB NEXT TO EE ROLLED OFF AND HIT EE ON THE LT SIDE CAUSING RT SIDE TO STRIKE THE ROOF BOLTER.EE STATED THAT EE HAD MADE A VISUAL EXAM OF THE AREA PRIOR TO BOLTING AND THE AREA APPEARED TO BE GOOD. THE RIB WAS 5' HIGH X 16" X 16". EE DID NOT START MISSING WORK UNTIL 03/07/05.

Upon an investigation the ee was involved in an incident where his right lower leg and foot was struck by rib coal. the ee was operating a continuous miner in pillar block #171, and had moved the machine out of the second lift into the entry. the coal rib, after being stressed, rolled out on the side where the operator was positioned.

UPON AN INVESTIGATION THE EMPLOYEE WAS OBSERVING THE CONTINUOUS MINER MINING THE FINIAL PUSH OUT IN THE PILLAR BLOCK #106. THE EE WAS POSITIONED APPROX. 40' FROM THE MACHINE WHEN HE WAS STRUCK IN THE CHIN BY A FLYING ROCK COMING FROM THE CUTTER DRUM. THE EE RECEIVED SUTURES TO CLOSE THE WOUND & RETURNED TO WORK ON HIS NEXT SCHEDULED SHIFT.

EMPLOYEE STATED HE WAS ATTEMPTING TO INSTALL THE FIRST BOLT IN THE ROOF OF THE 4XR CROSSCUT. A PIECE OF COAL & ROCK MIDDLE BAND APPROX. 2'X 3' SQUARE ROLLED OUT OF THE RIB STRIKING HIS LEFT LEG AND HIP.

EE STATED HE WAS RAISING THE CANOPY ON HIS SIDE OF THE BOLT MACHINE AND WHILE DOING SO HE HAD HIS HAND RESTING ON THE TOP OF THE CANOPY. HE STATED THAT HE RAISED THE CANOPY TOO HIGH AND MASHED THE TIP END OF HIS LEFT INDEX FINGER AGAINST THE MINE ROOF.

EE WAS INSTALLING THE OUTSIDE BOLT ON 3RD ROW OF BOLTS IN THE FACE OF NO.1 ENTRY. EE STATED HE DID NOT SEE ANYTHING. ALL HE COULD REMEMBER WAS THAT HE WAS HIT BY A ROOF ROCK. INVESTIGATION FOUND A PIECE OF ROOF ROCK 65"L X 13-28"W X 3-5"THICK FELL STRIKING EE ON HEAD & SHOULDER AREA.EVIDENCE INDICATES FORCE & WEIGHT OF ROCK CAUSED INJURY TO RIGHT KNEE ALSO.

UPON AN INVESTIGATION THE EE, AND MINER OPERATORWERE MINING THE FIRST TOP LEFT LIFT IN PILLAR #182. EE HEARD A PILLER BUMP AND SAW A PIECE OF ROCK (SLIP) FALLING AT AND NEAR WHERE HE WAS POSIT IONED. HE RAISED HIS LEFT ARM UP TOWARD HIS FACE& MOVED HIS BODY POSITION TO AVOID CONTACT. HIS LEFT ELBOW WAS STRUCK THE THE ROCK CAUSING A CUTTHAT REQUIRED SUTURES.

EMPLOYEE STATED THAT DURING THE MINING OF THE #86 CHAIN PILLAR THAT A PRESSURE BUMP OCCURRED. WHEN THE BUMP HAPPENED THIS CAUSED A PORTION OF THE COAL RIB TO FALL OUT AND STRIKE THE EMPLOYEE. THE COAL RIB WAS FIVE FOOT IN LENGTH BY TWO FOOT IN WIDTH BY TWELVE INCHES IN DEPTH. THE HEIGHT OF THE ENTIRE COAL RIB IN THE WORKING AREA WAS NINETY INCHES.

EMPLOYEE STATED THAT AS HE DROPPED THE HEAD OF THE DRILL MAST DOWN THE WRENCH FELL OUT OF THE DRILL POT. IN REACTION HE REACHED FOR THE FALLING WRENCH AND WHILE DOING SO THE FIVE FOOT DRILL ST EEL FELL FROM THE HOLE HE HAD DRILLED AND HIT HIS LEFT THUMB.

EE STATED THAT HE WAS PREPARING TO DRILL A RIB BOLT HOLE. HE STATED THAT HE POSITIONED MAST OILDRILL STEEL IN IT TOWARD RIB, A 2 FOOT BOLT WRENCH WAS EXPELLED FROM DRILL STEEL TRAY, STICKING EE ACROSS CHEST. UNKOWINGLY THE WRENCH HAD BEEN FOULED IN THE TRAY.
