Employee was using an air chisel to remove hardened co-gen ash from around the pad bolts on a D-8T dozer. Employee was installing new track pads. A small piece of the air chisel bit broke off the bit and struck the employee in the forehead just below the brim of hardhat. The small piece of metal was removed at a hospital emergency room.

Employee was operating a wheel dozer and plowing snow from the roadways. Approx. 12 inches fell overnight. He ran over a large pot hole or low spot along the roadway, which caused the dozer to bounce suddenly. He felt pain in his lower back and later down his right leg.

Employee struck the rear of this right arm just above the elbow sustaining a small laceration requiring sutures (2), while pulling on the start cord of a small, portable pressure washer. He struck the fender edge of a parked water truck.

Employee was excavating wet ash, mud and clay material when his bucket kicked full and sent the material through the front glass. He was struck in the right side of his face and head with the material. Emergency call to 911 was made. EMTs responded and employee was taken to Altoona Hospital. He returned to work at 3:00 with minor scratches & abrasions.

TWO EE WERE ATTACHING A SET OF DOLLY WHEELS TO THE PULL AREA A LOADER. BY USING A BACKHOE TO LIFT DOLLY WHEELS INTO PLACE. AFTER PULL PIN WAS SET, OPERATOR MOVED BOOM OF BACKHOE DOWN IN ORDER TO SLACK THE CHAIN TO UNHOOK. WHILE UNHOOKING, OPERATOR BUMPED BOOM LEVER, DROPPING THE BUCKET ON THE MANS RIGHT HAND.
