The contracted employee stated that "something" hit EE from behind after cutting a tree causing EE to roll down the hill EE was working on. This resulted in broken ribs, a fracture above the teeth, and cuts/bruises.

Showing all 8The employee was grinding on a push arm pin in order to weld the pin. The pin rolled on to their left pointer finger, while the grinding was being done, resulting in a broken bone.
Kneeling underneath dozer, utilized an air nozzle to clean debris from bolts to remove final drive. Debris blew back into employees face and underneath safety glasses of right eye.

Employee was grinding on a welded spot when the hand grinder jumped and hit him on the left hand between the pointer and middle finger causing a laceration that required sutures.
Employee entered the cab of the drill and mashed his thumb when he shut the door on it. He had to have sutures to close the wound on his thumb. Tip of bone was cracked. He missed no scheduled work.
The injury was not reported until 11/23/08. We were informed that the EE was grinding metal and got something in his eye at that time.
EMPLOYEE WAS CUTTING A PIECE OF METAL WITH A SET OF TORCHES AND A PIECE OF RUST POPPED AND WENT INTO HIS RIGHT EYE.
