Showing all 9Tool got caught in a head pulley resulting in pulling EE's arm into head pulley. Both arms became entangled in conveyor before de-energizing conveyor belt.
The crew were replacing the belt on the shaker with a rolled belt. The new belt was suspended by a pipe and chains. The new belt was egg shaped because of weight and needed to be lifted and pulled to meet the old belt so that it could be spliced in. As EE and 2 other crew members were pulling and lifting the belt, EE's shoulder popped.
Employee climbing upon conveyor to unhook chain that lifted belting up. His boots were muddy and he slipped and fell.
EE was inside impactor, removing rock by hand and placing back into feeder. A 5 lb rock fell from the edge of the feeder (6'). It slid down the impactor and struck his hand that was resting on the rotor. Fractured index finger tip on his left hand.
REMOVING HOSE FROM DRILL HOLE. TRAPPED AIR PRESSURE CAUSED HOSE TO BLOW OUT OF HOLE. PIPE ON END OF HOSE HIT HIM ON SHOULDER CAUSING BRUISE TO BONE.
EE DID NOT HAVE HOLD OF HAND HOLD THAT WAS PROVIDED TO ENABLE HIM TO HAVE 3 POINT CONTACT.
EMPLOYEE WAS STANDING APPROX 10' FROM DRILL WATCHING DRILLER WORK. SECONDARY DRILL STEEL FELL FROM RACK WHEN DRILL WAS BEING MOVED TO ANOTHER LOCATION. STEEL HIT EMPLOYEE IN NECK AND FRACTURED 6TH VERTEBRA.
4 MEN WERE PICKING UP RAILROAD TIE TO STACK ON TOP OF ANOTHER RAILROAD TIE TO MAKE A LEVER AREA TO SET A BIN ON WHEN SITTING TIE DOWN IT SLIPPED AND CHOPPED ABOUT 5" ONTO EE FINGER TIP AND A R OCK. CRUSHED FINGER TIP ON R.H. MIDDLE FINGER SOTHAT IT HAD TO BE REMOVED FROM 1 ST JOINT.
