Unbolting chute while sliding it into place it slipped pinning hand between chute and hoper below. Put bandage on hand & went back to work. EE called me Sunday 3/25/12 to tell me he was going to have his hand x-rayed. I had no notification he was having problems till Sunday.

Showing all 12He was working at the pit line area, flinging 2x4 wood boards out of the way. A splinter from one of the boards entered the underside skin of his left hand middle finger. He removed splinter right away on 7/7/09, but on 7/16/09 it looked infected so he went to doctor on 7/16/09, after work. It was cleaned & antibiotic prescribed.
Wind blowing - cutting pipe with saw. Wind blew a sliver into right eye. Doctor dug it out.
Worker removed guarding attempting to clean tail roll with shovel.
The conveyor that the employee was on was wet. When he stepped down to get to the walkway his foot slipped on railing causing him to loose balance, falling to the walk about 3 feet from belt.
Climbing down of feed belt #16 at tailend, twist lower back and strained lower back. belt was damp but not a factor. just stepped or twisted wrong way.
Employee was clearing brush. He rammed a cottonwood ple through window of track excavator. While he was cleaning the glass out of hinges on excavator pulling rubber that broke free sliced elbow (right arm) on broken glass windshield. Cut was 2 1/2 inches and required 13 stitches at hospital.
VICTIM WAS RAN OVER BY TRUCK AFTER MANUALLY RELEASING WHEEL BRAKES.
EMPLOYEE WAS WALKING DOWN HILL FROM FEEDER TO JAW AND CAUGHT EITHER HEEL OR TOE OF LEFT BOOT ANDFELL ON HIS LEFT SIDE. HE LANDED 5 TO 7 FEET FROM WHERE HE TRIPPED. HE STRAINED AND BRUISED HIS NECK AND BACK.

CHANING BOTTOM TRACK ROLLS ON D9-2 ON D9 CATERPILLR. BRACE THAT WAS HOLDING TRACK FRAME SUSPENED BROKE, CAUSING TRACK FRAME TO DROP, PINNING RIGHT HAND UNDERNEATH AIR WRENCH. AND SMASHING HA ND.
EE REPORTED HE WAS CLEANING UNDER JAW CRUSHER CONVEYOR WITH HAND SHOVEL. TWISTED WRONG & FELT SHARP PAIN IN GROIN AREA. EE DID NOT REPORT INCIDENT TO SUPERVISOR UNTIL MAY 9, 2000 WHEN EE WENT TO THE DOCTOR. SEE ATTACHED.
