EE was exiting from R-50 haul truck. When he slipped and fell 2 to 3 ft landing on pile of oversize rock hitting his lower back.
Employee was feeling sick and was sweating. He shut loader off, put machine in park and got out of cab to get some air. He remembers starting to exit loader and nothing else until truck driver found him laying on the ground beside the loader.
EE HAD INSERTED A TRACK HOE BUCKET INSIDE THE CASE LOADER BUCKET WHICH WAS ON A DROP DECK TRAILER. HE RAISED THE LOADER BUCKET 3-4", CLIMBED OUT OF THE HOE, CLIMBED ONTO THE DROP DECK TRAILER & PLACED HIS RIGHT FOOT UNDER THE LOADER BUCKET. THE LOADER BUCKET FELL ON HIS FOOT.
