Employee was operating a Volvo A35C articulated hauler, and was bitten by an unknown insect on the left elbow area. Three days later, he requested to see a doctor, and was diagnosed with a localized reaction, and an infection. He has been seen by a doctor four times to date, and received three injections for infection and swelling, as well as a prescription to be taken daily.
Employee was seated at his desk. He reached around directly behind him to pickup a pen on a table with his right hand. When he reached around, he heard a loud cracking noise come from his right shoulder and felt pain in that shoulder.
