The injured employee and another employee had a disagrement which broke out into an arguement. The arguement escalated into a physical altercation, the second employee struck with his fist to the left cheek. Resulting in a laceration to the left cheek requiring nine stitches.
While walking across yard another employeee threw a football and it hit him in head knocking him down.
WHILE EMPLOYEE WAS TRYING TO HOOK A CABLE TO THE DREDGE, WAVES CAME UP AND TOOK THE SLACK OUT OF THE CABLE AND STRUCK EMPLOYEE.
NOTIFIED OFFICE ON 4/15/02 LOWER BACK & RIGHT LEG HAD A PAIN WHILE CLIMBING STAIRS TO MILL ON 4-19 HE AGGRAVATED IT AND REQUESTED TO SEE DR. AT THAT TIME.
EE WAS WALKING FROM PAINT SHOP TO LOCKER ROOM & RIGHT KNEE GAVE OUT.
VERY WINDY CONDITIONS-DIRT/DUST BLEW IN EYES. HE WAS CLEANING DIRT OFF OF A CONVEYOR WHEN THE WIND BLEW THE DIRT/DUST INTO HIS EYES. HE WAS NOT WEARING PROVIDED SAFETY GLASSES.
DUST BLEW INTO RIGHT EYE. DID NOT REPORT UNTIL 5/14/01.
WAS WALKING BY SCRUBBER UNIT WHEN HE FELT BURNING SENSATION ON NECK.
EE WAS BURNED WHEN HOT DUST FROM RISER BLEW ONTO HIS BACK (SEE ATTACHED).
WHILE OPERATING DRAGLINE EMPLOYEE EXPERIENCED SOME NUMBNESS ALONG LEFT SIDE OF FACE ARM AND LEG.
