Employee was un-banding precut structural steel. When the bands were cut, the steel shifted and a piece landed on the top of the employees foot. Frature of third metatarsel bone of right foot.

Showing 50 of 245The employee was using a bar to pry up on a pulley shaft. They had a motor and gear box suspended from a come-a-long and the lifting eye broke. When the motor and gear box dropped, the employee's finger was caught between the pry bar and the conveyor rock box. (Fracture of the right ring finger)
Shortness of breath and numbness of limbs
Employee and a coworker were splicing a conveyor belt at the washplant. When the belt clamp was removed it fell on employee's right index finger
Employee was changing screen on wash plant. He threw old screen off of deck and strained elbow. Accident was not recordable until 6-29-05 when he was placed on restricted duty.
Work related Stress. Did not notify employer until 7/22.
While employee was changing screens on washplant, his safety harness caught on spray bar. He slipped and strained his back.
Employee was using a Razor knife, the blade slipped and it cut his R thumb.
Employee was stepping down from his water truck onto the ground, when his leg slipped and he twisted his R ankle
Employee was driving haul truck when he hit a pocket of silt which jolted the truck. He experienced pain in lower back.
Employee was throwing a weighted rope to capture a pump float, and felt a pop in his right arm. Ruptured right bicep
Employee was flipping a section of screen material over and strained his upper back. Thoracic strain
Employee was shoveling on elevated platform, he twisted and felt pain in lower back. Lumbar Sprain
Employee was walking on a spray bar JCI screen. His foot slipped from under him, and the employee fell backwards, landing on his lower back on the spray bar.
Employee was dismounting cat skid steer loader, slipped and hit shin and knee on loader bucket. Sprain of knee and leg.
Employee caught his hand in a troughing roller on the 04-031 Cross Belt Analyzer feed belt. He was inpsecting the belt for alignment when he leaned out over the belt his right glove was caught between the roller and the conveyor belt. He pulled back on his hand and separated the thumb nail from the nail bed.
Employee bumped knee against a protruding angle iron bracket next to the roller mill. This resulted in a contusion to her knee. She had been off work for non-industrial knee surgery within the past year. Though the injury was minor, it was determined that a physician should evaluate her condition.
Breathing difficulties. EE provided first notification of this incident 7/26/05.
Employee was cutting with a torch and hot slag popped into his eye around loosely fitted welding goggles.
Employee has been tested and found to have a significant shift in his hearing. MSHA mandates that such hearing loss be reported.
The individual was discovered in a structure designed to settle aggregate fines. His job duties involved keeping the area clean from spillage & debris. The Structure wall is over 4 feet high and there was no reason for the individual to have access to the top of the structure. He was discovered floating in the structure.
Ee was using hand grinder cutting thin plate metal. strip of cut metal flipped off the edge of the grinder and cut his hand. Sutures required.
Ee operating a dozer inside the Clinker Shed. He was leaning on the right arm rest when he was jostled. His right arm slipped off the rest and banged against the metal plate under the arm rest resulting in a contusion. The Ee coming off swing shift, not required to return to work until following Wednesday, no light duty, Rx for pain and swelling.
EMPLOYEE WAS CARRYING 90LB BAGS OF SAND AND FELT SPASMS IN LOWER BACK.
The injured employee and his partner were working on the secondary screen deck, he bent over to pick up an extension cord, and when he stood up he caught his nose on the edge of a screen. Lacerated Nose
Employee was removing a rock from a primary bypass chute, when the employee threw the rock over the side of the chute he caught his hand between the side of the chute and the rock. Left hand laceration
TOTAL LOSS OF MAINTENANCE SHOP. UNKNOWN CAUSE, NO MINERS ON SHIFT AT TIME OF FIRE.
EMPLOYEE WAS PULLING A SAMPLE FROM FOOT STOCK PILE AT THE BASE OF PILE, USING THE SAMPLE TUBE. WHILE SHOVING THE TUBE INTO THE PILE, EE TWISTED HIS HIP CAUSING PAIN IN RIGHT HIP. NOTE: THE REASON THIS INCIDENT WAS NOT REPORTED TIMELY WAS DUE TO WEATHER, THIS WAS AN EXISTING CONDITION NOT MADE AWARE OF UNTIL 10/08/04.
Employee was feeding crusher when going up ramp dropped a rock. When going down hit rock, jolting cab. Didn't feel stiffness until after work. Reported next morning to Leadman (10-06-04). Did not request medical attention from Company until 10-7-2004.
Employee was removing a steel liner from a chute using a pry bar. The pry bar slipped hitting the employee in the face.
Employee was clearing a clogged material chute, he hit a large rock slab with a hammer and small peice of dirt flew up and went under his safety glasses and got into his eye. (superficial injury of cornea)
Employee was walking on walkway, stepped in dip on walkway and twisted knee. Sprained Knee (Right).
EE WAS PUSHING A DUMPSTER ACROSS THE BURNER FLOOR & CAUGHT HAND BETWEEN DUMPSTER & I-BEAM.
Ee walking up stairs, tripped on top step and fell onto his hip. He was carrying a flashlight in his back pocket.
EMPLOYEE WAS SERVICING EQUIPMENT @ END OF SHIFT. AFTER COMPLETION OF REFUELING, HE REENTERED CAB TO TURN ON BOOM LIGHTS. WHEN EXITING, HIS FOOT SLIPPED & HE FELL TO THE GROUND LANDING ON A ROCK. HE FRACTURED HIS LT UPPER ARM IN 2 PLACES. INVESTIGATION REVEALED NO TREAD LEFT ON WORKBOOTS.
Employee was stepping up onto a ladder, and slipped, falling backwards. The fall resulted in a contusion to the back.
Ee developed callous on right hand from using hand tools. Callous is impinging on movement of right little finger.
Employee was cleaning chutes above the base transfer conveyor. The water hose was slipping and employee tried to hold on to it. It jerked the employees shoulder which cause him back pain.
The injured employee and another employee had a disagrement which broke out into an arguement. The arguement escalated into a physical altercation, the second employee struck with his fist to the left cheek. Resulting in a laceration to the left cheek requiring nine stitches.
EE was moving a rock chute into place, he had his arm between the two pieces of chute. The chute moved and pinched his arm.
Employee was shoveling around conveyor, shovel hit conveyor belt and kicked it back striking the employee on the left forearm. Left forearm contusion.
EMPLOYEE WAS WORKING WITH A HOSE ON THE WATER TRUCK. EMPLOYEE'S LOWER BACK BEGAN TO HURT AND DID NOT GET ANY BETTER.
A MANHOLE COVER ON TRUCK SCALE HAD A CORRODED SUPPORT BLOCK WHICH GAVE AS EE WALKED ON IT. EMPLOYEE FELL, HIT HIS KNEE AND SCRAPED HIS SHINS AND CAUGHT HIMSELF WITH HIS HANDS. THIS RESULTED IN MINOR CONTUSTIONS/SCRAPES. DR. PRESCRIBED CELEBREX & VICODIN AND EMPLOYEE WAS PUT ON MODIFIED DUTY. FOLLOW UP ON 7/19/2004, DR ORDERED EE OFF FROM WORK.
Employee complained of having a sore back to his supervisor about 9:00. He attended safety/process training from 7:30 to 8:00, drove a sweeper from 8:00-9:00. His back was too sore to drive the water truck at 9:00. Says his back had a "twinge" when he bent over to pick up wheel chock.
EMPLOYEE LOST HIS FOOTING WHILE COMING DOWN THE STAIRS OF THE WASH GRAVEL PLANT.
CARPAL TUNNEL SYNDROME- EE FIRST COMPLAINED ABOUT PAIN IN 03/30/04, IT WAS REPORTED TO WORK. COMP ON MAY 17TH HE CALLED IN & DECLARED THAT HE WAS SCHEDULED FOR SURGERY & WOULD NOT RETURN TO WORK ONCE RELEASED HE WOULD RETIRE.
Employee was walking to service vehicle, tripped over rock fell backwards causing left knee to twist. Employee didn't seek medical attention until 06/22/04
EMPLOYEE SLIPPED ON GRATING THAT HAD HYDRAULIC OIL SPILLED ON IT. EE REPORTED INCIDENT ON 5/28. ON 5/30 HIS DAY OFF, HE WENT TO THE DR. (FIRST KNOWLEDGE).
EE WAS OPENING A DOOR ON AN EXPANSION CHAMBER (1ST STAGE- 7TH HOUR). USING A PRY BAR, HE WAS PULLING AGAINST A STUCK HINGE. THE BAR SLIPPED, HE STEPPED BACKWARD & TWISTED HIS FOOT, FRACTURING A BONE IN HIS LEG.
Was cleaning under the machine the machine stopped and as he was climbing the ladder he slipped; held on to the ladder, but hurt his foot.
