Accessing work area EE took step on walkway left foot popped with pain. Note: walkway was clear and unobstructed! Employee was taken to Bluefield Regional Medical Center, employee being referred to specialist for examination, appointment made for later date.

Employee suffering from symptoms of diabetic trauma, contacted EMS for transport to Bluefield Regional Medical Center. Reported later that employee has infection in employee's foot from a prior amputation of toe.

Drill operator had bee sting, allergic to same, used EPI Pen and then we took to Princeton Community Hospital where employee was treated and released.

Mechanic changing fuel filter on rock drill. Complained of chest pain, contacted EMS they responded, transported to Princeton Medical Center. Treated and released that day returned to work next day. Referred to family doctor for more follow up testing.

***NOT WORK RELATED*** Employee arrived on the job with tightness in the chest. After 45 minutes on the job the tightness did not go away, employee left for medical attention.

Employee having chest pains and already taken two nitroglyceran tablets.

Employee reported having chest pain to his supervisor, local EMT notified and transported to Bluefield Regional Hospital. Diagnosis inflammation (reflux) medication prescribed back to work next shift.

Haul truck operator having chest pains and had taken 3 nitroglycerin pills during shift to treat pain. Called emergency response to treat and transport to hospital.

Individual is scale clerk, operating truck scales, observed by highway truck drivers of being incoherent and unresponsive. Emergency Response contacted and individual transported to Bluefield Regional Hospital, ER started treatment for Stroke. Later same day he was air lifted to Roanoke, found individual had stroke, heart attack, high blood pressure, diabetic and a hernia.

Individual complained of chest pain and shortness of breathe.

Plant operator was performing preshift check of the Primary screen system when he passed out. Employee was found by another employee who notified the supervisor, 911 was contacted at 7:40AM. Employee was alert and talking, Emergency services arrived 7:50, employee was allowed to walk to ambulance and transported to the hospital.
