Said Named Individual (SNI) was preparing to replace the bags in the Sly Baghouse. He had just opened the door to the bag area, leaned down to latch the door when the wind caught the door. The door closed on SNI's left arm and hand. SNI lifted the door and notified foreman of incident. SNI was diagnosed with a contusion to his left forearm.

He reported the incident to a Foreman on Monday, 6/4/2007. He was treated by a physician and a corneal abrasion of the right eye was diagnosis. Employee was given antibiotics.
