Observed the employee inspecting the skid steer. After a moment, EE could not speak properly or use their left side effectively. EMS was called immediately. EE was taken by ambulance to emergency services. This was reported immediately to MSHA (1-800-746-1553) at 3:50pm May 31, 2016.

Showing all 15Employee had just completed a cleaning task and sat down to wipe sweat from his face. He had taken his safety glasses off and a gust of wind blew some dust in his eyes. We sent him to the emergency room to get his eyes flushed and the Dr. requested that the employee remain out of work until Monday morning. scheduled shift on 11/11/15.
Employee was walking up stairs at break time and back muscles cramped up. Employee stated that he felt a muscle pull in his back two days earlier while operating a rivet buster during his shift. Employee requested to be taken to hospital.

EE reported to supervisor on 1/19/2010 that his left knee was swollen, the employee did not report any accident, but felt it was due to excessive walking and stair climbing. On 01/25/2010 the employee was sent to Healthworks for evaluation, diagnosis was left hip and thigh strain due to excessive use. Employee was placed on restricted duty on 1/25/2010.
Dust
Employee was cleaning payloader windows with "The Works" chemical. The wind blew some of the chemical into his right eye.
EE'S LEFT KNEE HAS BEEN BOTHERING HIM & HE HAS GONE TO A DR. EE STATES THAT HE DIDN'T HAVE AN ACCIDENT, JUST THAT HE FEELS HIS KNEE IS BAD DUE TO BENDING & CLIMBING THAT HE DOES AT WORK. THE 3/19/04 IS NOT A DATE OF ACCIDENT, IT IS THE DATE THAT THE EE TOLD MINE ABOUT HIS KNEE.
EE WAS DUMPING VINSOL INTO FEED CHUTE OF MILL. MATERIAL WAS BLOWN OUT OF CHUTE AND COVERED HIS ARMS AND FACE.
THE WEATHER WAS VERY WINDY. EE WAS DIPPING PIT IN 952 ENDLOADER. WHILE LIFTING BUCKET OF HOT MATERIAL, WIND BLEW STEAM ONTO HIM THROUGH DOOR.
WORKER WAS GETTING INTO A BOBCAT AND FELT PAIN IN HIS BACK.
WHILE WORKING ON A KILN ROLLER BEARING FINE METAL FLAKES BLEW INTO THE WORKERS RT. EYE.

SUPERVISOR WAS EXAMINING STONE STORAGE AREA WHEN THE PLASTIC DUST CONTAINMENT STRIPS HIT HIM IN THE FACE DUE TO THE WINDY WEATHER CONDITIONS.
