Employee was blowing down packing machine, cement blew up under safety goggles into both eyes, treated for chemical burns to both eyes, out of work 2 days.
Employee using hand grinder with cutting disc to trim off steel patch, disc caught on edge of steel and shattered. Piece of disc struck left forearm lodging under skin causing laceration. Stitches required to close wound.
The employee was going to the pumps in the quarry to check their status. EE stepped over a pipe and observed a snake in the shadow of the pipe. As EE turned to get away from the snake, EE caught boot on the pipe and hit knee and hand on the ground.
The employee was walking to a loader to begin operation. The ground was still wet from rain where the loader was parked. As the employee approached the loader (approximately 3' from the loader) EE slipped. EE was diagnosed with a contusion and an abrasion on EE's right arm.
Employee was using a knife to open packaging when EE nicked right thumb. The laceration required two stitches and EE was released to full duty.

Observed the employee inspecting the skid steer. After a moment, EE could not speak properly or use their left side effectively. EMS was called immediately. EE was taken by ambulance to emergency services. This was reported immediately to MSHA (1-800-746-1553) at 3:50pm May 31, 2016.
While closing the hand brake on a railcar, the employee pulled a muscle in employee's shoulder.

Supervisor was walking at ground level of kiln pier, EE slipped on unpaved surface twisting right knee. Slight tear to right ACL occurred. This accident became reportable on 05/31/2016 when employee had surgery on right knee.
Tightening a nut and bolt with a spud wrench and the wrench slipped and hit EE in the nose. Doctors are saying that it is suspected to be broken. The practitioner report says that this person is on 'modified duty'. Modified to include limited lifting, minimize bending and no heavy pulling or pushing. This person has been off of work since the injury happened.
While unloading a sheet of plywood from a stack of plywood the employee lost control of the plywood and dropped it on EE's foot from waist height. The employee was wearing steel-toe boots at the time and suffered a contusion on the left foot resulting in restricted duty.
The employee was working in a secondary containment for grinding aid; behind 2nd Bank Silos. The employee slid awkwardly while standing in the dry containment area and injuring EE's left foot/ankle.

No flame or smoke was identified. One of five vents in the South dust collector ruptured when the door to the dust collector was opened. This event is in connection with citation number 8900222.

Employee was using a come-along to move over a screw auger trough with a wire rope sling wrapped around it. Whilst pulling the lever the wire sling broke and the employee fell backwards and hit his upper back on the edge of a steel beam. This contact caused a contusion on his back. The doctor's note stated "ee was seen today in the ER. He should not work tomorrow".

High Coal mill inlet temperatures caused and explosion and fire in the coal mill cyclone and dust collector compartments. No personnel were injured and the coal mill' s fire suppression and explosion containment systems functioned properly minimizing property damage.

He received two small cuts which required a total of 3 stitches. Released for regular duty.
