Showing all 4On the 22nd of March EE said that wind blew dust in his eye. He did not report it until the 23rd. He refused to go to the dr. On March 28 EE reported to his supervisor that he was still feeling discomfort in his left eye. He was then taken to the company Dr.
Dust flew into employee's left eye This was not a lost time accident the dr looked at his eye administered eye drops and wrote out a prescription for eyedrops. EE returned to work no restrictions immediately. No Follow up visit to the doctor is required.
SHOULDER PAIN, CHEST PAIN ON LEFT SIDE, DIFFICULTY BREATHING. GOT HURT OFF THE JOB, PULLED A CHEST MUSCLE, NOT WORK RELATED.
