Forklift rolled over right edge of right foot. Employee was filling forklift up with propane. Strained right foot. Dr. released employee immediately to light duty. No light duty available. Employee will receive a 3 day non paid suspension and written warning. No re-check needed.
On the 22nd of March EE said that wind blew dust in his eye. He did not report it until the 23rd. He refused to go to the dr. On March 28 EE reported to his supervisor that he was still feeling discomfort in his left eye. He was then taken to the company Dr.

UNKNOWN EE WAS SEEN REPAIRING WIRING FOR SCISSOR LIFT. APPROX. 15 MIN LATER, HE WAS FOUND LAYING ON HIS BACK UNABLE TO SPEAK.
EMPLOYEE WAS PARKING TRUCK AT THE END OF HIS HSIFT. HE OPENED THE DOOR AND LOST HIS BALANCE AND FELL OUT. NO CUTS SCRAPES OR BRUISES EXCEPT FOR A SMALL SCRAPE ON ELBOW AND A SPRAINED WRIST .
