Employee was found collapsed & unconscioucous on the second floor of the mill. CPR was preformed and 911 called. He was pronounced dead at the scene.

The employee had spent approximately 5 minutes in a squatting position repairing a 1 1/2" wash down hose. When he stood up his knee (left) buckled backwards re-injuring his knee. Sprain of left knee.

Welding wear plates on the top deck subframe of a screen for approximately two hours in a sitting and/or squatting position. The employee experienced pain in his left knee after completing the job.

Employee was stung by a bee on her right hand and as a result swelling occurred and had to be transported to the clinic for treatment.

During his normal shift, employee was doing his plant check at the bottom of the main shaker (wet screen) when he felt a sting to his lower, inside middle lip. He tried to frighten the bee with his hand, but his reaction was too late. He immediately reported the sting to his supervisor & manager at the maintenance shop. Manager drove him to urgent care center.
