Fall to the walkway or working surface

During his normal shift employee was inspecting the head pulley for the surge plant stacker after completing his inspection employee was walking down the cat walk when he tripped and fell injuring his left shoulder. Employee immediately reported the injury to his supervisor. Supervisor drove employee to urgent care center.

Dozer trap and the grizly at end of its conveyor belt were plugged with boulders 1"". While he was removing by hand, they release boulders at chute hit his right hand (finger point 2,3,4) because his fingers were trapped between boulders. Fingers points were bleeding and broken. Surgery was done 7/27/11

Changing cutting edges on Loader, the EE was helping install bolts and nuts. The EE was trying to align one bolt and pushing up it with right #3 finger, when co-worker tried to tight. The finger snagged by bolt when turning and the laceration of the tip and it was necessary to put 4 sutures. Direct causes: failing to use PPE, improper placement, failure to follow proper procedure.

During his normal shift, employee was doing his plant check at the bottom of the main shaker (wet screen) when he felt a sting to his lower, inside middle lip. He tried to frighten the bee with his hand, but his reaction was too late. He immediately reported the sting to his supervisor & manager at the maintenance shop. Manager drove him to urgent care center.

EE REMOVED GUARD TO TRAIN BELT AT SCREW TAKEUP. AFTER BELT TRAINED BELT WAS SHUT OFF. EE #2 ENT TO INVESTIGATE WHY JAW CRUSHER WOULD NOT START AND TOLD INJURED EE THAT HE WOULD WORK ON IT. START SEQUENCE REQUIRES CONVEYOR BE STARTED FIRST AND WHEN EE PUSHED START BUTTON HEARD YELL AND PUSHED STOP BUTTON. EE 2 WENT TO HELP INJURED GET ARM FREE FROM PULLEY BY CUTTING BELT AND REMO
