During the EE's work shift, EE experienced what was reported to Contractor by the treating physician as a heart attack which resulted in EE's death. It is Contractor's understanding that the incident is classified as Degree 8 - Natural Causes.

Employee stepped over a hose on level ground, and felt ankle twist as they did. The employee continued working for an hour, and foot began to swell. It was discovered that they had a broken bone in their foot.

Employee was squatting down with a hose to wash up and when employee stood back up, heard a pop in left knee. Taken to doctor on 8/12/2020 and was placed on restricted duty.

EE knelt to inspect track pads. EE raised up, felt pop in knee. Knee popped a 2nd time, pain left. Ee cont working. Knee popped a 3rd time. EE called Supv. EE was diagnosed w/a sprain. MRI was ordered at followup visit. EE was diagnosed with Meniscal tear on 06/08/20. EE can no longer cont w/normal work duties. EE sent home pending medical evaluation. 1st shift missed 06/19/20

Employee got some dust in left eye and eye was irritated. Took to Urgent care clinic and eye drops and tetanus shot was prescribed to prevent infect. No days off or restrictions given. Employee back to full duty that day

Employee was exiting a diesel truck. As they stood and twisted to exit the employee felt something in their L thigh. Employee was sent for diagnosis. Dr.stated no treatment needed. EE reported pain persisted. Sent for follow ups. EE missed first day of work on July 5, 2017.

Employee was inspecting a work area. During the inspection the employee squatted down next to the supervisor who was inspecting the position of a damper. When the employee attempted to stand upright they had immediate pain in the right thigh. The employee then sat down on the deck and informed the supervisor of the issue. The employee was then taken for medical attention.

While working in the raw mill building, he felt a bite on his lower leg. The bite became infected(cellulitis) later and required medical treatment & lost days. A brown recluse spider is suspected to have bit him.
