Employee was walking outside and got dust in their eye. The eye was immediately flushed out. The irritation persisted and the employee was taken to a clinic where no foreign objects nor scratches were found. The next day, the employee still felt irritation and returned to clinic where antibiotic eye drops were prescribed to prevent infection. Employee was wearing safety glasses.

An employee was struck in the left thumb with flying metal debris. It was determined on 8/8/2013 that it was medically neccessary to remove the metal from his thumb.
Employee suffered apparent heart attack and was found unresponsive. EMS transported employee to local hospital where employee was pronounced deceased.
