Showing all 5Employee was in break room, turned to pick up drink can, turned twisted knee and felt a "pop". Complained of difficulty bearing weight on R leg. Evaluated in medical went home. Returned next day and continued to work regular schedule, not missing any work. Was seen by MD 09/10/2018 and had MRI 09/24/2018. Returned to work from MD visit wearing brace. Last day of work 09/30/2018.
GRM was in field maintenance shop reaching for a jack stand in a squatting position,& when ee attempted to resume a standing position, ee felt a muscle strain in lower back on the left side. EE reported back strain to nurse, and was later brought to Dr. for an x-ray and evaluation. Prescription medications ordered and out of work for four days.
MWT was working in maintenance shop. While seated on a stool ee reached forward to retrieve a wrench with left hand, and ee felt a pull in low back. EE was transported by ambulance to St. Elizabeth's ER where ee was treated and released to home later that evening.
Report only: CAH reported to Gulf Coast for return to work after being off for personal reasons. CAH informed Gulf Coast staff that EE's injury was related to work activity. No specific event was identified that led to the injury. CAH remained under the care of EE's personal physician until EE returned to work on 10/12/2016.
