EE was working on c3 tail pulley guard with the contractors. EE squatted down to put rubber guarding on the frame. When EE stood halfway up, EE's knee popped and EE dropped back down.

Showing all 18Employee says that EE's vest caught on the handle of the door of the truck EE was driving, no witnesses to this. Employee also states that when EE was working 3 weeks prior(under different owners) EE hurt self doing something else and injured the same body parts.
Employee had called in in April with a personal back injury and has been treated by PCP. EE has been on FMLA for several months and now wants to say this is work related. We are denying this claim.
Employee was cleaning off a screen when he stated having shortness of breath and chest pains. An ambulance was called and went to the hospital. It was determined that he had an infection and severe dehydration the stemmed from that infection.
Employee states that he was walking down the steps of the plant and felt a sharp pain in his back. This injury was not reported until late the next day.

The employee was repairing the pipeline at a joint when the wind caused the pipe to get away from him and while trying to pull back into position, he felt a pop in his left shoulder.
