EE ONSITE TO DUMP EXCAVATION MATERIAL. DUMPED LOAD THEN HAD TRACTION PROBS WHEN MOVING OUT. GOT OUT OF TRUCK TO SHOVEL SAND UNDER REAR TIRES THEN PUT SHOVEL BACK. WHEN TRUCK HADN'T MOVED YET, SWENSON FORKLIFT OPER. CHECKED ON HIM AND FOUND HIM OUTSIDE HIS TRUCK, COLLAPSED ON GROUND & UNRESPONSIVE. THAT OPER.IMMEDIATELY BEGAN CPR AND AN AMBULANCE WAS DISPATCHED-ARRIVED AT 1:18PM.
EMPLOYEE HAD UNHITCHED CHAIN FROM BOX, BEND DOWNTO PICK UP THE RADIO HE DROPPED, THE WIND CAUGHTAND WHEN HE STOOD UP THE HOOK HIT HIM ON HIS HARDHAT.
