Employee contacted the supervisor on 1/25/21 to report that EE would be out due to blood clots in EE's lungs and contacted the supervisor again on 1/27/21 notifying supervisor that EE believed the clots could be a result of the muscle strain reported in 12/2020 and EE would be examined again on 2/3/21.

Employee called into work on 1/13 that employee had shoulder pain but did not think it was work related. Pain was in shoulder that was replaced in 2/2019. X-rays showed damage to screws placed during shoulder surgery. Employee expressed to Dr. that employee thinks the injury is related to a 10/7/19 near miss incident.
