Absorption of radiations, caustics, toxic and noxious substances

While installing a side plate on a feeder, a vibrator activated and blew dust under EE's goggles and into EE's eye. EE went to urgent care, where a small lime particle was removed from EE's eye.

Employee slipped on snow covered step of loader resulting in knee pain.

Employee was changing out a bearing on a hydrator. While lowering the bearing it slipped and fell onto employee's right foot causing bruising and swelling.

Plant Support Specialist was descending stairs when they fell forward landing on their right heel and contacting the middle handrail on the landing with their knee.

The employee was installing rubber skirting at the plant when dust flaked off from above and fell between goggles and face. The material caused eye irritation requiring treatment.

a plant assistant was rolling a drive belt onto a crusher. While manually rolling the belt on, three fingers were pinched between the belt and sheave resulting in 2 fractured fingertips.

Employee reported knee pain on 6/19 that they associated with a funny feeling in their knee while operating a walk behind skid steer on 6/14. Employee was seen by a physician on 6/21 and put on restrictions.

Employee was descending staircase by C754 and slipped on the bottom step and rolled ankle on a rock at the stoop of the staircase.

Absorption of radiations, caustics, toxic and noxious substances

Employee opened an inspection port to check the flow of material when material broke free resulting in hydrated lime coming out of the port and into the employees face. The employee was taken to a clinic to be seen by a physician. No treatment was required but the employee was provided prescription antibiotic drops as preventative care.

Employee was operating a front end loader, they leaned on right arm to adjust seat, hit a pothole and jammed their right arm up into their neck resulting in right shoulder pain. The incident was reported on 12/13/23.

A plant assistant was assisting in a screen change. EE was kneeling down to hand another plant assistant a pry bar. EE knelt down on a rock causing EE some pain as EE stood up and had some slight pain in EE's knee. The employee continued to work and reported the EE's knee pain on 9/7 and then received treatment.

Employee was washing lime off of a piece of plant equipment to remove an access door and some lime and water got inside EE's rubber boot causing a lime burn on EE's calf. Employee reported on 8/2 after EE treated with first aid originally and then later got antibiotics for infection of the burned area.

Employee was loading a customer truck from T243 and turned to close a gate when employee felt a sudden sharp pop in lower left leg.

Employee was inspecting tracking on a bagging belt. When they squatted down to inspect the belt from beneath they brought a hand up and the belt caught their glove pulling their thumb between the belt and a pulley.

Employee was climbing down the ladder of loader. When they got to the last step they felt their shoulder pop before letting go of the ladder and pop back into place after letting go. They reported joint pain the following day and was placed on restricted duty.

Employee was sitting with body at a compound angle while shooting clinker in the preheater. Employee finished the task and stood up when employee felt a sharp pain in lower back.

Employee was applying pressure to a spanner nut while another was striking it to loosen it. When the bar the employee was holding slipped it caused EE to lose EE's balance and twist EE's knee. The injured employee said the incident occurred on 6/25/2022 and EE reported it to the operator on 7/6/2022.

Loader Operator was walking up stairs to cab and slipped on snow on the third step, causing lower left knee to strike the metal step.

Employee was tightening fittings with a pipe wrench and struck elbow on a steel door frame on 2/10/22. Employee was checked at a clinic on 2/11/2022 and released to work with no treatment or restrictions. Employee returned to clinic on 2/22/2022 due to swelling and was placed on work restrictions.

Employee was at the fuel station fueling the bobcat. EE caught EE's foot on the fuel hose which caused EE to trip and fall on EE's wrist.

On 1/19/2022 the employee reported to EE's supervisor that during EE's shift on 1/14/2022 EE strained EE's back. The employee was unsure of the time or cause. On 1/24/2022 EE began treatment and given work restrictions that could not be accommodated.

Employee was taking a vac hose off a storage hook. When the hose straightened out it caused EE to fall and hit EE's elbow on the floor. After the shift the employee realized the fall resulted in a cut to EE's right elbow which was treated at clinic with glue.

DURING THE REPAIR OF PICKUP WINDSHIELD WIPER ASSEMBLY THE MOTOR AND ARM RETURNED BACK TO THE CENTER POSITION WHILE THE PICKUP WAS OFF DUE TO STORED ENERGY. THE EMPLOYEE'S FINGER WAS PINCHED BETWEEN THE ARM AND BRACKET, CUTTING OFF THE TIP OF EE'S INDEX FINGER.

Employee called into work on 1/13 that employee had shoulder pain but did not think it was work related. Pain was in shoulder that was replaced in 2/2019. X-rays showed damage to screws placed during shoulder surgery. Employee expressed to Dr. that employee thinks the injury is related to a 10/7/19 near miss incident.

Employee was walking from a front end loader to a pick up at the dump pad and slipped due to slick ground conditions caused by snow and ice.

An employee was repairing a conveyor belt using a belt cutter to cut the belt. While turning the handle on the belt cutter, EE felt pain in the right shoulder, resulting in a strain. EE was seen at a clinic and prescribed pain medication.

The employee raised the box to change the oil on a Polaris Ranger ATV. Once the oil change was complete, the employee was lowering the box by pushing on the top of the box when they heard a pop and felt pain in the shoulder.

Employee was completing the shut down procedure at the plant in the quarry. With a lot of resistance when shutting the wash-back valve on the dewatering screen, the employee struck the lever with the palm of right hand to shut off the valve.

Employee was descending a permanent 4' ladder in the shop to the lube pit when they fell backwards to the ground hitting their head and shoulder causing a bump and concussion.

Employee was using a 12"" ratchet to remove a coupler fitting when ee felt a pop in right forearm. EE reported it on 10/13 but did not seek medical treatment until 10/24 and was placed on work restrictions for a possible torn ligament.

Employee was using a pick ax to clear built up material for a rail and tie inspection. Employee's arm began to hurt the next morning. Employee was sent to the Urgent Care where employee was diagnosed with tendinitis in the left forearm and placed on restricted duty.

Employee was cleaning out a pipe with a plunger hand tool and EE's finger was pinched between the wall of the pipe and the handle of the tool.

Struck by flying object

EE was driving pickup truck on site when an object (dust/debris) entered EE's eye when EE blinked. Physician removed item, and provided medication to prevent infection.

EE was performing a pre-shift inspection on a Cat 988 front end loader, was attempting open a stuck access door to the engine, when the door was freed it came off of its hinge causing the employee to lose their balance and fall to the ground injuring a rib.

EE reported lime burns on the toes of EE's left foot. EE was assisting the Maint. Dept. with lime-fines silo repair. EE stated EE noticed issues with EE's foot a couple of days earlier, but continued working until reporting it on 4/8/17. EE's physician prescribed burn ointment and scheduled a follow-up appt.

EE was replacing a blade on a razor scraper when the blade broke and lacerated their left thumb, requiring stitches to close the wound.

EE was barring down clinker in the preheater transition chute when ee felt a ""pop"" in right elbow accompanied by numbness in hand and fingers. EE was treated by a physician for an injury/irritation of the ulnar nerve and placed on restricted duty pending clearance to return to full duty.

EE was part maintenance crew removing/shooting clinker from K-2 Preheat Tower. EE was in a seated position shooting the kiln gun, when EE felt something hit left thigh. EE was taken to ER - received treatment to remove a fragment of the kiln-gun slug that ricocheted and struck EE. EE was discharged and is waiting to return to full duty pending clearance from the physician.

EE was cleaning material from hydrator discharge chute using metal shovel/scraping tool to push material down the chute into screw conveyer. The end of the shovel was caught by the auger and pinched EE's hands between the tool handle and side of the chute. EE was treated for bruising and strained ligaments in EE's right thumb, placed on restricted duty with follow-up scheduled.

EE was walking between plant equipment,stepped on a rock that was under snow and material and twisted knee while catching themselves. Was seen by a physician 2/10/17 and placed on restricted duty and provided a knee brace until cleared by physician.

EE was shoveling waste material from a clinker chute as part of maintenance duty, afterwards EE stated EE's back was hurting and EE's right leg was numb. EE provided a physicians note 12/22/2016 placing EE on restricted duty pending follow up.

EE was working as part of a maintenance crew shoveling clinker/waste material from kiln No. 2 when employee strained left shoulder. A physician placed employee on restricted duty regarding lifting, pending the release to full duty and prescribed anti-inflammatory medication.

EE was closing a gate valve on a material chute, as ee stepped down ee lost 3 points of contact when hand slipped causing ee to fall approx. 18"" onto the steel grating on the plant walkway. EE was treated by a physician for back bruising and placed on restricted duty - prescribed medication for pain.

EE finished cleaning the K-1 Tunnel and heading to the dust-off station when EE felt an object enter EE's left eye behind EE's safety goggles. EE used eye wash station, but EE's condition worsened overnight. EE was seen by a physician the following day for corrosion to EE's cornea from lime contact and placed on restricted duty.

EE was performing a walk through of K-1 Pre-Heat Tower, he was crawling through the chute when his foot slipped and as he braced himself he strained his right shoulder. EE reported incident immediately but chose to wait for treatment assuming his condition would improve. After not improving, EE was seen by a physician on 2/25/2016 and placed on restricted duty.

EE exited loader to move a different piece of mobile equipment, as he was returning to the loader he was walking in the packed snow tracks of the loader and slipped. His foot slid to the edge of the track and rolled over fracturing right his ankle.

EE was working with maintenance crew reinstalling a screen in the Hydrate Roto-Sieve.4 employees were sliding the screen into place which fits tightly screen slid suddenly and pinched his index/middle fingers between lip of the screen and screen housing. Examining physician lanced fingernails to relieve pressure and prescribed pain meds. EE has returned to full duty.

EE was cleaning out a plugged chute on K2 Preheat tower. EE was looking into cleanout port when the chute plugged again causing lime/dust to exit the port striking EE in the face/eyes. EE has been examined/treated by a physician for corneal abrasions and chemical injuries to his eyes. Return to full duty expected pending clearance from his physician.

EE was chipping out the Ducon Scrubber above the hydrator unit using an air chisel. A piece lime or scale entered his left eye behind his safety glasses and full faced mask he was wearing. EE was seen by a physician the following day who removed the particle and prescribed medications to treated corneal abrasion. No other restrictions were put into place.

A Pete Lien & Sons Concrete Delivery Driver was injured while exiting his mixer truck. The EE parked on an incline and misjudged the distance to ground level from the last step from operators station. EE slipped, and bruised his hand on the door's hand hold when he caught himself. EE saw a physician and was place on restricted duty limiting use of left hand.

EE was loading material bags manually onto the bagging machine. As he was putting a new bag onto the spout adjacent to a bag being filled, the bag being filled ruptured sending lime into his face and eyes. EE was seen by physician and provided Rx for medicated eye drops. Follow-up scheduled for 9/12/2014.

7/29/14 Employee was assisting with a screen change, after crew was finished-EE complained of lower back/hip pain. 7/31/14 EE was seen by physician and placed on restricted duty with no work activity allowed until cleared by phys. EE prescribed anti-inflammatory Rx, rest and ice for pain.

The employee stepped down off of the Hydrate loading platform onto the top of the customer trailer to load product. EE twisted/rolled his ankle as he stepped down spraining his ankle. He was seen by a physician, fitted with a boot/brace and placed on light duty until follow-up.

EE was injured while retrieving a part no. from the steam fan sheave wheel removed the guard as the fan was winding down - he inadvertently contacted the belt which drew his right hand into the sheave wheel and cut the tip of his thumb off. EE was treated by a physician who sutured the wound and placed him on restricted duty. Follow-up Appt. scheduled in 10 days.

6/12/14 EE reported back strain from oper. FEL. EE did not see Phys. until 7/1/14-ordered off of work for 2 wk. Follow up visit on 7/15/2014 EE was cleared for light duty starting 7/22/2014. Phys. has not determined if work related or existing non-work related injury from summer/2013 and again 3/2014 -incident being treated as reportable pending diagnosis.

EE misstep at the top of the ladder access on a Cat haul truck as he was about to descend. He grabbed the handrail and caught himself to keep from falling and strained his shoulder in doing so. EE saw a physician on 6-9-14 and was placed on light duty/use of right arm and prescribed anti-inflammatory & pain medications. Follow appt. up scheduled 6-20-14.

EE was operating a drill press to drill a piece of steel plate, the drill bit seized causing the plate to spin which struck the EE on the left ring finger. His finger was lacerated requiring sutures - follow-up appt. scheduled for 5/2/14 for re-check.

EE was barring/cleaning material off of interior of lime kiln walls - informed Supervisor the following day that he was suffering from muscle weakness and spasms in his shoulder-primarily on left side. Phys. has placed him on lifting restrictions with Rx. for muscle relaxers/anti-inflammatory meds. Follow-up appt. sched. for 4/29.

EE was filling customer truck at load-out when material stopped flowing out of tank, EE thought tank was empty and directed truck to move another tank. EE failed to close gate valve-material started flowing from tank again and EE walked through cloud of lime material to try to close valve causing lime to enter eyes.

EE was cleaning up material after emptying a RC Quarry bag-house into a dump truck when dust blew into his right eye. EE was wearing eye protection but the fine material was blown around them requiring him to seek medical attention. Phys. has prescribed medication for treatment.

Employee strained shoulder while trying to press new return roller into Hot Belt Conveyer. EE was applying pressure to roller, afterwards felt a strain in shoulder that worsened-saw phys. on 4/4 and placed on light duty until follow-up.

EE was performing maintenance on a waste tank @ Hydrate Plant with an air wand to remove material.Lime blew back and entered his right eye around his safety glasses. EE was loading a customer truck when the chute's actuator froze in position. Material backed up into the chute and was forced out of the sample port - lime fines blew around his safety glasses and into his eyes. Attending physician prescribed medication for eyes - follow-up visit on 12/17/13 cleared EE to return to full duty.

EE was returning tools and items to a tool box after completing maintenance work on a conveyer. While he was lowering the tool box lid, it slipped from his hands and pinched/lacerated the middle fingers on both hands. EE saw a physician that evening and was placed on restricted duty.

EE suffered an eye injury when dust was blown behind his safety glasses. EE saw a physician for the injury and was treated for a corneal scratch. This injury was originally treated as a non-reportable injury and that only over-the-counter medication was provided-11/19/2013 operator was made aware that eye medication was a prescription making this injury reportable.

EE sprained his ankle on uneven ground as he was exiting from a man-lift's operator's station. Physician has placed him on restricted duty with follow-up appointments scheduled.

EE was performing maintenance on an auger at the Hydrate plant-he was traveling on a walk way with low head room. He was accessing a lower level (which is signed for low head room) when he failed to duck low enough and struck his head and jammed his neck. EE has been placed on restricted duty for 2 weeks per attending physician.

Dump truck driver was driving with box in the air for too long when the box struck the overhead conveyor, driver was thrown from truck onto ground & truck rolled to a stop approx 1/4 mile later. Driver has some compressed & cracked vertebra but is out of hospital & walking. Conveyor has bent pieces but was retrained & working within 30 min. Hoist & subframe on truck were bent.

EE was unplugging the dragline in the Lime Plant, during the procedure lime fell behind his safety glasses and entered his left eye. Injury was treated with first aid measures-physician placed EE on driving restrictions for 24 hours making this a reportable injury.

EE was shoveling spilled material onto a conveyer, as he twisted he felt pain in his lower back. Was seen by a physician and placed on restricted duty and given a prescription for pain and back spasms.

EE informed MGT of back pain on 2/12/2013. EE had been running a skidsteer loader/water truck-did not see physician until 2/15/2013, an MRI was scheduled for diagnosis. 2/21/2013 Phys.diag. was a bulging/ruptured disk in lower back-placed on light/sedentary duty. Item being treated as work related unless/until directed otherwise by phys.

Employee was descending stairwell and missed the second to last step from the bottom. In trying to catch himself ended up falling on his back resulting in injuries to his neck, head and chest.Phys. prescribed medication for pain-no lost days or restricted duty.

EE was pulling out fuel line hose to re-fuel mobile equipment. He had appox. 25' of hose out when it snagged on a loader bucket. EE attempted to flip hose and free it-in doing so felt a pull/strain in right shoulder. EE has been placed on restricted duty/rest for arm and has scheduled follow-up appt. w/phys. in 1 WK.

EE was shoveling lime when a piece of lime got behind the side shield of his safety glasses and into his left eye. EE's eye was flushed with eye wash and returned to work. The following morning there was still irritation and EE went to the emergency room for medical treatment. His eye was irrigated and drops/salve were prescribed. Returned to full duty 7/15/2012.

EE has suffered from neck and shoulder pain for over 11 months prior to reporting item to MGT. EE has no lost work days/mod. duty and has continued normal work-but has not improved. EE elected surgery 11/26/12-condition being treated as reportable pending Phys. assessment.

Employee was dismounting the Cat D10J Bull Dozer and slipped as he stepped onto the access rung on the Dozer's push arm. EE fell, resulting in bruising/contusions to his chest wall and back-placed on restricted duty regarding lifting/climbing. Not reported to Management until 6/11/2012. Return visit to physician 6/13/2012-approved to return to full duty-no restrictions.

Employee was operating a dump truck, the truck's automatic locking mechanism on the tail gate was stuck and wouldn't close. Employee attempted to free the mechanism with his hand and when the latch freed itself- it pinched his middle finger on his right hand.

Miner was lifting metal cones/tubes out of a crate and placing them on a fork lift when he felt a ""pop"" in his back. The incident happened on 03/16/2012, but employee did not see a physician until 03/23/2012 following his scheduled time off (night shift crew works 7 days on, 7 days off).

Employee strained shoulder/rotator cuff lifting drill steel from a pick-up. An initial visit to Md. place EE on restricted lifting and physical therapy-but resulted in no lost time or change in duties. After no significant improvement with physical therapy-an MRI was ordered 4/23/12-surgery performed 6/14/2012.

Over-exertion in lifting objects

EE was helping 2 other employees lift a 30-40lb bolt bin on top of another shorter bin. He grasped the bin with both hands from the back side of the bin. While lifting, one of the ends of the bin slipped down causing EE to grip tighter. This motion caused a strain to EE's bicep. EE was taken to a clinic for the injury on 7/22 where they restricted his duty until 7/25.

While crossing rebar matting employee slipped/tripped and twisted his left knee. He reported the incident on 5/10/2011 but did not seek treatment initially. Finally in July, he sought medical treatment where a doctor drained the fluid buildup in his knee and gave him a cortisone shot. The employee is able to work with no restrictions.

Our employee was using a circular saw to cut dry wood. The saw kicked back and the blade caught his coveralls causing the blade to make a cut into the fatty tissue of his abdominal area. He was treated at the hospital and released the same day. He was not given any work restrictions or time off duty by the attending physician.

Struck by falling object

Employee was on conveyor cutting belt with a chop saw when he slipped and the chop saw fell on his left hand. Employee received surgery to repair tendons in hand.

EE was rebuilding an off road truck box at Truck Shop. He was hammering a steel plate into place when he struck his elbow on the corner of the box. He just thought it was a bump/bruise. It was not until 4/26/2010 that this was reported as an injury. He went to doctor on 4/26/2010 where they drained the fluid buildup in his elbow and gave him a cortisone shot.

Employee was getting off the crane and lost his footing. He caught his fall by hanging onto the handrail. This caused him to wrench his shoulder. He didnt think he needed to go to the doctor. On 3/15/10 he explained that his shoulder was still sore. We set up an appointment where they diagnosed a strain. No meds given. It was recommended he attend physical therapy for one week.

Employee received noise induced hearing loss. This was found after the employee's retest that showed a standard threshold shift of > 10dB at 2000, 3000, and 4000 hz.

Angle iron 3"" x 3"" x 1/4""x 3 ft. long slipped out of chocker and hit worker on right shoulder causing a bruised shoulder.

Injured employee had left hand on a stop bracket while lowering a dust collector hose. The dust collector hose was being lowered by hand, the hose assemble is counter weighted. The hose got away from injured employee and smashed the tip of his left thumb between the jig and stop bracket.

Employee sustained a whiplash type injury while driving a backhoe. He drove through a trench in the haul road that caused the machine to buck up and threw his head back and forth.

Employee removed guard to adjust conveyor take-up pulley. While cleaning mud off the pulley the wrench was caught in the roller and his finger was pulled into and pinched by the frame.

Employee was working on a disconnect box and arced a screwdriver across incoming power leads causing a flash that burned his hand and lips.

Was walking from parking lot to the plant. Stopped to get newspaper and slipped, twisted knee and fell down.

Employee was using a plasma cutter to cut a piece of plate steel. He was holding a straight edge with his left hand as a guide. As he passed by his left hand a piece of slag entered his left glove at the finger seam, burning his left middle finger at the tip. The gloves were in good repair and properly used.
