Did not loose consciousness and was transported to Renown Medical center for observation. Released on the next day. Doctors do not know the reason for the collapse
Employee was sitting @ the lunch table and a 2nd employee came up behind the 1st employee and slightly hit him open handed to the side of his head. Employee indicated that he has a headache and neck hurts.
UNKNOWN, EMPLOYEE PUT ON HIS ACCIDENT REPORT THAT HE DID NOT KNOW. HIS LEFT WRIST WAS SWOLLEN. HOW OR WHY IT HAPPENED HE DID NOT KNOW. WHAT CAUSED THE SWELLING HE DID NOT KNOW.
EE walked underneath Finish Mill #1 feed conveyor when he felt something in his right eye. Later that day he washed it several times. Next day he was taken to the medical center and subsequently to an optometrist were an imbedded foreign object was removed.
EMPLOYEE WAS WORKING ON #2 COAL MILL, THE WIND BLEW DUST/DIRT INTO EMPLOYEE'S FACE, DRIFTING BEHIND EMPLOYEE'S PRESCRIPTION GLASSES AND CONTACTING HIS LEFT EYE. TAKEN TO DOCTOR 3-27-03.
