employee was unplugging a product line with a vacuum truck when the product freed up and pushed the hose out of their hand causing product to hit them in their face

Employee stepped in a pothole, twisted right ankle and fell. This caused a sprain to the right ankle. Employee released to full duty. The sprain had not been getting better so was referred to an ortho, transferred care to a specialist. Employee started Physical Therapy on 2-17-25 released to full duty.

Employee was climbing up a ladder out of a pit and when employee was looking up dust landed in employee's left eye. The employee was prescribed a preventative antibiotic eye drop by urgent care.

Mechanic was repairing primary feeder, guiding a bar with EE's left hand on top of the bar and running a hoist with EE's right hand. The bar moved unexpectedly and caught the mechanics left hand/fingers between the bar and the wall of the bin.

The employee received burns on their wrists and back of neck when they came in contact with cement dust off of dust collector bags during disposal.

Vacuum hose plugged while cleaning spillage. Employee was attempting to clear the line by lifting it over head. Once the line was lifted over head ee felt a pop in the lower left side of back.

Electrician positioned self in a manner that allowed a 10 lb inspection door to indirectly fall on left hand causing a laceration to index finger and a bruise to thumb. Laceration required stitches.

EE was replacing a return roller and had to tap on it causing foreign material to drop from somewhere and landed in eyes.

Employee climbed onto the passenger side of the OTR truck while the engine was running, to wash the windshield when the right hand slopped into the air-conditioning belt/pulley. This resulted in a partial amputation of the tip of the index finger and lacerations to the other three fingers.

Employee was pushing down on pry bar with a closed grip. The pry bar slipped causing the load to fall on top of the pry bar. Due to the closed grip the employee's finger was pinched between the concrete and the pry bar. The pressure caused the tip of the finger to split. Leather gloves were worn.

Employee picked up a 35# piece of steel with the left hand it hit a beam above and dropped on to the 2nd finger of the right hand. Splinted.

The employee reported the swelling of left hand on 1/8/17 and decided EE did not need medical attention that day. On 1/12/17 wanted an X-ray and showed no broken bones and the doctor modified use of the left hand and had EE wear a splint.

EE was lifting a box off of the desk to move it to another area of her desk and felt a pull in her shoulder. She was taken to Concentra medical and returned to work the same day. Nature of injury was a thoracic strain. On 5/1/2015 she had a MRI done on that shoulder hence the reason for reporting

Employee slipped on an uneven surface while shoveling debris into a wheel barrel. The employee struck his middle finger on the edge of the wheel barrel as he caught himself. The impact resulted in a fractured middle finger.

Mechanic was operating a 4 1/2"" hand held grinder with a cutting wheel cutting a metal window frame. The cutting wheel stuck and kicked back coming into contact with the mechanics right hand/wrist resulting in a laceration.

Miner was conducting repairs on a screw conveyor. Miner failed to lock out tag out the screw conveyor. The miner indicated that the screw conveyor inadvertently started. Miner received severe injuries to his right hand.

Miner was using a bar to dislodge build up on the feed shelf of #2 kiln. The miner was attempting to push the built up material off the shelf with the bar and smashed his RH middle finger between the bar and the metal door opening frame.

Miner was climbing up stairs and injured his right foot. Miner indicated that he had overstepped one of the stair platforms as he was stepping up the stairs. As the miner continued to climb the stair, the right foot became wedged in-between two sections of the stairs which caused (2) metatarsal bones to break in his right foot.

Laborer was unloading a coal rail car. He indicated that he had stepped down off a concrete platform onto the coal grizzly platform and sprained his left knee. The coal grizzly platform is approx. 4"" to 6"" lower than the concrete platform. The laborer indicated that he may have stepped onto built up rock or coal material that had not passed through the coal grizzly.

A miner was operating a trackmobile that was struck by a rolling empty rail car that was found to have a faulty hand brake. After complaining of back soreness the next day the miner was taken to a medical facility and found to have likely strained his middle back area and was then placed on restrictive duty on 10/21/2011. The miner was given 3 prescriptions.

Loading drill pipe into Atlas Copco L8 30 drill carousal with winch installed on drill mast. Sliding hook weight got caught on part of the mast about half way up. Man standing below at controls was struck in the hard hat by weight after it came loose and side down the cable. Hard hat webbing cut the mans scalp. 10 staples needed. No lost time.

AS OTHER EE WAS EXITING THE LADDER, INJURED EE WALKED OVER TO THE LADDER LOOKED UP AND A 12"" CRESENT WRENCH FELL OUT OF OTHER EE'S BACK POCKET AND DROPPED, STRICKING INJURED EE IN THE BRIDGE OF HIS NOSE BREAKING IT AND REQUIRING STICHES.

EE walked underneath Finish Mill #1 feed conveyor when he felt something in his right eye. Later that day he washed it several times. Next day he was taken to the medical center and subsequently to an optometrist were an imbedded foreign object was removed.
