employee was unplugging a product line with a vacuum truck when the product freed up and pushed the hose out of their hand causing product to hit them in their face

Employee was moving too fast for the conditions. Slipped with EE's left foot and fell to left buttock. Caused swelling and the employee was taken to the ER. Was admitted over night for observation and prescribed time off work until 10-1-2025

EMPLOYEE WAS USING A SPUD WRENCH TO BREAK A BUTTERFLY VALVE LOOSE WHEN IT MOVED UNEXPECTEDLY CAUSING THE EMPLOYEE TO CATCH A FINGER BETWEEN THE SPUD WRENCH AND THE HOUSING OF THE RSV. THIS SPLIT THE FINGER OPEN AND IT REQUIRED STITCHES

Employee was reaching over a table to install bolts into a probe flange while standing in mud. right foot slipped and EE felt a pop in EE's knee

employee was removing a guard with another employee when the second employee lost their grip and let go causing the weight to transfer to the injured employee causing a strain to the left bicep

Employee stepped in a pothole, twisted right ankle and fell. This caused a sprain to the right ankle. Employee released to full duty. The sprain had not been getting better so was referred to an ortho, transferred care to a specialist. Employee started Physical Therapy on 2-17-25 released to full duty.

Employee was climbing up a ladder out of a pit and when employee was looking up dust landed in employee's left eye. The employee was prescribed a preventative antibiotic eye drop by urgent care.

Employee got left foot caught by conveyor. Employee was taken to medical. Resulted in broken ankle and contusions.

Employee was pulling on a pipe when unexpected movement happened it caught the employees right thumb between a flange and hanger bracket.

While lighting the small torch in preparation to light main flame residual natural gas in the location caused a flash. Employee experienced 2nd degree burns to locations to the left side of face, neck and hand

On 6/17/2023 at approximately 10:45 am an employee reported feeling discomfort in the right arm. The employee was climbing a scaffolding ladder at the time the discomfort was noticed. On 6/20/2023, the employee was evaluated by a doctor and prescribed medication for treatment.

Employee was being trained in a new task, changing out separator fan blades, when their right foot slipped and caught a fan blade resulting in a strain/sprain.

Employee fell off of the ladder while climbing down from a wheel loader, missed/slipped a handhold, landed on EE's left leg and broke the ankle and leg.

The employee was taking a wagon full of parts and tools down a ramp when employee slipped and fell onto right side.

EMPLOYEE HAD BEEN USING A JACK HAMMER AND A SLEDGE HAMMER TO LOOSEN A BOLT FOR A LINER IN THE MILL. WHEN EE FINISHED EE NOTICED THAT EE'S ELBOW WAS RED, SLIGHTLY PAINFUL AND SWELLING

While cleaning on the ground level under the #2 & 3 finish mills, the employee got dust in right eye.

Employee was swinging an 8lb hammer backhand to fit a piece of flat bar to a pipe. Did not position self properly for the task at hand.

Was clearing chunks off of the top of the hopper with a shovel when product sloughed off and dust blew back into the employee's face and under safety glasses.

Mechanic was repairing primary feeder, guiding a bar with EE's left hand on top of the bar and running a hoist with EE's right hand. The bar moved unexpectedly and caught the mechanics left hand/fingers between the bar and the wall of the bin.

The employee received burns on their wrists and back of neck when they came in contact with cement dust off of dust collector bags during disposal.

Vacuum hose plugged while cleaning spillage. Employee was attempting to clear the line by lifting it over head. Once the line was lifted over head ee felt a pop in the lower left side of back.

Electrician positioned self in a manner that allowed a 10 lb inspection door to indirectly fall on left hand causing a laceration to index finger and a bruise to thumb. Laceration required stitches.

EE was replacing a return roller and had to tap on it causing foreign material to drop from somewhere and landed in eyes.

Employee climbed onto the passenger side of the OTR truck while the engine was running, to wash the windshield when the right hand slopped into the air-conditioning belt/pulley. This resulted in a partial amputation of the tip of the index finger and lacerations to the other three fingers.

Employee was pushing down on pry bar with a closed grip. The pry bar slipped causing the load to fall on top of the pry bar. Due to the closed grip the employee's finger was pinched between the concrete and the pry bar. The pressure caused the tip of the finger to split. Leather gloves were worn.

Employee was bent over and stood up stepped over a pipe and walked into a valve handle hitting employee in the face between eye brows required 10 stitches

Employee picked up a 35# piece of steel with the left hand it hit a beam above and dropped on to the 2nd finger of the right hand. Splinted.

EE was lifting hydraulic Ram and slipped. EE re-grabbed it and felt a pop in left hand. The employee reported the swelling of left hand on 1/8/17 and decided EE did not need medical attention that day. On 1/12/17 wanted an X-ray and showed no broken bones and the doctor modified use of the left hand and had EE wear a splint.

Employee was unloading coal in high wind conditions when EE received coal dust in left eye.

During #1 Kiln outage employee was installing brick in the Kiln, the air flow through the Kiln blew brick dust around and the employee got some in right eye.

EMPLOYEE WAS WALKING TO THE WAREHOUSE AND SLIPPED AND FELL ON SNOW COVERED ICE

Employee was scooping up a bucket of clinker with a 966 loader and felt a pain in his back that also affected his sciatic nerve.

Maintenance Mechanic was hooking up a trailer welder to a fork lift, the wheel of the trailer was against a rock when he pulled it to hook up felt a pain in his groin. After seeing the Doctor it was determined he has a hernia on the right side

EE was lifting a box off of the desk to move it to another area of her desk and felt a pull in her shoulder. She was taken to Concentra medical and returned to work the same day. Nature of injury was a thoracic strain. On 5/1/2015 she had a MRI done on that shoulder hence the reason for reporting

Employee was standing & looking up at the feed end of #2 Finish Mill he collapsed for no reason hitting the back of his head. Did not loose consciousness and was transported to Renown Medical center for observation. Released on the next day. Doctors do not know the reason for the collapse

Employee slipped on an uneven surface while shoveling debris into a wheel barrel. The employee struck his middle finger on the edge of the wheel barrel as he caught himself. The impact resulted in a fractured middle finger.

On 11/27/13, employee was carrying a bucket of parts and felt a sharp pain in the back of his right knee. Employee did not wish to seek medical attention at that time. On 2/24/14, employee was placed on restricted duty.

Employee bent down to open apron door on crusher when he felt pain in his left shoulder. On 11/14/2013 the employee was diagnosed with a shoulder sprain and was placed on restricted duty and prescribed physical therapy.

Employee twisted his upper body while holding a grate and felt pain and stiffness in his right knee. Employee was diagnosed with a knee strain.

EMPLOYEE WAS CLIMBING DOWN THE LADDER OF A RAIL CAR AND MISSED A RUNG WITH HIS RIGHT FOOT. THE LEFT FOOT BECAME DISLODGED FROM THE SUDDEN DROP. THE EMPLOYEE HELD ON WITH HIS HANDS AND DID NOT FALL. EMPLOYEE STRAINED HIS LOWER BACK FROM THE SUDDEN DROP.

Mechanic was operating a 4 1/2"" hand held grinder with a cutting wheel cutting a metal window frame. The cutting wheel stuck and kicked back coming into contact with the mechanics right hand/wrist resulting in a laceration.

Miner was conducting repairs on a screw conveyor. Miner failed to lock out tag out the screw conveyor. The miner indicated that the screw conveyor inadvertently started. Miner received severe injuries to his right hand.

Miner was using a bar to dislodge build up on the feed shelf of #2 kiln. The miner was attempting to push the built up material off the shelf with the bar and smashed his RH middle finger between the bar and the metal door opening frame.

Miner was climbing up stairs and injured his right foot. Miner indicated that he had overstepped one of the stair platforms as he was stepping up the stairs. As the miner continued to climb the stair, the right foot became wedged in-between two sections of the stairs which caused (2) metatarsal bones to break in his right foot.

Employee was hitting the side of a hopper discharge chute with a bar. The employee lost balance and inadvertently smashed his right hand ring finger in-between the bar and chute. Employee received finger contusion/lacerations. Employee received stitches due to the injury.

Laborer was unloading a coal rail car. He indicated that he had stepped down off a concrete platform onto the coal grizzly platform and sprained his left knee. The coal grizzly platform is approx. 4"" to 6"" lower than the concrete platform. The laborer indicated that he may have stepped onto built up rock or coal material that had not passed through the coal grizzly.

A miner was operating a trackmobile that was struck by a rolling empty rail car that was found to have a faulty hand brake. After complaining of back soreness the next day the miner was taken to a medical facility and found to have likely strained his middle back area and was then placed on restrictive duty on 10/21/2011. The miner was given 3 prescriptions.

Loading drill pipe into Atlas Copco L8 30 drill carousal with winch installed on drill mast. Sliding hook weight got caught on part of the mast about half way up. Man standing below at controls was struck in the hard hat by weight after it came loose and side down the cable. Hard hat webbing cut the mans scalp. 10 staples needed. No lost time.

While jackhammering #2 kiln, equipment kept braking and having to jump in and out of bobcat put stress on my right knee. Long periods of running bobcat stressed right knee, at which point I was unable to put any weight back on my right knee.

Employee was sitting @ the lunch table and a 2nd employee came up behind the 1st employee and slightly hit him open handed to the side of his head. Employee indicated that he has a headache and neck hurts.

EE WAS WAITING FOR ANOTHER EE TO EXIT A LADDER ON #1 KILN HEAT EXCHANGER. AS OTHER EE WAS EXITING THE LADDER, INJURED EE WALKED OVER TO THE LADDER LOOKED UP AND A 12"" CRESENT WRENCH FELL OUT OF OTHER EE'S BACK POCKET AND DROPPED, STRICKING INJURED EE IN THE BRIDGE OF HIS NOSE BREAKING IT AND REQUIRING STICHES.

UNKNOWN, EMPLOYEE PUT ON HIS ACCIDENT REPORT THAT HE DID NOT KNOW. HIS LEFT WRIST WAS SWOLLEN. HOW OR WHY IT HAPPENED HE DID NOT KNOW. WHAT CAUSED THE SWELLING HE DID NOT KNOW.

Employee was cribbing a finish mill when he stepped off a concrete block twisting his left knee.

Worker was trying to close tail gate latch when it closed with workers right thumb in the latch.

Removing spitzer from mill with chipping hammer, when a metal fragment came off hammer, penatrating employees right middle finger.

May of 2010 I injured my back working on the tail end of the drag chain.

Hammer struck washer & washer broke striking employee in the nose.

Employee was talking to a co-worker while opening a door not aware of where this thumb was, smashing it in the latch.

Truck driver stepped out of truck twisting his left ankle on uneven ground.

Employee was moving a coal pipe for reinstallation in a klin when the pipe fell for a forklift landing on the employees right foot breaking the matatarsal.

Aligning bolt hole with a bar, bar slipped and crushed his left hand between the bar and the clinker breaker housing.

EE was welding the roof of the firing hood from a ladder when he fell from the ladder, striking his head on the ground below. Employee suffered a laceration to the back of the head.

Employee was stepping onto a curb while coming to work. Employee tripped on uneven surface and rolled his right ankle, causing a fracture.

EE was kneeling while welding/gouging from 8/12-8/17. On 8/18 the ee was fitting shell, which account for about 25% work on knees. Morning of complaint, there was no kneeling until approximately 11am. EE was wearing knee pads (as well as using 1' thick felt pads from 8/12-8/17).

Employee was moving materials on a flatbed maintenance truck. Employee over stepped into the hitch frame causing a contusion on his left shin.

Employee was replacing a liner on the 115 chute and felt some pain in his neck and upper back.

Employee was drilling mounting holes when the drill bit seized causing the drill handle to spin out of employee's hand.

Employee was moving a flop gate on a material transfer chute. Employee was lifting the gate in an awkward position when he strained his right shoulder.

Employee claims to have sustained an occupational illlness (possibly some other form of Tenosynivitis) after going to his personal physicians. The illness is currently not fully diagnosed and there is no specific point of injury or description of how the illness may have occurred, whether work related or non-work related.

Employee tripped over an uneven surface and fell, causing a laceration on his left knee.

Employee was walking down steps and slipped with his left foot causing his right ankle to twist.

Employee was climbing down a vertical ladder carrying a bucket, began to slip and caught himself, causing the injury.

Employee was stepping down from deck of tractor and fell against chain hook when bottom step broke. Employee was released back to regular duty immediately. Employee was pulled out of work by the doctor when the injury flared up on 9-21-2007.

Employee was exiting Bobcat and slipped off of the step causing ankle to twist.

Employee was working in the warehouse and stepped on a bolt slipping and fell backwards hitting his head on a motor. This caused a gash in his head that needed stitches.

Employee was closing a roll-gate on a cement storage silo when the bracket holding a come-a-long broke, causing employee to fall onto transfer pipes. Employee hurt left shoulder.

The employee and others were changing a cable on the clinker crane. He was pulling on the cable and a little while later he felt pain in his shoulder.

Employee & co-worker were lowering a separator with a rope when it got hung up. Co-worker was dislodging the separator while employee was holding the rope. Employee was not focusing on co-worker's effort when the separator became dislodged. When he noticed the free fall he tried to release the rope but it hung on to his left index finger causing an open wound and dislocated bone.

Employee was placing a starter into the bed of a pickup when he felt a pain in his lower back.

Employee was connecting 6"" hose to guzzler truck's hose when truck's hose struck him in the face causing an open wound.

Employee was dismounting from guzzler truck when the step broke causing him to fall hard on his knee and hit his back against the truck.

Employee was loading raw mill #2 with steel balls from a 55 gal drum. When drum was almost empty it was tipped over to access the balls easier. Employee felt minor pain in his shoulder when he lifted the bottom of the drum, which was lying on its side, to make the balls roll out. Employee did not report the incident to his supervisor until 3-14-06.

EE walked underneath Finish Mill #1 feed conveyor when he felt something in his right eye. Later that day he washed it several times. Next day he was taken to the medical center and subsequently to an optometrist were an imbedded foreign object was removed.

Employee was stricing a coal hopper bin with a steel bar to unclog a build up. When he struck the bin the bar bounced and struck him on his forehead causing an open wound.

Employee was greasing a bearing on an air piston when he pinched his left hand index fingers causing an open wound.

EE was working at her desk in her office. When she stood up from her chair she twisted her right ankle when she failed to set her foot flat on the floor. This resulted in a sprained ankle.

Employee was walking up some stairs when he lifted his right leg to step on the second step of the stairs he strained a muscle in his right leg calf.

EE was greasing the bearing of the Silo 7 tipping valve when he pinched his left-hand index finger, causing an open wound at the tip of his finger.

Employee was working on #2 Finish Mill when he lost his balance and grabbed the hand rail. This caused him pain in his right arm but continued working on the 25th. On 1-26-05 he went to see the doctor and was put on restricted duty.

EE was placing a barrel bearing into the bucket of a bobcat when his right middle finger got pinched between the coupler on the bearing and the edge side plate of the bobcat's bucket.

EE was manually adjusting the throttle on Kiln #2 Pony Motor when his hand slipped hitting the motor fan with his right index finger. Resulted in an open wound requiring five stitches.

Employee was removing grinding bowl from grinding machine to set it on a table. When he was carrying to the table he felt a sharp pain from the back of his left shoulder to bottom left side of his back. Bowl weighs approx 15 lbs.

Employee was carrying damaged cement bags up five stair steps to recycle the cement when he felt pain in his left knee.

EE was driving a semi-truck. While waiting to load, he rested his right arm on the stick shift between his elbow and wrist. When he lifted his arm to shift gears he felt a sharp pain in his right hand ring finger. His right hand became inmobilized to where he couldn't form a fist. He felt presurre in the palm of his right hand and numbness in his fingers.

ON JUNE 18TH, THE EE WAS WORKING WITH ANOTHER EE TRYING TO PULL A 4"" GUZZLER HOSE OUT OF A HOLE WHERE IT WAS STUCK. THEY WERE USING A BOBCAT WITH A ROPE TO PULL IT OUT. IT CAME OUT LIKE A STRETCHED RUBBER BAND & HIT THE EE IN THE BACK OF THE LEGS. IT KNOCKED HIM DOWN & KNOCKED THE WIND OUT OF HIM. HE LANDED ON HIS RT. SIDE & HAD SOME SCRAPES.

EMPLOYEE IS A MACHINIST, HE WAS USING THE LATHE TO CUT METAL WHEN A STRINGER HIT HIS NOSE JUST AS HE WAS BENDING DOWN TO DISENGAGE THE CLUTCH. HE HAD TO HAVE 5 STITCHES PUT IN THE END OF HIS NOSE.

EE WAS MOVING A 4"" GUZZLER HOSE & FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE WAS INVOLVED IN A PHYSICAL ALTERCATION WITH ANOTHER EMPLOYEE. EMPLOYEE HAS A BRUISE ON CHEST AND LOWER BACK PAIN.

THE EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE INSTALL BUCKETS ON THE 2117 BUCKET ELEVATOR, WHEN EMPLOYEE FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE STEPPED IN A HOLE AND TWISTED HIS KNEE. HE VISITED THE DOCTOR ON 2/13/04. HE WAS RELEASED TO FULL DUTY THE SAME DAY AND NO MEDICATION WAS PRESCRIBED. HE HAD AN MRI ON 3/4/04 AT WHICH TIME HE WAS PLACED ON LIGHT DUTY FOR A MENISCAL TEAR TO THE LEFT KNEE.

EE WAS WORKING ON SECONDARY CRUSHER. EE TRIPPED OVER OXY-ACETYLENE HOSE AND LANDED ON HIS LEFT SHOULDER. ACCIDENT HAPPENED ON 11-3-03, DID NOT GO SEE DR UNTIL 11-25-03.

THE EMPLOYEE WAS MOVING A MOTOR & GEARBOX. THE GEARBOX SHIFTED PINCHING THE EMPLOYEE'S FINGER BETWEEN THE GEARBOX AND MOTOR. SMASHED THE LEFT MIDDLE AND RING FINGER.

EMPLOYEE WAS REMOVING A STEEL DOOR ON THE SHAKER SCREEN WITH A PRY BAR. THE DOOR CAME OFF AND SLID DOWN THE BAR AND STRUCK THE EMPLOYEE'S RIGHT INDEX FINGER.

EE WAS STANDING ON A PIECE OF IRON WHILE FABRICATING ASSEMBLING A PLATORM. EE WAS TIED OFF THE ANGLE IRON HE WAS STANDING ON GIVE WAY (WELD BROKE) EE CAUGHT HIMSELF WITH HIS ARMS AND STRAINED THE MUSCLES IN HIS BACK

EE WAS DISMOUNTING THE WATER TRUCK AND CAUGHT HIS HEEL ON THE STEP. THIS CAUSED THE EE TO FALL & UPON LANDING THE EE TURNED HIS ANKLE; SPRAINING HIS ANKLE.

EE WAS HELPING PLACE A GEAR SPROCKET ON A SHAFT AND FELT PAIN IN THE LEFT ARM AND SHOULDER. WAS ON #1 KILN DRIVE CHAIN IN THE COOLER BASEMENT OF THE MILL.

EMPLOYEE WAS WORKING ON #2 COAL MILL, THE WIND BLEW DUST/DIRT INTO EMPLOYEE'S FACE, DRIFTING BEHIND EMPLOYEE'S PRESCRIPTION GLASSES AND CONTACTING HIS LEFT EYE. TAKEN TO DOCTOR 3-27-03.

EE WORKING ON AIRSLIDE (LAYING ON HIS SIDE). RAW CEMENT DUST DROPPED FROM THE AIRSLIDE HITTING EE ON THE SIDE OF THE FACE. A PARTICLE SLIPPED BEHIND SAFETY GLASSES AND INTO EE RIGHT EYE.

EMPLOYEE WAS SHOVELING CLINKER DUST INTO WHEELBARROW. THE DUST DRIFTED BEHIND EE'S SAFETY GLASSES AND CONTACTED HIS LEFT EYE. DOCTOR PRESCRIBED AN ANTIBIOTIC EYE DROPS. EE DID NOT REPORT INJUR Y UNTIL 9 AM 1-24-03.

EMPLOYEE WAS USING CHAIN FALLS & LIFTING LINERS WHEN HE FELT PAIN IN HIS SHOULDER. THE DOCTOR DIAGNOSED A STRAIN OF THE SHOULDER. THE PAIN CONTINUED & THE DOCTOR SENT HIM TO PHYSICAL THERAPY O N 12/3/02 FOR 3 TREATMENTS & PUT HIM ON RESTRICTED DUTY UNTIL A MRI COULD BE TAKEN.

""REPORTABLE HEARING LOSS"". NOISE INDUCED HEARING LOSS IN ACCORDANCE WITH PART 50. AN AVERAGE CHANGE IN HEARING (FROM BASELINE) OF 25 DB OR MORE. HE WILL HAVE A RETEST WITHING 30 DAYS.

EE WAS GOING UP STAIRS, HIS FOOT SLIPPED ON THE LAST STEP. HE FELL DOWN AND HIT HIS KNEE ON GRATING. HE WILL BE TAKING PHYSICAL THERAPY 3 TIMES A WEEKS FOR 2 WEEKS.

EE WAS WORKING WITH ANOTHER EE ON A DRAG CHAIN. THEY WERE PUTTING A DRIVE CHAIN ON A SPROCKET WHEN HE FELT A PAIN IN HIS LOWER BACK. HIS BACK DID NOT HURT UNTIL THE WEEKEND. HE WENT TO THE DR. ON MON. 3/18 AND HE IS NOW ON RESTRICTED DUTY AND TAKING PHYSICAL THERAPY 3X A WEEK FOR 3 WEEKS.

EE AND OTHER EE WERE PUTTING LINERS IN A FINISH BALL MILL. THE LINER ABOUT 250 LBS WOULD NOT FIT IN THE HOLE, SO THEY PULLED IT BACK OUT TO MOVE IT OVER WHEN AN EE FROM INSIDE OF THE MILL USED A BAR TO PRY THE LINER OVER. THIS CAUSED THE ROLLER CONVEYOR TO MOVE AND THE WEIGHT OF THE LINER SHIFTED AND IT FELL ON THE EE LEFT FOOT STEEL TOE BUT CRACKED THE TOE.

EE WAS WORKING ON A COOLER DRIVER WHEEL (ABOUT 12"" DIA. SOLID STEEL). THE WHEEL WAS STUCK ON THE SHAFT AND AS HE PULLED ON IT THE WHEEL CAME OFF AND LANDED ON HIS LEFT LITTLE FINGER SMASHING IT. HIS FINGER WAS NOT BROKEN BUT IT TOOK 3 SUTURES TO FIX IT.

EE WAS WALKING DOWN 2 STEPS WHEN HE REACHED TO BOTTOM ON LEVEL GROUND HE TRIPPED. HE FELL WITH HIS RIGHT ARM EXTENDED OUT. WHEN HE LANDED HE DISLOCATED HIS RIGHT SHOULDER.

EE WAS USING A ROUND BAR TO POKE ON A FEED BIN FEEDER. THERE WAS A CHUNK IN THE FEEDER & WHILE POKING HE HIT THE SIDE OF THE BIN. HIS HAND SWELLED UP & X-RAY WAS TAKEN. HE HAD AN OLD ""BOXER"" BREAK THAT COULD POSSIBLY BE CRACKED BUT NOT KNOWING HOW.

EMPLOYEE WAS WORKING WITH OTHER EMPLOYEE ON CHANGING OUT A BAD KILN TRUNION WHEEL. THE WHEEL WAS IN PLACE AND THEY WERE PUTTING THE CAPS ON WITH THE CRANE WHEN ONE OF THE CAPS SLIPPED CATCHING HIS FINGER UNDER THE CAP. THIS BROKE HIS SMALL FINGER IN HIS LEFT HAND.

EE WAS WORKING ON A GARDNER DENVER DRILLAT THE QUARRY. HE WAS WALKING AROUND THE DRILL WHEN HIS HEAD HIT THE CORNER OF A TOOL BOX WHICH WAS ATTACHED TO THE DRILL. THE IMPACT OF THE TOOL BOX AG AINST HIS FOREHEAD CAUSED HIM TO RECEIVE 16 STITCHES TO HIS HEAD.

EE WAS CLOSING BOTTOM COAL CAR DOORS WITH A LONGBAR. WHILE PULLING ON THE BAR IN A BENT OVER POSITION, HE FELT A PAIN IN HIS BACK. HE RECEIVED ALUMBAR SPRAIN & HAS PHYSICAL THERAPY 6 TIMES FOR THE NEXT 2 WEEKS.

AN EE WAS SITTING ON THE FLOOR IN FRONT OF A BALL MILL. HE HAD A 50 GAL DRUM LAYING ON IT'S SIDE WITH STEEL BALLS SPINNING OUT OF IT. THIS EE WAS THROWING 3"" STEEL BALLS INTO THE MILL. THE VIC TIM CAME UP AND SAT DOWN ON THE 50 GAL DRUM AND LOOKED OVER INTO THE DRUM JUST AS THE EE WAS THROWING A BALL INTO THE MILL. THE BALL HIT THE VICTIM ON THE NOSE BREAKING THE NOSE.

EE WAS USING HIS POCKET KNIFE TO CUT A PLASTIC BAND ON A KEYSTONE VALVE. THE KNIFE SLIPPED AND HE CUT THE TOP OF HIS THUMB ON HIS LEFT HAND. HE HAD TO HAVE 3 STITCHES IN HIS THUMB.

WHILE WORKING AT THE PLANT, EE REPORTED THAT HE WAS WEARING PERSONNEL PROTECTIVE DEVICES (CONFIRMED BY A WITNESS) & DID NOT RECALL THE SPECIFIC INSTANCE WHEN THE FOREIGN OBJECT ENTERED HIS EYE . HE INITIALLY THOUGHT INJURY WAS DUE TO A FLASHBURN & DIDN'T SEEK MEDICAL ATTENTION UNTIL FEB. 15. DIAGNOSIS WAS A SMALL PIECE OF METAL IN EYE.

TWO WORKERS WERE AT THE PIRANHA METAL CUTTER TRYING TO CUT A PIECE OF METAL. THIS WAS A METAL STRAP 12""LONG BY 4""WIDE BY 1/4""THICK. THEY WANTED TO CUT THE STRAP IN HALF THE LONG WAY WHICH WOUL D GIVE THEM 2 EA PIECES 2"" X 12"". THEY PUT THIS STRAP INTO THE PIRANHA BUT IT WOULD NOT SIT ON THE CUT LINE. THE STRAP WAS TEETERING SO HE TOLD HIM TO STEADY THE STRAP WHILE HE BROUGHT THE CU

EMPLOYEE WAS CUTTING SOME METAL WITH ARCH, CUTTING ROD AND THE SPLATTER CAUSED HIS SHIRT TO CATCH ON FIRE. HE RECEIVED 2ND DEGREE BURNS ON HIS BACK AND UNDER HIS LEFT ARM.

EE WAS THROWING 3"" MILL BALLS INTO A CHUTE & HITHIS LEFT HAND ON THE CHUTE. HE CUT HIS LITTLE FINGER & THEY PUT 5 STITCHES IN HIS FINGER HE WAS NOT WEARING GLOVES AT TIME OF ACCIDENT

EE WAS WORKING AT NOCC DURING KILNSHEET DOWN. HE WAS REMOVING REFACTORY FROM THE KILN WITH A 90LB JACKHAMMER & CHISEL BIT. THE BIT BROKE IN HLF CAUSING EE TO PITCH FORWARD WHILE HOLDING ONTO THE JACKHAMMER . THE JACKHAMMER THEN PINCHED HISFINGER BETWEEN THE WALL & ITSELF. RESULTING IN A BONE BRUISE ON HIS FINGER

ENJURED EE WAS HOLDING A WRENCH ON A NUT WHILE ANOTHER EE WAS TIGHTENING THE BOLT. THE WRENCH SLIPPED WHILE TIGHTENING THE BOLT AND HIT THE INJURED EE ABOVE THE LEFT EYE. THEY WERE WORKING ON CRUSHER LINERS AND WERE IN CLOSE QUARTERS TO EACH OTHER. THE INJURED EE HAD TO HAVE 3 STITCHES ABOVE THE EYE.

EMPLOYEE WAS MOVING A LARGE MOTOR FROM PALLET TOPALLET AND FELT A PAIN IN HIS LOWER BACK; 5/8/00HE WENT TO DR. AND AFTER CHECKUP, WAS ASKED TO RETURN ON 5/10/00 FOR POSSIBLE THERAPY. WHEN HE RETURNED, DR. HOSPITALIZED HIM; HE WAS OPERATED ON FOR A BROKEN DISC IN HIS BACK ON 6/12/00 AND WILL BE OFF WORK FOR SOME TIME.

EMPLOYEE WAS WORKING ONT EH MUFFLER OF A PETER BUILT TRACTOR. HE AHD TH EPASSENGER SIDE DOOR OPEN AND STANDING IN THE DOOR WAY PUTTING THE TOP CLAMP ON. HE STARTED TO GET DOWN WHEN A RUBBER BOOT FROM INSIDE THE CAB FELL DOWN UNDER HIS FEET CAUSING HIM TO TRIP AND FALL TO THE GROUND. HE SPRAINE DHIS RIGHT ANKLE, LANDED ON HIS LEFT BUTTOCK AND BROKE HIS LEFT WRIST.

EE AND OTHERS WERE PICKING UP BRICK TABLE(PLATFORM) WHEN HE NOTICED A PAIN IN HIS LOWER STOMACH. IT DID NOT BOTHER HIM AT THAT TIME AND HE DID NOT THINK ANYTHING OT IT UNTIL 3-9-00 THAT IS WHE N HE NOTICED A PART OF HIS BODY STICKING OUT AROUND HIS UMBILLICAL AREA AND A DULL PAIN WITH IT. HE WAS OPERATED ON FOR A HERNIA ON 4-14-00 HE LOST 4 DAYS AND IS NOW WORKING ON RESTRICTED DUT

EE WAS USING A 2X12 PLANK TO STAND ON TO CLEAN THE SIDE OF A HOPPER. THE PLANK SLIPPED AND HE HURT HIS BACK. HE WILL BE TAKEN PHYSICAL THERAPY FOR 3 TIME PER WEEK FOR 2 WEEKS.

EE WAS REPORTING FOR WORK. HE WAS CARRYING OBJECTS IN BOTH HANDS WHEN HE SLIPPED AND FELL ON AN ICY PARKING LOT. THE LOT HAD BEEN CLEARED OF SNOW EARLIER BUT IT CONTINUED SNOWING DURING THE DA Y.
