Was around Fines Plant and told an EE that self could not breathe. Another EE and another ee came over to ee and noticed ee was spitting up and told them ee could not breathe. They put in another ee truck and we transported ee to the local hospital.

Employee called me on July 31, 2008 and stated that on July 28, 2008 he was using the Port-a-John at our Mill Spring Quarry and noticed spiders in the facility. He said he killed them, then by Tuesday evening noticed some swelling on his left side of the trunk. This swelling had gotten worse by Wednesday evening. He said he thought he had been bitten by a spider.

Employee was found slumped over in scale house by two customer truck drivers. He had been seen approximately 5 minutes before. 911 was called and Polk County NC EMS arrived by 9:00am. They treated the employee for approximately 20 minutes however he could not be revived.
