The employee was at the pump station, to load water truck. The employee was using a pocket knife to cut a garden hose when employee's hand slipped and employee stabbed self in the opposite hand. The left hand required stitches.

The injured employee was at the washing plant to clear debris from the catwalk and screening surface. While walking on the catwalk employee slipped on ice and fell.
As EE and other employees where lifting the plates off the back of the service truck using a jib crane EE got EE's right thumb between two plates and it was mashed.
While cleaning out a blockage in the surge tunnel a large rock fell against the pry bar employee was using causing it to hit employee's chin.

It was the start of the shift and employee was going up the steps to enter the control room for the start of the shift. As they reached the top step they turned to talk with a coworker. As they turned they slipped backwards and Right foot became caught between the stairs. This prevented them from falling down the stairs, but it injured the foot.

Was around Fines Plant and told an EE that self could not breathe. Another EE and another ee came over to ee and noticed ee was spitting up and told them ee could not breathe. They put in another ee truck and we transported ee to the local hospital.

Our maintenance mechanic was doing pre-operational inspection and preventative maintenance on a Hitachi EH 1100-5 haul truck on the morning of Wednesday, April 19, 2017. As ee was exiting the equipment ee slipped on the bottom step and fell to the ground. Landing on right side causing pain to this area.

Employee performing maintenance duties on the morning of the 9th. At lunch complained of being nauseous and lite headed. Went to quarry office and used restroom, then came out and fainted and fell to the floor.

Employee went into the Shaker Screen to flip over some gates. He had his hand located at the top of the gate and was shaking out some rocks that had become lodged in the gate. The rocks fell and the metal gate fell on his left hand.

Employee was helping remove the cover from the shut that allows materials to flow from the 8x20 Telsmith Sizing Screen to the #15 conveyor. As he was lifting the left side of the cover he lost is balance and fell forward into the cover. He hid his mouth on the cover as he fell.
Welding burning ring an nut on cone crusher at the Miller Creek Quarry. After one weld employee move over to start another weld. Did not realize that he had put his left leg on the new weld and caught pants on fire.

The employee was operating our hydraulic hammer in the pit at the Miller Creek Quarry. A piece of rock he was breaking entered the cab on the right side through the side glass. It hit him in the right eye.

Employee was changing an OTR tire in the parking lot of Thompson Cont.. when he got his finger caught between the clamp assembly and rim of the tire. Employee broke his right ring finger
EE was replacing side rails on the 6x16 Seco Shaker Screen.He was holding a bold head with his right hand and using an impact wrench to tighten a nut on the bolt.EE engaged the nut on the bolt turned catching his glove and wrapping around his hand. The bolt head cut through the glove and cut his right 3rd. finger.

Injured employee was helping with maintenance work on our primary jaw. They had disassembled the feeder rods and were removing the bolts from the washers. He was holding a bolt when another employee slipped and hit his thumb with a hammer.

Two employees were dislodging a rock that had become stuck on conveyor #1. One employee moved the rock and the injured employee's left thumb was pinched between the rock and conveyor frame. This caused a laceration which required stitches.
