Loader Operator came to work Monday morning. Loader was sitting still and operator was laying back in cab of loader. Operator suffered major heart attack per rescue squad personnel. MSHA was called on Monday October 15, 2018 and was assigned to Field Office. MSHA representative called and spoke with Quarry Superintendent. We are filing information.

Standard threshold shift of more than 10 db due to miner's lack of use of PPE. *Amended should be resinded shift was not 25 DB total shift, was 10 DB when age correction was applied. Corrected to a call from field office supervisor after contact with company that did testing*.
