Bodily reaction, (Not Elsewhere Classified)

Cleaning window (Plexiglass) of Rock Breaker. Injured party states was cleaning window with left arm/hand bending down to reach bottom of window and heard a pop sound in shoulder.

Struck by falling object

The jack used to lift dump truck off ground broke causing tire to land on mechanic left foot behind steel toe insert and bruised foot.

Injured party states EE was putting a conveyor belt on the back stop at the primary crusher and while leaning forward pulling heard a pop on EE's right side of EE's rib cage.

Backing off of a rock breaker machine a boulder with sharp edge cut the back of the right calf of our employee.

Mechanic states was trying to loosen nut on hydraulic line and when nut gave way from force mechanic fell down with left hand exposed trying to brace fall and hurt hand.

Injured party states they had turbo in hand and as they are walking around engine being rebuilding, catches a foot on engine stand, looses balance, falls to shop floor and hits left cheek and cuts left pinky.

Employee was walking in between pallet on floor and a forklift. Employee got to close to pallet which was holding a crankshaft and end made a gash in employee left shin requiring stitches.

Injured states was walking in shop area when injured stepped on a hose and the hose rolled out of injured's foot and caused injured to twist left side.

Injured states EE was trying to get pin out of conveyor leg to lower. When EE was successful in getting pin out conveyor leg folded and pin right hand causing some bone to be fractured.

Loader Operator came to work Monday morning. Loader was sitting still and operator was laying back in cab of loader. Operator suffered major heart attack per rescue squad personnel. MSHA was called on Monday October 15, 2018 and was assigned to Field Office. MSHA representative called and spoke with Quarry Superintendent. We are filing information.

Injured party states EE was trying to pick up 3 x 3 sheet of metal and when EE raised up heard a pop sound in EE's lower back.

Injured party states EE was grinding on cab shield in truck bed. After EE was done EE was climbing out through tailgate and ladder that was propped up kicked out from gravel and injured party landed on EE's right side bruising EE's ribs on bed of dump truck.

Contact with heat

It was a seasonally humid day. Mechanic working on mobile equipment. As EE carried jugs of antifreeze to equipment to add EE stopped and got a drink of water. After carrying jugs to equipment EE came back in to get water bottle and as EE was walking out door mechanic blacked out and fell to ground.

Scale person was standing and turned quickly and right ankle rolled to the right.

Injured states ee was installing a starter on work truck. EE was underneath truck when ee was pushing self up by elbow to reach the top bolt of the starter. This caused left elbow to develop a fluid sack.

Injured states employee was grinding slag off metal washer and a piece came back at individual and got in employee's eye

Shift was over and employee was getting out of excavator. Employee became dizzy standing on tracks and fell backwards onto ground.

Injured states was climbing on equipment using handrail to pull self up approximately 4 to 5 feet off ground when heard a pop in shoulder.

Injured individual along with another individual was changing impact roller. Each individual on side of belt prying up roller to flip and when roller flipped caught injured individual on middle index finger at tip on left hand smashing tip.

Replacing impact bars and one of the bars came down and struck employee on right foot. Employee was wearing steel toes shoes but still caused contusion to right foot.

Injured states he was scraping paint off dump bed of truck, then sweeping mess off floor when a foreign object went into the eye. Injured states he tried washing it out at quarry but would not come out.

An empty train traveling from Winchester, VA. to Gore, VA struck an OTR Delivery Dump Truck at the Quarry Grade Crossing. Dump Truck Driver stated he did not see or hear the train until he was on the tracks. (Driver was listening to I-Pod with earphones in ear)

WHILE INJURED WAS HAMMERING ON PINS, A PIECE OF THE PIN (STEEL) BROKE OFF AND WENT INTO HIS LEG (2-3 STITCHES-TETANUS SHOT). PIECE OF STEEL IS STILL IMBEDDED IN LEG-MAY OR MAY NOT REMOVE LATER.
