THIS ACCIDENT DID NOT BECOME LOST TIME UNTIL 10/03/2011!! The employee was cleaning the shop (sweeping) when he bent over to pick up a metal shaft and felt a pain in his back. The employee continued to work his regular shifts and job task until he called in on the morning of 10/03/11 and said an MRI showed a blown disc.

Employee alleges that he was performing work while kneeling on knee pads. When he stood up he felt a cramp behind his left knee. Employee was able to return to work with normal duties. His injury became reportable as lost time on 2/4/11.

EE was to go to the 4A inlet duct to clean out material from the bottom of the duct. The men working at that location reported starting the cleanup at approx. 6:00 am. At approx 6:45 EE lost consciousness falling forward to the surface as he was sitting and shoveling material down the ducts slope.

EE WAS REPAIRING SOME OF THE FLOOR DECKING ON THE PREHEAT TOWER. DURING THIS TIME THERE WERE SWIRLING GUSTY WINDS THAT BLEW SOME DUST PARTICLES INTO HIS RIGHT EYE, EVEN THOUGH HE WAS WEARING H IS SAFETY GLASSES WITH SIDESHIELDS.

A FELLOW WORKER FOUND HIM THERE AT APPROX. 1PM & CALLED FOR ASSISTANCE.THE EE NEVER LOST CONSIOUSNESS, SHOWED NO SIGNS OF TRAUMA. DIAGNOSED AS A PERSONAL ILLNESS.

THE INJURED WORKER HAD JUST FINISHED MOVING THE CLINKER CONVEYOR TRIPPER AND WAS DESCENDING A SHORT LADDER TO THE CATWALK BELOW WHEN RAIN STARTED TO FALL ON THE ROOF CAUSING DUST THAT HAD SETT LED ON HORIZONTAL SURFACES TO SHAKE LOOSE. HE LOOKED UP AS HE HEARD THE RAIN HIT THE ROOF. AT THAT MOMENT DUST FELL AND HIT HIS FOREHEAD JUST ABOVE HIS SAFETY GLASSES. DUST GOT INTO HIS EYE.

THE INJURED WORKER AND OTHERS WERE PUTTING A LARGE TARP OVER A RAILCAR AS RAIN BEGAN TO FALL. THERE WERE STRONG BLUSTERY WINDS THAT LIFTED AND JERKED THE TARP VIOLENTLY. THE EE TRIED TO HOLD T HE TARP DOWN BUT IT PULLED HIS ARM UPWARD AND SPRAINED THE RIGHT SHOULDER BEFORE HE LET GO OF THE TARP. THE DR PUT HIM AN RESTRICTED DUTY AND MEDICATIONS.

THE EE WAS PERFORMING HIS NORMAL DUTIES AS A PREHEAT TOWER LABORER ON THE 2ND FLOOR OF THE PREHEAT TOWER. A GUSTING WIND BLEW A PARTICLE OF DUST INTO HIS LEFT EYE. FLUSHING THE EYE OUT BROUGHT NO RELLER, SO HE WENT TO THE NEARBY HOSPITAL EMERGENCY ROOM. THEY ALSO FLUSHED OUT HIS EYE AND WERE ABLE TO GET THE PARTICLE OUT.
