WEATHER CONDITIONS WERE VERY WINDY AT SOME POINT A METAL SLIVER BECAME EMBEDDED IN EE S RIGHT EYE IT BECAM E IRRITATED OVERNIGHT, MEDICAL ATTENTION DETERMINED THE CAUSED AND EE HAD SLIVER REMOVED FROM EYE BY OPTHOMOLOGIST ON 4-7-94. NO PERMANENT DAMAGE NOTED.

EMPLOYEE WAS CLIMBING LADDER ON LOADER TO GET IN THE LOADER, LOST HIS BALANCE & HIT HIS KNEE ON RUNG OF LADDER. HE RETURNED TO WORK OF 7/13/93. ON 10/1/93 HIS KNEE GAVE OUT AS HE WALKED ACROSS THE ROOM & HE IS AGAIN UNDER DOCTOR'S CARE AS A CONTINUATION OF THE ORIGINAL INJURY OF 7/7/93.

EMPLOYEE DEVELOPED A SORENESS IN HIS HEEL ON 1-12-89. THINKING IT WAS A PREVIOUS INJURY HE DID NOT REPORT IT. FEB. 7, 89 HE STARTED DISABILITY WITH A DOCTOR REPORT STATING IT WAS NOT WORK RELA TED. FEB. 24, 89 THE DOCTORS REPPORTED THAT IT WAS POSSIBLY A NEW INJURY AND COULD BE WORK RELATED.
