Showing all 4While operating a motor grader, an employee started having shortness of breath and chest pains. The employee was taken to a medical facility for treatment. Diagnosis revealed the employee had a pre-existing medical condition not related to an occupational injury or illness.
Contractor employee sustained a laceration to the forehead when struck by a cyclone gate during a severe storm event and related power outage. Gate unexpectedly blew open striking the employee as he was exiting the pond enclosure. Laceration required suturing by a medical professional. All appropriate PPE was being worn.
WHILE UNLOADING A PNEUMATIC BULK TRAILER WITH A TRUCK TRACTOR, HE FELT DEHYDRATED AND ASKED OTHE EE TO FINISH UNLOADING THE TRAILER WHILE HE SAT INSIDE OF THE TRUCK CAB. OTHER EE COMPLETED THE UNLOAD WITHIN 15 MINUTES. UPON CHECKING ON DRIVER IN CAB, HE FOUND HIM LAYING ON FLOORBOARD UNCONSCIOUS. EMTSWERE SUMMONED FROM MINE, THEY COULD NOT REVIVE HIM.
