Employee was cutting drip line on the leach pad with a box cutter. The knife slipped and cut EE's lt. Wrist. There was profuse bleeding reported by the co-worker and they helped apply pressure and elevation as they assisted the injured worker to the aid station. Arterial surgery repairs were performed at the emergency hospital.

Another employee was removing teeth from a 992K loader. While using a hammer to break the teeth loose it chipped the tooth causing a piece of metal to go flying which then hit IE in the chin. This broke the skin open and imbedded into IE's chin.
Employee had a personal medical accident. EE was driving a company vehicle and had a seizure and ran the vehicle into a fence at the warehouse stock yard.

It has been muddy on the mine. The operator was shoveling the tracks of the dozer and was pushing hard. EE then felt something in the right knee pop.
Employee was changing screens at the crusher and while moving the screens they hurt their side.
Miner was removing loader teeth off a bucket using a 12lb sledge hammer when a piece of metal flew off and lodged into EE's left leg. The leg immediately bruised and slightly bled. The miner was treated onsite and released early to seek further medical attention.

Employee was shoveling precipitation in the refinery when they twisted and felt pain in lower back.
On 24 Aug. 2020 employee noticed an irritation to right eye, employee did not report incident and went home. On 25 Aug. 2020 employee's eye was still irritated, employee reported incident to the supervisor, attempted to flush eye out but could not do so. Employee was taken to Fallon, NV where eye was examined and found to be scratched, employee given prescription medication.
Employee was descending stairs on the Sandvik Drill when they slipped and fell against the large mammoth drill wrench causing a bruise and pain to their left rear rib cage, employee was taken to Banner-Churchill Hospital, attending physician refuse to give employee a release and told them to return to work ""when they felt like it"" no medication or shots given
Employee was unwinding a 1"" cable that had turned into itself, cable sprung loose catching the employee's left thumb causing a sprain, employee seen by ER, issued a brace for the thumb and returned to work.
Employee was working along the belt line when they slipped in some mud, fell and struck right knee causing pain. Employee stated they did not want or need medical treatment at that time however on 07 Feb 20 went to the ER for complaint of pain to right knee, employee was given a full release to work at that time.

Employee was walking to the portable toilet when a gust of wind is alleged to have caused employee to loose their balance and sprain their right ankle, employee did not report incident until 29 Sept.2019 when they called off work due to pain in the right ankle, employee did not see a doctor until 30 Sept.2019 when they again reported off work.
Employee was removing a bar cart away from the furnace when the wheel of the cart became stuck, employee pulled on cart to free it, the cart went to the left and caught employee's left little finger between furnace frame and the edge of the cart, finger was fractured and wound required stitches.
Employee was removing guard from roller on crusher, struck guard with hammer causing guard to spin and strike employee on bridge of nose and side of face, employee given injection due to complaint of pain.
Employee was shoveling-cleaning the ""tunnel"" at the crusher when they experienced pain in the groin area, employee suffered a hernia/rupture. Employee was released to light duty.

Employee attempted lift one end of cribbing from a standing position. While lifting employee twisted and felt tightness in lower back. Upon doctor's visit was diagnosed with mild back strain and given injection of muscle relaxer. Told to follow up with personal physician if pain persisted.

He was released to full duty the next morning before completing his scheduled shift. Injury reportable due to RX for eye meds. There were no modified or lost days.

Injured employee was lowering the man-basket on a telescoping mechanical lift and pinched his small finger on his left hand between the handrails and a fixed lifting eye. Employee referred for medical treatment(2 stitches) for a laceration. Employee treated and immediately returned to full duty.
Refiner was pulling a precip tray out of the retort and strained his lower abdominal muscle. Employee didn't report injury until 9/13/13 and was referred for medical treatment on 9/16 and was subsequently placed on modified work.
While operating a motor grader, an employee started having shortness of breath and chest pains. The employee was taken to a medical facility for treatment. Diagnosis revealed the employee had a pre-existing medical condition not related to an occupational injury or illness.
The drilling crew was performing maintenance on the tophead swivel. The driller helpers hand was pinched/lacerated by the swivel. He was taken to local emergency room for 2 stitches on the back of his hand. He returned to work directly from the ER.

Operator was descending the access steps/ladder of an articulating haul truck. When his right foot reached the bottom rung, his right knee hyper-extended and he fell to the ground. This miner had a history of problems with his right knee including hyper-extending it in 2009. Miner experienced a bone chip that will need to be removed by surgery.
Miner does not know how or when he sprained his left ankle. He worked the day of the injury and the following day, but he could not work on August 11th due to pain and swelling of his ankle.
A driver lost control of his haul truck resulting in a tip-over. According to the driver, he was wearing his seat belt, yet he stated that he impacted the left side of the cab. The driver stated that he had pain on his left side, left lower back and left shoulder. No external bruising, or abrasion was visible. X-ray and CT scan showed no internal injuries.

Contractor employee sustained a laceration to the forehead when struck by a cyclone gate during a severe storm event and related power outage. Gate unexpectedly blew open striking the employee as he was exiting the pond enclosure. Laceration required suturing by a medical professional. All appropriate PPE was being worn.

OPERATOR WAS TRYING TO PUT AIR CONDITIONER DIRECTIONAL COVER BACK IN PLACE AT THE SAME TIME AS HE WAS OPERATING THE LOADER. THE COVER SLIPPED & HIS LEFT HAND ENTERED THE OPENING. HIS LEFT RING FINGER CAME IN CONTACT WITH THE SQUIRREL CAGE FAN, LACERATING THE FINGERPRINT PORTION OF THE FINGER & ALSO RESULTING IN AN OPEN DISTAL TUFT FRACTURE ON THE END OF THE FINGER.
EE WAS PULLING A GREASE HOSE OFF A REEL TO GET ENOUGH TO REACH A PULLEY. WHEN ANOTHER EE PULLED ON THE LINE TO HELP THE EXTRA SLACK CAUSED THE EE TO STUMBLE. HE WENT FORWARD TO TRY TO REGAIN H IS BALANCE. HE THEN STEPPED ON A ROCK AND FELL TO THE GROUND AND TWISTED HIS RIGHT SIDE. COMPANY WAS NOT AWARE UNTIL 9/19/02.
EMPLOYEE HAD JUST POSITIONED THE TRACK ADJUSTER CYLINDER IN THE SWING FRAME WHEN IT SLIPPED AND SMASHED HIS FINGER BETWEEN THE CYLINDER AND THE TRACK RAIL.
EE HAD LOADED TWO ZINC CANS ONE ON TOP OF THE OTHER ONTO A HANDTRUCK TO MOVE THEM TO THE ZINC FEEDER APPROX. 20 FT AWAY. WHEN HE LEANED THE HANDTRUCK BACK THE CANS STARTED TO FALL FORWARD. HE REACHED OUT AND STOPPED THE CANS FROM FALLING BUT STRAINED HIS BACK IN THE PROCESS.
EE WAS CLIMBING DONW OFF OF THE LUBE TRUCK AND GRABBED THE RAIL WITH HIS LEFT HAND. WHEN HE WENT TO GRAB THE NIPPLE OF THE GREASE TANK WITH HIS RIGHT HAND HE MISSED HIS POINT OF CONTACT AND FE LL BACKWARDS LOSING HIS GRIP AND FALLING APPROX.FT TO THE GROUND LANDING ON HIS RIGHT ARM AND CAUSING A FRACTURE ABOVE THE WRIST.
EE WAS PUSHING THE RIGHT FORK OF THE GRADALL TOWARDS THE CENTER OF THE ATTACHMENT. WHILE HE WAS PUSHING THE FORK IT SWUNG BACK ONTO HIS LEFT HAND CATCHING IT BETWEEN THE FORK AND THE FRAME OF THE ATTACHMENT.

THE EMPLOYEE WAS FABRICATING A LOCKER. SOMETIME DURING THIS A PIECE OF CINDER LODGED IN THE EMPLOYEE'S EYE. AT 3:00 PM THE EMPLOYEE NOTICED THAT HIS EYE WAS RED AND IRRITATED. ON MONDAY HE THO UGHT THAT HE HAD RECEIVED A WELDING BURN AND WAS USING WELDERS EYE DROPS TO RELIEVE THE IRRITATION AND TOLD HIS SUPERVISOR THAT IT FELT A LITTLE BETTER. BY THURS. HIS EYE WAS STILL SORE & RED
WHILE UNLOADING A PNEUMATIC BULK TRAILER WITH A TRUCK TRACTOR, HE FELT DEHYDRATED AND ASKED OTHE EE TO FINISH UNLOADING THE TRAILER WHILE HE SAT INSIDE OF THE TRUCK CAB. OTHER EE COMPLETED THE UNLOAD WITHIN 15 MINUTES. UPON CHECKING ON DRIVER IN CAB, HE FOUND HIM LAYING ON FLOORBOARD UNCONSCIOUS. EMTSWERE SUMMONED FROM MINE, THEY COULD NOT REVIVE HIM.
EE WAS REMOVING A BELT CLAMP FROM THE BACK OF A P/U BED. THE EE FELT A SHARP PAIN IN HIS SHOULDER WHEN HE LIFTED THE CLAMP AND DROPPED IT. THE EE REPORTED THIS TO HIS SUPERVISOR AND SAID HE TH OUGHT HE WOULD BE OKAY. THE NEXT DAY THE PAIN PERSISTED AND HE WAS TAKEN TO THE DOCTOR. ON MARCH 9 THE EMPLOYEE AGAIN WENT TO THE DOCTOR AND WAS GIVEN A CORTIZONE INJECTION.
EMPLOYEE WAS TRIMMING AWAY THE TELLOWMINE PIPE WALL TO EXPOSE A BROKEN SPLINE FOR REMOVAL. EMPLOYEE HAD JUST CHANGED THE BLADE IN HIS RAZOR KNIFE. DURING THE TRIMMING OF THE WALL THE BLADE SLIPPED OFF THE PIPE AND LACERATED THE FINGER.
EE WAS TIGHTENING A PIECE OF UNISTRUT CONDUIT FIXTURE DOWN WHEN THE COUPLING STRIPPED THROUGHT THE THREADS & THE UNISTRUT FELL KNOCKING HIM BACKFROM THE LADDER. HE GRABBED FOR ANOTHER PIECE OF CONDUIT THAT HAD A ROUGH EDGE WHICH CAUSED THE INJURY.

EE WAS REMOVING THE DUST HOOD FOR SC-01. THE HOOD BOUND UP AND WHILE THE EE WAS PUSHING ON IT THE HOOD CAME LOOSE. THE EE FELL THROUGH THE CHUTE AND STRUCK HIS SHIN CAUSING THE LACERATION.
TWO EMPLOYEE'S WERE REMOVING THE BAFFLE PLATE TO POSITION IT DIFFERENTLY. AS THEY WERE CUTTING THE PLATE LOOSE EE PULLED ON IT AND THE PLATE CAME LOOSE. A STIFFNER BRACE ATTACHED TO THE PLATE STRUCK EE'S ARM AND CAUSED THE INJURY.
EE WAS PLACING A 65 LB DOR BAR IN THE SAFE AND THE BAR STARTED TO TURN OVER. WHEN THE EE TRIED TO CATCH THE BAR HIS FINGER WAS PINCHED BETWEEN A SHELF IN THE SAFE AND THE DORE BAR CAUSING THE INJURY.
EMPLOYEE WAS USING A RAZOR KNIFE TO CUT 80 MIL. LINER. AS THE EMPLOYEE WAS CUTTING AWAY FROM HIMSELF THE KNIFE SLIPPED AND HE FELL FORWARD ONTO A CORNER OF THE FRESHLY CUT LINER. THE LINER P UNCTURED HIS LEFT WRIST CREATING A LACERATION (SMALL).
EE HAD STOPPED AT TR-06 TO FUEL AND LUBE. WHEN HE CHECKED THE RADIATOR FLUID LEVEL HE MOVED THE CAP TO THE FIRST CLICK AND NOTHING HAPPENED. WHEN HE MOVED THE CAP TO THE SECOND CLICK THE CAP B LEW OFF AND SPRAYED ANTI-FREEZE ON HIS RIGHT HAND CAUSING A SECOND DEGREE BURN.
EE WAS PARKED, WAITING TO BACK UP AND GET LOADED. HIS HAUL TRUCK WAS FACING THE HIGHWALL APPROX. 30 FT. FROM IT. A LARGE ROCK APPROX. 5 SQUARE FT. ROLLED DOWN THE HIGHWALL 180 FT. AND STRUCK T HE FRONT GRILL OF THE HAUL TRUCK. THE FORCE LIFTED THE REAR WHEELS 3 FT. OFF THE GROUND CAUSING THE EE TO STRIKE HIS FACE AGAINST THE STEERING WHEEL.
