contract labor engaged in cleaning activity...windy day...got foreign material in eyes...rubbed them repeatedly in disregard to both advice and instruction...washed out with eye specific solution...EE went to WRMC ER later that evening...ER staff flushed eyes...prescribed an antibiotic as a precaution...released EE to return to work with no restrictions 04/21/2018

EE was entering the plant oil house when a red wasp stung EE on the bridge of the nose. EE immediately began to swell. We transported EE to the local hospital where EE received steroids and medicine to counter inflammation through an IV. EE was released to return home soon after the swelling started going down. We are now aware of EE's allergy to certain insect stings.

EE was operating front end loader when he felt slight discomfort in chest & slight numbness in lt arm. He contacted supervisor & removed himself from loader. Ambulance was called for him to be transported to med. facilities. CPR was not performed, AED was not used & he remained conscious. Tests were ran & were negative for heart attack. He spent 1 night in the hospital

EE was simply stepping up concrete steps to gain access to the stone mill operating room when he felt a pull or tear in his lower left calf. Days later diagnosed as a pulled muscle following an MRI, resulting in light duty and prescription muscle relaxers to be followed by two weeks physical therapy. EE continues to perform his normal job each day.

Employee was working on top of bulk hydrate tank re-bagging a dust collector...he is very explicit in that he is unaware of what happened that resulted in his left elbow being swollen and hurting...he can't recall striking anything or being stung (wasp were in the area)...he has had fluid drawn from the elbow and is taking a antibiotic...he continues to work unrestricted.

as reported to supervisor 04/04/07 and to administration 04/13/07...while installing a slow speed indicator on a new bucket elevator during a period of high wind received lime irritation to eyes...sought medical attention 04/06/07...no lost time

EE was wearing his safety glasses. Was performing his bid job as a bagger. Wind started blowing rather hard for the 1st time in weeks & some foreign material got in his right eye irritating it. After 24 hours the redness didn't recede so he was sent to a doctor. Medication is working wonders as the redness is going away as of today 9/15/05. EE missed 9/19-20. Worsened over weekend.

DURING A SHUTDOWN EE WAS ASSISTING ANOTHER EE WITH MAINTENANCE/REPAIR TO THE UNIT II KILN COAL/COKE MILL AND THIS INCLUDED USE OF A CUTTING TORCH IN A TIGHT PLACE. WIND GUST BLEW SLAG BACK TOWARD EE FACE, WE WASHED THE EYE OUT ABOUT FOUR TIMES, LOOKED CLEAR.EE CONTINUED WORKING. WENT TO DOCTOR, REMOVED FRAGMENTS FROM UNDER THE EYE, NO ADDITIONAL PROBLEMS.

Contract repairman/welder had just ascended appx 100' high spiral staircase to top of storage tank w/coworker. They were planning to raise tools by rope. As victim bent over handrail to begin raising tools he told coworker he had hurting in side & stomach. He collapsed on platform he was standing on. Attempts to resuscitate failed. Coroner believes natural causes.

THIS EE WAS WORKING AS A BAGGER & WAS BAGGING HYDRATED LIME. THIS IS A TASK THE EE HAS BEEN DOING FOR MONTHS. IT WAS A VERY WINDY DAY & GOT SOME DUST/GRIT IN HIS EYES. THE EYES WERE IRRITATED EVEN FOLLOWING SEVERAL WASHINGS. EE WAS SENT TO DOCTOR TO HAVE HIS EYES CHECKED & ANY FOREIGN MATERIAL REMOVED. EE RETURNED TO WORK LOSING NO TIME.
