Mechanic was kneeling down on one knee and bent forward to turn a 10" gate valve. When EE stood up from one knee, EE felt pain in lower back. EE reported this to the supervisor immediately and received first aid but did not seek medical attention until the following day when pain had increased. A doctor gave EE restrictions that prevent EE from doing the full job.

Employee was cleaning the inside of the BC103 head pulley, from the top of the enclosure. After completing the task and climbing down, they reported feeling pain in the left knee. Employee was seen by a doctor on 10/16/18.

A maintenance mechanic got an infected eye from exposure to Calcium Oxide (Lime).

Employee was walking through the plant when there was a gust of wind and dust got into the employee's eye. Employee went to a local clinic and was treated for a foreign object in their eye and medication was prescribed.

Employee was working under high wind condition when he felt something in his right eye. Supervisor flushed eye for 25 minutes, then transported employee to medical care facility for further evaluation. Medical facility diagnosed employee with conjunctivitis and prescribed an antibiotic for use in right eye. Employee returned to work w/ no lost time or restricted duty.

Employee turned,twisted right knee while stemming blast holes. Dr.visit 9/13-Rt knee strain-Ortho visit 10/13- suggest surgery- WCofArizona requested 2nd opinion-12/13-Ortho visit-suggested surgery- Surgery on 1/16/14:incident became a reportable incident on 1/17/14.

THE HYDRATOR WAS PLUGGED. EMPLOYEE WAS ASSISTING. OTHER EMPLOYEE BENT DOWN CLEANING OFF THE MAGNET ON HUMMER SCREEN WHEN HE STOOD UP CAUSING LIME TO STIR UP. WIND BLEW THE LIME PARTICLES TOWARD INJURED AND WENT INTO HIS LEFT EYE. HE WAS WEARING SAFETY GLASSES. HE WASHED EYES AND WAS TAKEN TO THE DR. HE HAS CONJUNCTIVITIS AND AN ABRASION OF THE EYE.
