Showing all 17A Contract Truck Driver arrived on the Pueblo Plant South Truck Bay Shipping Loadout. Two GCC employees noticed this driver was laying on the platform with their head and neck area on the midrail of the gangway. After assisting the drive to their feet, driver was able to climb up the stairs under their own power.
Miner pivoted on right leg to turn towards workbench. Their knee made a "pop" sound and immediately they felt pain and informed their supervisor.
Currently under investigation with MSHA
Employee was walking across parking lot and felt pop in knee. This was deemed NOT a workers comp case but then review by insurance providers and determined by laws in the state of New Mexico it was a working comp case. This is the reason for the late reporting.
Clinker dust blew into face and under safety glasses causing eye irritation.
Employee was found non-responsive around 5:30 pm on June 13, 2017. CPR was initiated and employee was not revived. Initial reports and indication points towards natural causes, possible heart attack. Waiting for autopsy results.
While working on #18 Cement Silo to ascertain the nature of a leak from behind the dust collector housing to get a closer look at the problem. As EE looked around the DC housing, approximately 2 to 3 feet from the leak, the wind changed direction and blew cement into face and eyes.
While cleaning/shoveling a cement spill on top of silo 4, a strong wind gust blew cement under the employee's safety glasses and into eyes. Employee visited eye clinic the next day and has no work restrictions.
After walking up to belt 31124, employee kneeled down. When he did, his knee popped and has been hurting since.

Employee was kneeling on a grate and pulled his right knee as he was rising up
Employee was working a routine shift in the control room when he felt he had heart burn at 4 AM. When he returned home at 7:30 AM, he looked pale and his spouse took him to the hospital. The attending physicians determined he had suffered a heart attack.
Employee was moving debris with a skid loader. When he went to dump into the trash receptacle, wind blew back some material into his face. A small piece of metal was blown around his safety glasses and embedded itself in his left eye. Particle was removed by a physician and an antibiotic was prescribed.
The employee was setting rods and plates on the bottom floor of the Clinker Cooler. The wind blew material in the eye.
A beam glider struck the employee in the back.
EMPLOYEE WAS WALKING BEHIND A CO-WORKER WHO WAS PERFORMING MAINTENANCE AT A WATER WELL LOCATION. CO-WORKER WAS CROUCHED DOWN, AND FELL BACKWARDS AFTER LOSING HIS BALANCE. CO-WORKER FELL ONTO I NJURED EMPLOYEE'S RIGHT KNEE. INJURY BECAME "LOST TIME" BEGINNING 11/28/01 AFTER EMPLOYEE'S PHYSICIAN DETERMINED EMPLOYEE REQUIRED TIME AWAY FROM WORK TO RECUPERATE.
