Undetermined. Team member (T/M) felt irritation in left eye Sat. Nov. 19 approx. 4:00pm. Could not attribute to anything but performing grinding on Fri. Nov. 18th. approx. 5pm. Investigation showed t/m was following all safety expectations including elevated PPE requirements. T/M also stated there was no indication of irritation during or after completion of work.

Employee was walking on North roadway adjacent Kiln 5 bag house when dust/particulate blew into left eye. Employee was wearing safety glasses at the time. Flushing with saline solution was not effective and employee sought medical attention. Examination revealed minor abrasions to cornea and topical antibiotic was prescribed. Employee returned to duty.

Employee was cleaning out a dust chamber when a strong gust of wind caused dust to get in the employees eyes. The employee was wearing a face shield and safety glasses as is required for this task.

Employee was pulling cages out of baghouse, cages were very rusty. The wind was blowing heavily with a lot of dust. Dust flew into his eyes. He was wearing safety glasses at this time.

While performing an inspection inside the kiln #3 baghouse, a strong gust of wind blew through the doorway of a module creating a large cloud of lime dust. The dust blew into face and into eyes.

Their were high winds the day of the injury. The employee was working on the floor when a high wind blew lime dust into his eyes and face. The employee had safety glasses on at the time of the injury. The employee had his eye and face flushed immediately. The employee received medical treatment, Rx eye drops, and missed two days of work due to his injury.

IT WAS A WINDY DAY & THE EE WAS WALKING OUTSIDE FROM THE WAREHOUSE TO THE MAINTENANCE SHOP WHEN SOME DUST/LIME GOT INTO HER EYE. THE EE WAS WEARING HER SAFETY GLASSES AT THE TIME. THE EE'S EYE WAS FLUSHED AT WORK THEN TREAT/FLUSHED AT A MEDICAL FACILITY. THE MEDICAL FACILITY GAVE THE EE A RX FOR EYE DROPS AND PAIN PILLS.
