Employee was sitting in the lunch room at the end of shift when ee stood up and lifted arm ee shoulder popped out.

Grader operator entered cab of grader, sat down in the seat and began to take their jacket off when they felt discomfort in their back. The operator was seen by a medical provider the following day and given work restrictions which could not be reasonably accommodated.

Employee felt pain in lower back after bend over to chock tires.

IE experienced a heart attack while inspecting a loader bucket GET which resulted in IE collapsing backwards and striking the back of head on the ground. This resulted in a laceration requiring sutures to close. As a result of the heart attack the employee has not returned to work.

Employee was sitting in service truck and twisted to the right to reach for hard hat that was in between the seats. Employee then twisted to the left to open the door and exit the service truck. Upon placing leg out the door felt lower back lock up and employee was unable to move.

Contract employee delivered load of mill concentrate with OTR truck and side dump trailers. Emp dumped pup, pulled away from pile to re-position main trailer. Emp traveling to re-position. Emp continued past pile stopping on berm. Loader Oper made May Day call, ERT responded, found emp unresponsive. Emp transported off site by mine emerg. vehicle, transferred to county ambulance.

Employee was driving up the ramp and began to feel light-headed and dizzy. EE stopped truck and called supervisor and got out of truck until help could arrive

Periods of dusty environment from dust coming down the Leeville decline lead to a mechanic getting dust in the eye making is irritated. He was wearing contacts, so dust collected under contacts.

Windy conditions, blowing dust, goggles that did not fit properly on the sides. Driver was bottom off loading in windy conditions. A small amount of lime dust entered thru the side of his goggles getting into his left eye.

As an employee got up to exit the cab he was turning to the right to walk around the pilot control shut off lever, he felt a sharp pain in the back of his knee.

Employee felt debris in left eye while riding in a truck.

At the end of shift the employee was waiting for the cage to arrive when something got in his eye. The employee flushed out his eye but continued to feel discomfort in the morning. When seeking medical attention nothing was found in the eye and the eye was not infected, however the employee was given a precautionary prescription to prevent infection.

The employee was walking and the wind blew some debris into the left eye causing a corneal abrasion.

Sup.was completing daily paperwork inside the site office trailer when he believes that he was bitten by a mosquito.He did not report the incident that day. On 4/30,he arrived at work, the lt.side of his face was noticeably swollen, an area appeared to be infected.He ind.that he tried to"lance"that area with his pocketknife while at home.EE seen a phys. who admin.IV antibiotics.

After medical investigation it was determined that the injury diagnosis is a low back strain, primarily caused by the employee bending and working in an awkward position.

Employee was servicing a mobile crane on the apron and felt pain in his back while lying underneath the rear differentials.

employee was cleaning up his work area after welding. dirt and dust was blowing around and something got into his eye. he was taken for treatment and debris was removed. he returned to work.

Employee was grinding bed liners and at some point during the day, strong winds blew something in his right eye, behind his safety glasses. A metal sliver was removed the next day by the doctor.

Employee was walking uphill with a hose and his knee started to hurt. Was seen in ER and was taken off work for a knee injury.

Employee was found on the ground and was unresponsive.

Employee fainted in one of the office buildings on the mine site. It was deemed to be non-occupational.

The employee had an elevated mercury level as indicated by a standard precautionary biological test. No injury or illness was found when the employee was examined by a doctor. This report is being submitted only to terminate a citation.

Employee was working on a derrick screen boot and standing on some grating next to screen. The narrow grating buckled causing the employee to loose his balance and twist his body which strained his back.

Injured employee walked away from half empty coffee cup. After working for a short time employee returned to his coffee and drank a couple of "gulps". Employee noticed "chemical" taste in coffee and began to vomit. It was discovered that a co-worker had put a liquid chemical de-icer into the coffee causing a possible poisoning situation.

The wind caught the door of a track drill while the employee was exiting the equipment and caught his left "little" finger in the door of drill. He was wearing leather gloves at the time. Employee was taken to a medical facility, received 5 stiches, and returned to work.

Employee was checking oil level on rockbreaker, when he felt something in his eye. He was taken to a medical facility, given medication for the eye, and released to full duty.

At approximently 3:30 pm on June 30th supervisor was on the 4020 level west sump area. He noticed a mist coming from the acid wash totes in the back of the drift. When he entered the wash bay he noticed a mild throat and eye irritation and became nauseous. He was examined by a Dr. and given a full duty release.

The employee was holding on to the edge of the door of a pickup speaking with the driver of another pickup who also had his pickup door open. A gust of wind blew catching one door blowing it into the other door. The employee's pinky finger was caught between the two doors. The employee suffered a laceration which required stitching.

Bent over to pick up a pallet and felt pain in her back.

Employee states while walking from drill deck to control panel his knee gave out.

EMPLOYEE REPORTED BEING BIT BY A SPIDER ON THE LOWER LEG WHILE OPERATING A D9R DOZER. ON 11 AUG, HE SOUGHT MEDICAL ATTENTION FOR A SWOLLEN AND DISCOLORED LEFT BIT TOE. HE WAS TREATED AND PLACED ON RESTRICTED DUTY.

The employee noticed irritation in his right eye on May 11th. He does not know the exact time that the eye started to bother him. About 5:00pm that day he reported it. He asked to seek medical attention on May 13th.On the 13th the Doctor Removed a tiny piece of metal from his eye and gave him a prescription for an antibiotic.

Employee altercation resulting in cut above eye on one of the the employees involved.

Employee started feeling nausea, headache and tingling up and down her arm. She was transported to the University of Utah Hospital where she was operated on for 2 brain aneurysms.

Employee was leaving the property, when he felt something in his eye. He sought medical treatment after repeated attempts to remove the irratant. He was given prescription eye drop medication for his eye.

A gust of wind blew through the open shop door, employee thought he got the particle out of his eye. He returned to work the next day with irritation and sought medical attention. The doctor removed a piece of plastic from under the eye lid that was not imbedded. He was given a prescription for antibiotics.

AT THE END OF SHIFT ON 9/30 THE EMPLOYEE NOTICED A SMALL SORE ON HIS FINGER. HE REPORTED IT THE NEXT DAY AND WAS TAKEN TO THE DOCTOR. HE WAS TREATED FOR A SUSPECTED SPIDER BITE AND RELEASED. O N 10/7 HE WAS SENT FOR A FOLLOW-UP VISIT AND THE SORE HAD BECOME INFECTED. ON 10/14 HE WAS SEEN AND GIVEN A FULLY DUTY RELEASE.

EE WAS SPLASHED WITH MILK OF LIME WHEN THE DRAIN LINE BURPED AS SHE WAS WALKING TOWARDS IT. EE WAS WEARING HER PRESCRIPTION GLASSES AT THE TIME OF THE OCCURANCE, HOWEVER SHE DID GET SOME UNDER THE GLASSES. SHE WAS SEEN AT A MEDICAL FACILITY AND WAS GIVEN PRESCRIPTION MEDICATION FOR A CORNEAL ABRASION.

EE'E EYE STARTED TO BOTHER HIM DURING SHIFT. WHEN HE WAS TAKEN TO THE ER DIRT WAS REMOVED FROM HIS EYE.

EMPLOYEE FELT A "POP" IN HIS KNEE AS HE ROSE FROM HIS SEAT ON A PASSENGER VAN USED TO TRANSPORT EMPLOYEES TO THEIR WORK AREA.

EE SUFFERED A HEART ATTACK ON MINE PROPERTY AND DIED AS A RESULT. THE HEART ATTACK WAS NOT CAUSED BY AN ACCIDENT OR ANY EXPOSURE IN THE WORK ENVIRONMENT, NOR WAS IT WORK-RELATED OR RELATED TO WORK ACTIVITY.

EE WAS KNEELING DOWN TO CHECK FLOW ON A SMALL PUMP. WHEN HE STOOD UP HE FELT SOME SORENESS IN HIS LEFT KNEE. THIS IS REPORTABLE BECAUSE OF FOUR RESTRICTED DAYS.

WHILE EMPLOYEE WAS LOOKING UP AT A INSTRUMENTATION DEVICE A FOREIGN OBJECT (DUST) WENT INTO HIS RIGHT EYE. THIS WAS REPORTABLE BECAUSE HE WAS GIVEN A PRESCRIPTION FOR ANTIBIOTICS.

EE WAS TRANSPORTED TO HOSPITAL FOR MEDICAL EVALUATION. MEDICAL DIAGNOSIS WAS THAT EE WAS HAVING A HEART ATTACK AND SURGERY WAS PERFORMED TO REMOVE CORONARY BLOCKAGE.

THE EE FELT AN IRRITATION IN THE RIGHT EYE. HE WAS UNSURE OF WHAT COULD BE CAUSING THE IRRITATION. AFTER FLUSHING THE EYE AT THE MINE SITE, THE EE WAS TAKEN TO A MEDICAL FACILITY WHERE A DR. R EMOVED A PIECE OF METAL FROM THE EE'S EYE.

EMPLOYEE STUCK HIS HAND UP TO STOP A DOOR FROM OPENING. ME MISSED THE FRAME AND PUT HIS HAND THROUGH THE WINDOW IN THE DOOR. THIS WAS WIRED GLASS. FURTHER INVESTIGATION INDICATES HORSEPLAY WAS INVOLOVED.

EE WAS CLOSING THE OFFICE DOOR WHEN A GUST OF WIND SLAMMED THE DOOR CLOSEDF CATCHING HER FINGER BETWEEN THE DOOR AND THE DOOR FRAME. XRAYW REVEALED A FRACTURE TO THE TIP OF HER LEFT INDEX FING ER.

THE EE WAS OPERATING DZ265 WITH ALL THE WINDOWS AND THE DOOR CLOSED. A LARGE WIND STORM BLEW DIRT THRU THE AIR CONDITIONING VENT INTOHIS RIGHT EYE. AFTER EXTENSIVE FLUSHING OF THE EYE AT THE M INESITE, HE WAS TAKEN TO A MEDICAL FACILITY.

EMPLOYEE WAS LIFTING TWO PEOPLE TO THE ROOF IN ABOOM TRUCK. AS HE WAS DOING THIS, HE FELT SOME-THING IN HIS LEFT EYE; HE WAS WEARING HIS SAFETYGLASSES.

EMPLOYEE WAS WORKING IN THE WELDING SHOP WITH THE DOORS OPEN; THE WIND WAS BLOWING THROUGH THESHOP, WHICH CAUSED METAL FILINGS TO ENTER INTO THE EMPLOYEE'S EYE.
