Grader operator entered cab of grader, sat down in the seat and began to take their jacket off when they felt discomfort in their back. The operator was seen by a medical provider the following day and given work restrictions which could not be reasonably accommodated.

IE experienced a heart attack while inspecting a loader bucket GET which resulted in IE collapsing backwards and striking the back of head on the ground. This resulted in a laceration requiring sutures to close. As a result of the heart attack the employee has not returned to work.

Employee was sitting in service truck and twisted to the right to reach for hard hat that was in between the seats. Employee then twisted to the left to open the door and exit the service truck. Upon placing leg out the door felt lower back lock up and employee was unable to move.

Windy conditions, blowing dust, goggles that did not fit properly on the sides. Driver was bottom off loading in windy conditions. A small amount of lime dust entered thru the side of his goggles getting into his left eye.

At the end of shift the employee was waiting for the cage to arrive when something got in his eye. The employee flushed out his eye but continued to feel discomfort in the morning. When seeking medical attention nothing was found in the eye and the eye was not infected, however the employee was given a precautionary prescription to prevent infection.

Employee was working on a derrick screen boot and standing on some grating next to screen. The narrow grating buckled causing the employee to loose his balance and twist his body which strained his back.

Injured employee walked away from half empty coffee cup. After working for a short time employee returned to his coffee and drank a couple of "gulps". Employee noticed "chemical" taste in coffee and began to vomit. It was discovered that a co-worker had put a liquid chemical de-icer into the coffee causing a possible poisoning situation.

The wind caught the door of a track drill while the employee was exiting the equipment and caught his left "little" finger in the door of drill. He was wearing leather gloves at the time. Employee was taken to a medical facility, received 5 stiches, and returned to work.

At approximently 3:30 pm on June 30th supervisor was on the 4020 level west sump area. He noticed a mist coming from the acid wash totes in the back of the drift. When he entered the wash bay he noticed a mild throat and eye irritation and became nauseous. He was examined by a Dr. and given a full duty release.

The employee was holding on to the edge of the door of a pickup speaking with the driver of another pickup who also had his pickup door open. A gust of wind blew catching one door blowing it into the other door. The employee's pinky finger was caught between the two doors. The employee suffered a laceration which required stitching.

The employee noticed irritation in his right eye on May 11th. He does not know the exact time that the eye started to bother him. About 5:00pm that day he reported it. He asked to seek medical attention on May 13th.On the 13th the Doctor Removed a tiny piece of metal from his eye and gave him a prescription for an antibiotic.

O N 10/7 HE WAS SENT FOR A FOLLOW-UP VISIT AND THE SORE HAD BECOME INFECTED. ON 10/14 HE WAS SEEN AND GIVEN A FULLY DUTY RELEASE.

EE WAS SPLASHED WITH MILK OF LIME WHEN THE DRAIN LINE BURPED AS SHE WAS WALKING TOWARDS IT. EE WAS WEARING HER PRESCRIPTION GLASSES AT THE TIME OF THE OCCURANCE, HOWEVER SHE DID GET SOME UNDER THE GLASSES.
