The Supervisor was helping load some trash wire on the forks of a TH-83 extendable boom forklift. He gave the proper signal to roll the forks back & did not realize that he had his hand in the pinch point until the injury occurred. The index, middle & ring fingers were cut & middle finger required 5 stitches & index 1. He was released on light duty.

While scraping muck out of a hoist cylinder boss, employees hand connected with a metal edge. The employee was taken in for medical treatment and was given 6 sutures.

A miner just pulled his Jumbo Drill out of the N.W. 5785-971 heading. The miner noticed some dribbling coming off the back in an intersection. The area was secured from unauthorized entry of personnel. The back fell above anchorage points. The fall of ground occured between shifts.

Bent over to pick up a pallet and felt pain in her back.

Stepping off a tractor the employee felt his foot pop. He worked for a while but his foot was hurting so he had it checked and it was broken.

Operating a haul truck when she struck her head on top of the cab after hitting a bump. Company was told about the restricted duty until the 2/7/06.

Getting out of haul truck and stepped on rock twisting left ankle.

He was holding a part that was being removed when the part fell hitting his hand

Was prying with a bar, a rock that was wedged between wheel and frame when he strained his back.

Changing hose on bolter when he tripped and caught himself with his right arm and strain his shoulder. The accident happened on 11/14/05 but he did not get medical attention until 11/29/05 when he was placed on restricted duty.

Employee was loading a 12 ft drill steel into the back of his haul truck. When he raised his arm to load the steel, his left shoulder dislocated.

Employee was carrying a core rod and stepped on a hose twisting his knee. Employee had knee surgery on February 16, 2006.

Employee originally hurt his hand on the 10th of October, employee was compensating for hand injury and hurt his shoulder.

Unloading a bin, the gate released and another bin came down and smashed her fingers. Drug results pending. Doctor told her no work until 8/29/05. (Letter from Newmont regarding validity of claim attached to original report).

While cleaning for a drill hole with a Fin Hoe a rock rolled down and he hit his finger on the rock. Cutting his middle finger requiring 4 sutures.

Employee was operating loader, he went over a bump and hit his head on the ROPS.

Employee was bolting when he woke up next to the bolter. Employee was unaware of what happened.

Operating haul truck and his eyes started burning, next day right eye was irritated. Employee received prescription antibiotic.

Employee was getting into mucker when his light cord got caught on the door handle which snapped his neck back and hit his hard hat on the ROPS.

Employee was cleaning out a lifter when a rock fell from the face and struck him on the left back, fracturing three ribs.

While bolting, a bolt mis-loaded. EE went to look and the bolt slid out, hitting him in the lip.

Employee was hanging ventilation bag in his work area, while lifting a section of vent bag with one arm, the miner felt acute pain in his right shoulder.

Employee struck in lower back with overshot when driller pulled the wireline control instead of rotation control. Overshot pulled up to the pulley at top of mast and wireline broke. Overshot fell on deck and bounced striking employee in back.

Employee was hanging utilities in a mucker bucket, once the bucket was lowered, he forgot he was tied off. He hit the end of the lanyard and lost his footing. He landed on his left elbow on the lip of the mucker bucket and received a laceration.

Employee was getting off tractor and rolled his left ankle. Employee had surgery on 4/4/06 for ankle reconstruction.

Employee was putting in a hydraulic pump by hand when he felt a tear in his right shoulder.

Employee was unloading bags of prell when he felt a strain in his left shoulder.

Operator was getting off haul truck at end of shift, he stepped onto some uneven ground frozen mud and sprained his ankle. requiring a splint on his ankle.

EE GOT INTO THE CAB OF LOADER 29 AND REACHED TO CLOSE THE DOOR. THE DOOR BEGAN TO CLOSE UNDER ITS OWN WEIGHT. THE TIP OF EE'S RIGHT MIDDLE FINGER WAS CUT BETWEEN THE DOOR AND THE CAB, REQUIRING 6 STITCHES TO CLOSE.

Employee started feeling nausea, headache and tingling up and down her arm. She was transported to the University of Utah Hospital where she was operated on for 2 brain aneurysms.

GROUND FAILURE. APPROXIMATELY 400 TONS OF MATERIAL FELL OUT OF THE 5230 INTERSECTION COVERING THE FRONT END OF UFR-004. WHILE THE UNIT WAS BEING PULLED OUT, THE BOOM OF THE UNIT WAS TORN AWAY. TOTAL FAILURE - APPROXIMATELY 700 TONS.

Miner was showering after shift when some debris washed into his left eye.

While on surface dumping a load operator hit a bump and hit knee on steering box, bruising knee. On 2-8-05 operator needed surgery on knee.

Miner was ramping the 5305-614. he had his hand outside of the cab of uldoo5 and pinched his left middle finger wheile making a tight corner.

employee tripped up a stair and caught himself with right arm, when he caught himself he felt a twist and pain in right shoulder.

FALL OF GROUND IN THE WORKING FACE OF THE 5470-397.

EMPLOYEE WAS HOLDING A SOCKET WHILE REMOVING THE LUG NUT, THE KEEPER PIN CAME LOOSE CUTTING HIS HAND. (8 SUTURES)

Employee states when he raised his right arm to swing tool to strike core tube his right elbow struck protruding pipe that was on back of truck

WHILE JACKLEGGING IN THE DRIFT, A ROCK FELL FROM THE BACK, HITTING EE ON THE ARM.

EE WAS CLEANING HIS AREA AT THE START OF SHIFT AND SLIPPED ON THE ICE. ON 5/5/04 EE HAD SURGERY ON HIS KNEE AND TURNED THE INJURY INTO A LOST-TIME ACCIDENT.

EE WAS INSTALLING DAGGERS IN THE BACK WHEN A ROCK FELL OUT OF THE WIRE HITTING HIM ON THE FACE.

EMPLOYEE WAS GOING TO GET SOME EQUIPMENT OUT OF A MUCKER BUCKET AT THE SAME TIME THE OPERATOR WAS ROLLING THE BUCKET DOWN. THE INJURED EMPLOYEE HAD HIS FOOT UNDER THE BUCKET LIP AND THE MUCKER BUCKET LANDED ON HIS FOOT.

WHILE TURNING TO THE RIGHT EMPLOYEE HAD HIS RIGHT FOOT PLANTED CAUSING HIS RIGHT ANKLE TO TWIST.

EE STEPPED BACKWARDS INTO A POT-HOLE.

EMPLOYEE WAS LIFTING A ROLL OF WIRE AND STRAINED HIS RIGHT WRIST. ON 10/21/03 THE INJURY WAS STILL BOTHERING HIM SO HE WENT TO A DOCTOR WHO PUT A CAST ON HIS WRIST TO STABILIZE IT.

EMPLOYEE WAS MUCKING WHEN HE DROVE INTO A TIRE RUT CAUSING THE MUCKER TO HIT THE RIB AND BREAK THE RIBS OFF STRIKING THE OPERATOR.

EE'E EYE STARTED TO BOTHER HIM DURING SHIFT. WHEN HE WAS TAKEN TO THE ER DIRT WAS REMOVED FROM HIS EYE.

WHILE ENROUTE TO A HEADING, EE TRIPPED AND TWISTED HER ANKLE.

WHILE TIGHTENING UP A BOLT WITH A JACKLEG EMPLOYEE TWISTED HIS RIGHT ARM. THIS INJURY BECAME A REPORTABLE INJURY ON 1/9/03.

EMPLOYEE WAS INSTALLING BUCKET PINS WITH A SLEDGE HAMMER. WHEN HE SHIFTED HIS WEIGHT TO HIS LEFT KNEE IT POPPED OUT.

EE WAS TRYING TO BEND A METAL PLATE WITH THE BLUNT END OF A MINE AXE. AS HE SWUNG DOWN HIS HAND CAME IN CONTACT WITH A PART THAT WAS IN THEWAY.

MECHANIC WAS POUNDING ON A BENT EXHAUST CLAMP, THE U BOLT FLIPPED UP AND HIT HIM IN THE MOUT.

CREW WAS SPOOLING WIRE LINE WHICH HUNG UP ON SOMETHING CAUSING WIRE LINE SPOOL TO LIFT AND SLIDE FORWARD AND HIT EMPLOYEE IN THE RIGHT ANKLE.

MINER TRIPPED OVER A ROCK AND STUMBLED INTO THE RIB BUMPING HIS HEAD.

WHILE MOVING A CABLE OVER TO THE RIB THE EMPLOYEE STRAINED HIS BACK.

EMPLOYEE WAS BOLTING UP THE HEADING. THEY HUNG THE STEEL AND WAS UNABLE TO FREE IT SO THEY DECIDED TO BEND THE STEEL AND TIE IT OUT OF THE WAY.THEY WERE USING THE LOADER BUCKET TO BEND THE STEEL WHEN THE STEEL SLID OFF THE BUCKET AND HITTHE EMPLOYEE IN THE HAND.

EE WAS HELPING TO CHIP OUT THE MIXER AT CARLIN EAST BATCH PLANT, ANOTHER EE WAS CHIPING AND A PIECE OF METAL FLEW OFF OF THE BIT. THE METAL LODGED IN HIS LEFT CHEECK.

EE WAS AT TOP OF THE STAIRWAY AT THE CARLIN EASTBATCH PLANT WHERE SHE WAS PAINTING POLES. WHEN SHE STARTED TO WALK DOWN THE STAIRS HER R. BOOT CAUGHT ON THE STAIR. EE TWISTED HER R. ANKLE, THIS ACCIDENT HAPPENED ON 6-20-01 BUT SHE DID NOT SEEK MEDICAL ATTENTION UNTIL 7-13-01, AND WAS PLACED ON RESTRICTED DUTY.

EE WAS COLLARING BOLT HOLE WHEN A ROCK FELL STRIKING HIS SAFETY GLASSES AND EYE. EE IS CURRENTLY OFF WORK.

THE DRILL HELPER WAS STACKING DRILL RODS AGAINSTTHE RIB. THE RODS STARTED TO FALL STRIKING HIS FOOT.

EE WAS LEANING OVER A TRUCK TORQUE CONVERTER AREA FIXING A LEAK WHEN HE FELT A SLIGHT PAIN IN HIS ABDOMEN. EE IS SCHEDULED FOR CORRECTIVE SURGERY ON 4-5-2001.

EE WAS SEPARATING DUAL CONE SEALS FOR FRONT HUB ON ULD 002. WHEN SEALS SEPARATED, THE SHARP MACHINE SURFACE CUT THE EE THUMB.

EMPLOYEE WAS INSTALLING A DWYDAG WHEN THE DRIVER CAME OFF OF THE DWYDAG. THE DWYDAG FELL OUT OF THE HOLE AND STRUCK THE EMPLOYEE ON THE RIGHT HAND.

EMPLOYEE STEPPED ON A ROCK BOLT; IT FLIPPED UP &STRUCK HIS LIP.

WHILE BOLTING WITH A JACKLEG DRILL FROM THE MUCKPILE, EMPLOYEE FELL TO THE GROUND STRAINING HIS LEFT SHOULDER. A MRI REVEALED A PROBLEM WHICH REQUIRED SURGICAL REPAIR WHICH WAS DONE ON 2/23/01

GROUND FALL IN 5630-307 TOP CUT. TWO MEN SHOT THEIR ROUND AT 3:15 PM ON 9/28/00 AND RETURNED TO THE HEADING TO BAR DOWN AND RAMP TO BOLT. THE HEADING THEN SAT IDLE UNTIL 10:00 AM ON 9/29/00 WH EN THE SAME TWO MEN RETURNED TO FIND THAT THE OPEN GROUND HAD WORKED ABOVE AND THROUGH THE EXISTING GROUND SUPPORT.

THE DRILL HELPER WAS KEEPING TENSION ON THE EXCESS SLACK 9N THE WIRELINE AS IT WAS BEING SPOOLED. THE WIRE LOOPED AROUND THE DRILL HELPER'S FINGER AS IT WAS SPOOLING CAUSING A FRACTURE OF THE LEFT LITTLE FINGER.

THE ELECTRICIAN WAS WORKING ON A LOADCENTER, ANDHE INADVERTENTLY STEPPED ON A ROCK AND TWISTED HIS KNEE.

TWO MECHANICS WERE ATTEMPTING TO MOVE A HAUL TRUCK, WHICH HAD A FLAT TIRE, TO A LOCATION WHERE THE TIRE COULD BE CHANGED. THE MECHANICS ATTEMPTED TO PULL THE TRUCK AWAY FROM THE RIB WITH A 4 Y D LOADER ALONG WITH THE ASSISTANCE OF ONE MECHANIC AT THE CONTROLS OF THE TRUCK. DURING THE PROCESS OF MOVING THE TRUCK THE LOADER PULLED AWAY CAUSING THE TRUCK TO JAR.

AS THE MINER WAS LOADING THE FACE HE TURNED TO CONVERSE WITH HIS SHIFTER. AS HE ROTATED HIS UPPER BACK AND SHIFTED HIS WEIGHT, HIS RIGHT KNEE CAP DISLOCATED.
