A FATAL ACCIDENT OCCURRED AT THE 5520-237 LONGHOLE STOPE WHEN A WATER TRUCK OPERATOR BACKED OVER THE BERM AND INTO THE STOP WHICH AT THE TIME WAS BEING BACKFILLED.

The Supervisor was helping load some trash wire on the forks of a TH-83 extendable boom forklift. He gave the proper signal to roll the forks back & did not realize that he had his hand in the pinch point until the injury occurred. The index, middle & ring fingers were cut & middle finger required 5 stitches & index 1. He was released on light duty.

While scraping muck out of a hoist cylinder boss, employees hand connected with a metal edge. The employee was taken in for medical treatment and was given 6 sutures.

A miner just pulled his Jumbo Drill out of the N.W. 5785-971 heading. The miner noticed some dribbling coming off the back in an intersection. The area was secured from unauthorized entry of personnel. The back fell above anchorage points. The fall of ground occured between shifts.

Stepping off a tractor the employee felt his foot pop. He worked for a while but his foot was hurting so he had it checked and it was broken.

Changing hose on bolter when he tripped and caught himself with his right arm and strain his shoulder. The accident happened on 11/14/05 but he did not get medical attention until 11/29/05 when he was placed on restricted duty.

Employee was loading a 12 ft drill steel into the back of his haul truck. When he raised his arm to load the steel, his left shoulder dislocated.

While cleaning for a drill hole with a Fin Hoe a rock rolled down and he hit his finger on the rock. Cutting his middle finger requiring 4 sutures.

Employee was cleaning out a lifter when a rock fell from the face and struck him on the left back, fracturing three ribs.

While bolting, a bolt mis-loaded. EE went to look and the bolt slid out, hitting him in the lip.

Employee was hanging ventilation bag in his work area, while lifting a section of vent bag with one arm, the miner felt acute pain in his right shoulder.

Employee struck in lower back with overshot when driller pulled the wireline control instead of rotation control. Overshot pulled up to the pulley at top of mast and wireline broke. Overshot fell on deck and bounced striking employee in back.

Employee was hanging utilities in a mucker bucket, once the bucket was lowered, he forgot he was tied off. He hit the end of the lanyard and lost his footing. He landed on his left elbow on the lip of the mucker bucket and received a laceration.

EE GOT INTO THE CAB OF LOADER 29 AND REACHED TO CLOSE THE DOOR. THE DOOR BEGAN TO CLOSE UNDER ITS OWN WEIGHT. THE TIP OF EE'S RIGHT MIDDLE FINGER WAS CUT BETWEEN THE DOOR AND THE CAB, REQUIRING 6 STITCHES TO CLOSE.

Employee started feeling nausea, headache and tingling up and down her arm. She was transported to the University of Utah Hospital where she was operated on for 2 brain aneurysms.

GROUND FAILURE. APPROXIMATELY 400 TONS OF MATERIAL FELL OUT OF THE 5230 INTERSECTION COVERING THE FRONT END OF UFR-004. WHILE THE UNIT WAS BEING PULLED OUT, THE BOOM OF THE UNIT WAS TORN AWAY. TOTAL FAILURE - APPROXIMATELY 700 TONS.

EE WAS HELPING TO CHIP OUT THE MIXER AT CARLIN EAST BATCH PLANT, ANOTHER EE WAS CHIPING AND A PIECE OF METAL FLEW OFF OF THE BIT. THE METAL LODGED IN HIS LEFT CHEECK.

EE WAS AT TOP OF THE STAIRWAY AT THE CARLIN EASTBATCH PLANT WHERE SHE WAS PAINTING POLES. WHEN SHE STARTED TO WALK DOWN THE STAIRS HER R. BOOT CAUGHT ON THE STAIR. EE TWISTED HER R. ANKLE, THIS ACCIDENT HAPPENED ON 6-20-01 BUT SHE DID NOT SEEK MEDICAL ATTENTION UNTIL 7-13-01, AND WAS PLACED ON RESTRICTED DUTY.

GROUND FALL IN 5630-307 TOP CUT. TWO MEN SHOT THEIR ROUND AT 3:15 PM ON 9/28/00 AND RETURNED TO THE HEADING TO BAR DOWN AND RAMP TO BOLT. THE HEADING THEN SAT IDLE UNTIL 10:00 AM ON 9/29/00 WH EN THE SAME TWO MEN RETURNED TO FIND THAT THE OPEN GROUND HAD WORKED ABOVE AND THROUGH THE EXISTING GROUND SUPPORT.

TWO MECHANICS WERE ATTEMPTING TO MOVE A HAUL TRUCK, WHICH HAD A FLAT TIRE, TO A LOCATION WHERE THE TIRE COULD BE CHANGED. THE MECHANICS ATTEMPTED TO PULL THE TRUCK AWAY FROM THE RIB WITH A 4 Y D LOADER ALONG WITH THE ASSISTANCE OF ONE MECHANIC AT THE CONTROLS OF THE TRUCK. DURING THE PROCESS OF MOVING THE TRUCK THE LOADER PULLED AWAY CAUSING THE TRUCK TO JAR.
