Contact person was called by the Illinois Tollway on the afternoon of 1/27/26 about a missing person that was suicidal that may have jumped in the West Quarry off of the 80/94 tollway. Second contact was contacted around 4:00PM to come help search the West Quarry. At around 4:50PM the body of the trespasser was found and reported to authority's and the department of labor.
Employee was entering the driver's side door of the plant maintenance truck. After sitting in down in the seat, EE was stung in the lower back by a yellow jacket. An allergic reaction ensued, and the employee was transported to the local hospital.
Employee was checking their surroundings while reversing the loader. When they turned to look beside them, they lost feeling in their lower extremities.
Windy and gusting weather conditions blew dust and debris into employee left eye
EE1 arrived at Hanson Harding Street, checked out a 775 haul truck and drove it over to the restroom. EE2 noticed EE1 slumped over steering wheel. EE3 stopped to check on EE1 and EE1 was unresponsive and EE3 called 911. Ambulance crew gave EE1 medicine to revive EE1, then took EE1 to the Hospital.
Employee was lifting liner plate into chute above crusher, when wind caught plate turning it into metal guarding causing dust and dirt to come down onto EE and into EE's eye.
IE felt pain in lower back during pre-shift inspection when IE bent over. IE informed supervisor at 9:09am and was taken to hospital. Reportable due to restrictions, prescriptions and lost time. IE was transferred to different job based on restrictions and missed 10/27/17 & 10/30/17.
Employee was doing maintenance on plant feeder. Debris blew into left eye.
Employee walked to haul unit upon start of shift, climbed up steps of truck and sat down in seat to begin preshift inspection. Another employee noticed that employee was in distress, ERP was initiated and employee was eventually taken via ambulance to ER where he was pronounced dead. Causes of death on coroner's report are Coronary Atherosclerosis and Adenocurcinoma of Lung.
Second-hand info: Five minor children approx. 15 y/o were trespassing on mine property, bypassed several No Trespassing signs, a locked gate, and a barricade in front of an old underground portal. Emergency services contacted the mine site to report people stuck in the underground. Children were off property before mine personnel arrived at the old portal.
Personal health issue, not work related. Employee was receiving site specific hazard training at the mine when the employee collapsed and started having seizures. The employee was transported via ambulance to the local hospital. Employee returned to work without restrictions two days later. The employee had a history of seizures.
Non occ. non work related heart attack.On 2/3/12 a haul truck operator at the Quarry experienced chest and arm pain. EE's at the site called 911. The haul truck operator was transported to the local hospital via ambulance.
Contributing conditions unknown-employee had heart attack while on the job.
EE was in the pep screen chute. As he was climbing down the chute, a strong gust of wind came up through the chute and blew sand under his safety glasses and into his right eye. The sand entered his right eye and he was not able to flush all of it out with water. The decision was then made to go to US Healthworks to have his eye flushed.
Employee was originally being treated for heat exhaustion, but after being transported to the hospital, they discovered he was having a heart attack.
Employee brought in Physician's Report of Work Ability to GM on 4/20/05 at 4p. Report stated that EE was totally disabled from work from 4/21/05 to 5/8/05 w/ no DOI listed, but claim #02-816744. This claim relates back to 4/3/02 & 10/22/04. When the GM asked employee what EE did to hurt back, EE replied, "nothing EE could recall."
Employee woke up Friday morning with back pain and brought a doctor's slip in excusing him from work from 10/22/04 through 10/31/04. This injury was originally reported on 4/2/2002 and this injury report is being refiled per Bob LeMasters per 11/2/04 phone conversation.
BEFORE SHIFT STARTED, EE WAS THROWING UP AS HE WALKED AWAY FROM JOB TRAILER. HE SLUMPED OVER, UNABLE TO RESPOND VERBALLY TO COWORKER. ONE SIDE OF HIS FACE WAS SAGGING. TAKEN TO HOSPITAL - ADMITTED FOR MILD STROKE.
USING A SCREWDRIVER TO CLEAN THE HOLE OUT OF JAW DIES, EMPLOYEE BELIEVES THE WIND BLEW THE METAL PARTICLE UNDER HIS SAFETY GLASSES. HAD TO BE REMOVED BY A DOCTOR.

NOTICED AN IRRITATION IN LEFT EYE AROUND 2:OOPM.NOT SURE WHEN HE GOT DEBRIS IN HIS EYE. WENT TO HOSPITAL TO HAVE IT REMOVED.
