Miner was walking on roadway at cooler, felt something go in right eye, was wearing all required PPE including safety glasses, had no issues with eye rest of work day, eye became irritated during the night, reported incident on 4/24/2018, sent to eye specialist, metal flake removed from right eye, written out of work for 4/24/2018 and 4/25/2018. Full duty 4/26/2018

Employee walking out of control room front door, opened door and wind blew door out of hand and sharp edges on handle of door cut right hand index finger
EE reported to supervisor on 1/19/2010 that his left knee was swollen, the employee did not report any accident, but felt it was due to excessive walking and stair climbing. On 01/25/2010 the employee was sent to Healthworks for evaluation, diagnosis was left hip and thigh strain due to excessive use. Employee was placed on restricted duty on 1/25/2010.
Dust
EE WAS DUMPING VINSOL INTO FEED CHUTE OF MILL. MATERIAL WAS BLOWN OUT OF CHUTE AND COVERED HIS ARMS AND FACE.
SUPERVISOR WAS EXAMINING STONE STORAGE AREA WHEN THE PLASTIC DUST CONTAINMENT STRIPS HIT HIM IN THE FACE DUE TO THE WINDY WEATHER CONDITIONS.
