EE became ill from suspected (but unconfirmed) heat exhaustion; lost consciousness and was transported via ambulance for further treatment. EE still under evaluation, and incident still under investigation.

Employee was getting tools from toolbox. When turning to go back towards workstation, felt a "pop" in right knee. Entire body was not facing direction of travel causing knee to twist.

Injured was working on a 994 loader. EE had to lay down and reach out to get hydraulic hoses into position. This caused awkward body position and exertion caused a back strain.

Employee was bending over to move a chain on the floor and felt a strain in back. Employee was seen by a doctor and referred to a chiropractor on the 20th.

NOTE: Did not have enough information to say is reportable until today. Hospital refused to provide info. Employee was shoveling sand in windy conditions, sand got into eye. Was wearing safety glasses but not goggles. Went to ER. Employee today stated he was given prescription antibiotic and corneal abrasion. No lost time/restricted duty.

Employee removing lagging off of snub pulley on boat end shuttle when rust or dirt got into eye. Safety glasses and face shield was being used. Wind was swirling and material made it into his eye.

The millwright reported to work for the second shift starting at 7:00 a.m. At about 6:50 a.m., while sitting in a chair in the lunchroom, he began to put his work boots on. As he bent over to tie his boots his back began to hurt. The injured has a history of back spasms.
