Employee fell through opening in walkway.
EE was attempting to loosen a bolt with a wrench. When putting force/pressure on the wrench, it slipped off the bolt causing the EE to lose balance and fall backwards onto a railing. EE had strain/bruising as a result of the incident.
EE became ill from suspected (but unconfirmed) heat exhaustion; lost consciousness and was transported via ambulance for further treatment. EE still under evaluation, and incident still under investigation.
Millwright cut knee while cutting a conveyor belt. The belt knife slipped and cut through millwright's overalls and into knee. Received 5 stitches at ER.

Employee was lifting a piece of scrap steel into the metal disposal dumpster, and felt a sharp pain in right shoulder. Employee has continued to work, however, the strain had caused damage to tendons which resulted in surgery on 2/1/22.
After finishing loading a boat, injured walked up the stairs to shut power off in 2 north tunnel. While walking up the stairs injured felt a pop in their knee.
Employee was walking from the caboose back to the 7523 engine to restart it after stalling. EE was in the process of dumping cars and was walking along the dump shack parking area. While walking along the track, EE stepped in a hole twisting EE's right ankle.

While operating the locomotive pushing a cut of empty cars into the WQ loading station the EE felt pain in right shoulder.
Great Lakes Trader employee was standing near mooring wire preparing to move wire off of mooring bit. When a second Trader employee started the winch, the mooring wire struck injured in the stomach, knocking them to the ground. When they fell to the ground, hard hat fell of and they struck head on the ground.

Employee was manually moving rail road ties to forks on loader when ties shifted pinching finger between two ties. Resulted in fracture and laceration.
While closing slide Gate in # 8 tunnel, Ratchet slipped off the shaft causing EE to fall backwards striking lower back.

While backing a dozer down a stockpile, EE went over a lip in the pile which shook the dozer. This in turn caused some pain in EE's neck. After continued discomfort, EE went for MRI on 9/28/2017. EE was given restrictions at that time.
Employee was bending over to put on work boots and felt a pop in lower back. Employee was unable to move.

Surgery 12/19/16 - Carrying a pale of stemming, slipped down a hill, pulled groin
Boat was docking and throwing heaving lines and when the EE attempted to catch it the EE stepped into a hole/washout on the dock. The EE fell and twisted right ankle.
Embers from the exhaust of the locomotive used to transport material from the quarry to the mill ignited a grassy area and spread to cover ~5 acres of wetlands at the mine site.
While changing bags in the bag house EE was kneeling down to change a bag and turned to grab a tool EE left knee popped.
When EE's left foot stepped from the ladder onto the surface it slipped from the ladder to a cleat in the walkway. The EE was able to catch self with EE's right arm by hanging onto a ladder rung. The EE felt strain in shoulder and knee but continued working. Asked to see a doctor on 2/4/2106 at which time EE was placed on restriction
Employee was performing maintenance on car 123 and discovered a damaged brake cylinder, which was removed and placed in back of service truck. The employee +1 were removing the cylinder from the truck to dispose of it, it slipped and crushed the EEs finger between the brake cylinder and step of the service truck, resulting in facture of right pinky.

Employee slipped and twisted his knee when he was coming down from the hill. He caught himself but twisted knee. Site conditions were snow covered and slippery in some areas.

Employee was walking across loading dock area between shuttle and lunch room when he slipped and fell. Area had received 1"" of snow, underlying ice was not obvious.
Employee was cleaning up stone that had spilled off a conveyor from previous loading. He slipped on loose particles that were on concrete surface and landed on shoulder and elbow. Minor scrapes occurred on arm, a large contusion formed on elbow, and soreness was reported in shoulder. Local med provider placed him on a lifting restriction. Possible rotator cuff issue.
Employee was repositioning (on a pallet) a railroad car brake beam that had been previously loaded with chain onto the pallet. Employee felt an abdominal strain the next day.

Employee was operating Cat 990 Loader to clean around a railbed. As he was backing up and looking backward, he ran over uneven ground which jostled him. He claims to have heard a pop in his lower back and that pain followed. NOTE: Incident was first reported as a first-aid-only case on 10/24/13. Management was informed of a 12/18/13 visit to a physical therapist, on 12/19/2013.
Employee was climbing down haul truck steps. When he stepped from the bottom step to the ground he felt pain in his left knee. The ground onto which he stepped was level and bottom step is 28"" above ground. Employee claims he continued working thinking the pain would go away. HS Management received indication of desire to seek evaluation on 5/17/13.
A grass fire was noticed next to train rail. An area of about 150ft by 400 ft was burned by the time the fire was extinguished. No injury to persons or to property except to the area of grass. It is thought that the fire may have been started by sparks from a train engine's exhaust. This is being reported since it took slightly over 30 minutes to extinguish.
Employee reports that when walking up the screen tower he slipped on ice and his knee struck the midrail of the handrails along the walkway. Medical treatment was sought about 3 months later.
Employee was walking with sledge hammer over shoulder going back to tool room after using it to knock down ice that was hanging off a building. As he was walking his feet went out from under him and he landed on his right shoulder. The area that he was walking in had not been sanded, although surrounding areas had been.

Person was trespassing on mine property on the evening of November 14, 2012. He crashed his off road vehicle (ORV) and suffered serious head injuries. Michigan State Police investigated & indicated the cause of the crash was related to trespasser's alcohol impairment, excessive speed, and a defective throttle on the ORV. He died from his injuries on Nov 16.
Employee was shovelling. Reported a back strain. Restricted work activities on 10/19/2012.

Employee was removing copper shielding from 5kv 4-0 wire with retractable blade utility knife. The knife slipped and cut his Left thumb requiring 6 sutures.
Employee walked around train to shovel spillage, stepped in oil, slipped and fell. He reported it that day. However he did not go to the doctor until July 26, 2011. He was then prescribed physical therapy.
Stripping in east quarry, a cedar tree came off the bank onto the loader across the bucket & windshield. In an effort to remove the tree from the loader without damaging the windshield, employee climbed on top of the loader bucket. As he tried to move the tree, it moved & knocked employee to the ground. His back and head struck the ground.

Three grass fires each covering 1/2 acre or less were discovered burning along the railroad tracks. They had started from burning material exhausted from the locomotive stacks that had built up from extended periods of engine idling. The fires were extinguished within an hour of discovery. There were no injuries, illnesses, nor loss of property.
Employee removing lagging off of snub pulley on boat end shuttle when rust or dirt got into eye. Safety glasses and face shield was being used. Wind was swirling and material made it into his eye.
Replacement and cutting of structural steel on #3 Stacker caused hot slag to fall between belt and rubber lagging on drive pulleys, igniting belt. The stacker acting as a chimney caused flames to climb the belt to the upper floor igniting wooden floor boards, with slight damage to the floor.
Employee was operating Cat 777 haul truck hauling overburden stripping from the quarry south face to the stripping dump. As employee was being loaded by LeTourneau loader, a large chunk of frozen material about 6 ft long rolled against the front of the truck box and jarred employee's body suddenly while in the cab. Employee felt pain shooting from her shoulder down to her elbow.

Employee was using an 8 foot pole with a hook on the end, poking and prodding at material stuck in chute. Material broke free causing employee to jerk - causing pain in both shoulders and back.
Left arm rest in loader was too low and unable to adjust to higher position. Controlling ""joy stick"" withour support to arm caused soreness in shoulder/arm/hand. EE took next day off as ""sick day"" so it was not reported as a LTA. On 9/16/2009 we received a letter from attending physician stating probale cause for work related injury. Sick day then acknowledged as LTA.

After dumping loaded haul truck at jaw crusher, worker drove down ramp when a tie rod broke sending haul truck into a mud bank causing front end impact.
While cutting heads off of bolts with plasma cutter, EE was using left hand to secure bolt, then plasma cutter came across left hand.
Employee was helping carry a calrod heater, walking backwards between screen deck and bonnett and his foot slipped off end of plywood and went into lime burning chute.

EE was welding on a railcar I-beam. Stood up and welding helmet fell off. EE went to jump down to pick up welding helmet on the ground, striking head against corner of railcar.
While trying to put new support tube in scalper, tube got jammed up in back. When we went to free it, one end slipped down and struck top of employee's foot.
While working on 200 bonnet with co-worker, EE was burned by co-worker. Both employees were kneeling down in close proximity, when co-worker turned with a lit torch and passed near EE's left hand and wrist, causing a burn.

Employee was in the process of dumping a car at the crusher. As the employee pulled the handle, the car started to dump. A rock fell back toward the employee glancing off his head and right shoulder.
Employee was in the process of separating two railroad cars. The employee positioned himself too close to the air hose coupling. When the railcars separated the metal glad hand on the air hose of the railcar being pulled struck him in the groin.
While welding at jaw crusher, chipped slag from weld and slag flew in behind safety glasses into right eye.
On 150 Screen, working on 140 stone box - going down, slipped - striking left elbow and hip on screen deck.
