Employee was stung by an insect and had an allergic reaction. Saw a doctor and received medication to reduce swelling. No impairment from EE's daily life.

Employee was attempting to cut a rubber hose attached to the pump in the pond with a bush hook. While in the process of cutting the rubber hose, employee collapsed and fell in the boat. After falling he rolled out of the boat into the pond.

Employee fainted and became unconscious after eating lunch. He was transported to the hospital by ambulance and later released. The employee was not injured and returned to work on his next scheduled shift. The employee was not taking his medication properly.

Employee was operating a dozer that did not have a closed in cab under windy conditions. Some type of trash got around his safety glasses and got into his eye and he had to go to the eye doctor the next day and had a return visit the following day.

EE was struck in the head by another employee. He was standing in the mill office talking too another employee, when another employee entered the room and struck His in the head. He was taken to the Martin Hospital Emergency room where he recieved stitches. Jim was then transported to Nashville where he went thru sugery on 9/26/04.

EE was cleaning a bag storage area and was throwing trash into a dumpster that was about shoulder high in the top. While doing so the wind blew some of the finer material back into his face and got into his right eye.

EE WAS WORKING ON THE TRUCK LOADING PLATFORM. THIS AREA IS LESS TRAVELED THAN ANY OTHER AREA OF THE PLANT. THE STEEL H BEAMS ARE A PRIME LOCATION FOR WASPS TO BUILD NESTS. AS HE WAS PERFORM ING HIS REGULAR DUTIES IN THE AREA WHERE A NEST WAS LOCATED, HE WAS STUNG BY WASPS ON THE CHEEK & THE BACK.
