Employee was operating plant house for East Jaw crusher when he was found slumped over in his chair.
Customer truck driver suffered a massive heart attack and died while waiting in his truck to be loaded with material. NOT AN EMPLOYEE.
Driver was loading the shot from the inside the truck and the wind was blowing toward the cab when emulsion blend blew through the window going in the left eye.
EMPLOYEES NOTICED PLANT MANAGER WAS VOMITING. THEY MONITORED HIS CONDITION. HE ASSURED THEM HE WAS FINE & DIDN'T HAVE ANY OF THE "CLASSIC" SYMPTOMS OF A HEART ATTACK. HE WAS LATER OBSERVED SLUMPED OVER HIS DESK & AN EE CAUGHT HIM & LOWERED HIM TO THE FLOOR. EMERGENCY RESPONDERS WERE CALLED & HIS CONDITION WAS MONITORED UNTIL THEY ARRIVED. DEATH OCCURRED APPROXIMATELY 3:45 PM
Employee, while in pickup truck (not moving), temporarily lost speech & memory functions. Doctor reportedly diagnosed that a stroke had occurred. Degree 8 incident.
THE CASE IS STILL UNDER INVESTIGATION AND CONDITIONS CONTRIBUTING TO ACCIDENT HAS NOT BEEN DETERMINED.
